About PGIMER, Chandigarh:-

The postgraduate Institute of Medical Education and Research is a 2000
bedded tertiary care hospital which caters to five states of Northern India. I did
a three year Residency that led to the M.S. (Surgery) degree. During these
years, I underwent rotations in the following departments:General Surgery (15 moths)
Accident and Emergency Surgery (6 months)
Neurosurgery (3 months)
Plastic Surgery and Burns (3 months)
Cardiothoracic Surgery (1.5 months)
Paediatric Surgery (1.5 months)
Urology (3 months)
Orthopaedics (1.5 months)
Transplant Surgery (1.5 months)
My duties including in patients care, attendance at two patient clinics per
week, assisting at operations and operating under supervision. I obtained a great
deal of experience in managing surgical emergencies, including multisystem
trauma. I also took part in clinical research and teaching other residents.
Training and Experience in urology at PGIMER:Senior Resident (Registrar) Urology as a M.Ch. Resident at Postgraduate
Institute of Medical Education and Research (PGIMER), Chandigarh.
This is a premier Institute for Medical Education and research in Northern
India and offers post-graduate and superspecialty training in almost all
specialties, both paraclinical and clinical. The associated Nehru Hospital is a
2000-bedded tertiary care hospital of north India offering advanced state-of-theart treatment facilities. It caters to a large population of Northern and Eastern
India. Department of Urology has a strength of 64 beds including 12 beds to
emergency admissions. In addition, at all times, 15-20 patients are admitted in
the private wards.
Supervisors: Prof. A. K. Mandal, Dr. S.K. Singh, Dr. Santosh Kumar, Dr.
Mayank M. Agarwal. In PGIMER, the course in urology has been partially,
rather than fully compartmentalized, so as to maximize exposure to one
subspecialty at one time, at the same time maintaining good exposure to others
at all times.
Postings during training:  Emergency
During this period I became proficient in rapid but through evaluation of
and appropriate management (both operative and non-operative) of acutely ill
patients with urological pathologies, including urinary colic, urosepsis,
hematuria, acute renal failure and trauma, to name a few. I performed many
emergency operations, both open surgical and endourological, both under
supervision and independently.
 Uro-Radiology and minor procedures
This included both diagnostic as well as therapeutic uroradiology both in
elective and emergency situations. I have performed diagnostic procedures (as
well as managed the complication of) including retrograde urethrograms,
cystograms, nephrostograms, fistulograms, retrograde ureterograms,
pouchograms (of bowel urinary reconstructs), ultrasonography of KUB and
Pelvis region, female pelvic examinations and therapeutic procedures including
percutaneous nephrostomies (ultrasound and fluoroscopic guided), endoscopic
ureteral stenting, precutaneous suprapubic cystostomies, cystopanendoscopies,
urethral dilatations, hydrocoelectomies, varicocelectomy, circumscisions,
intravesical BCG instillation, urodynamics (uroflowmetry, pressure flow
studies and urethral pressure profilometr y) and many other in adequate
numbers so that now I am confident in performing and have been teaching these
to my junior fellows.
 Endourology and extracorporeal lithotripsy
I learnt diagnosis, workup, management, pr eoperative care and follow up
of urological conditions, e.g. hematuria, urothelial malignancies, urolithiasis,
benign and malignant prostate diseases, obstructive uropathy and stricture
urethr a, to name some. Some of the endourological procedures that I am
confident in performing are OIU, TURP, TURBT, URS, ureteral stenting, PCN,
PNL etc. apart from using endourologic instruments, I also learnt the proper
handling and proper sterilization methods.
I learnt technical aspects of the lithotriptor (Siemens lithostar multiline)
and became proficient in performing extracorporeal lithotripsy. I also became
well versed with the complications related to ESWL and their management.
 Uro-Oncology
I had the opportunity to diagnose and manage most of the urologic
malignancies. Also, treatment planning with other department, including
radiation onclology, general surgery, nephrology, nuclear medicine, radio
diagnosis, pathology, plastic surgery and neurosurgery, helped me develop a
comprehensive and wholesome attitude towards the problem. Now, I have
become proficient in the management of urothelial, prostate, testicular, renal
and penile malignancies. I have assisted and performed enough numbers of
TURBTs, Radical nephrectomies, radical cystectomies and urinary diversion
(ileal conduit, orthotopic neobladder and continent cathetersable stomas),
nephroureterectomies, partial cystectomies and nephron-sparing.
 Reconstructive Urology
I learned the basic techniques and proper tissue handling required in
reconstructive urology and now I am confident in performing pyeloplasty,
ureteroureterostomy, ureteric reimplantation, vasoepididymal anastomosis,
repair of vesicovaginal fistula, progressive perineal urethroplasty, stage I and II
urethroplasty.
Apart from that, I have assisted in a number of cases of orthotopic
neobladder construcution, continent cutaneous urinary diversion, difficult
urethroplasty (including progressive perineal, transpubic and buccal mucosa
substitution), hypospadias repair, etc.
 Andrology
I learnt the work up of erectile dysfunction and infertility, both obstructive
and non-obstructive. I have performed procedures like varicocoelectomies,
epididymectomies and vaso-epididymal anastomoses confidently.
I have been a part of organizing-committee for a live work-shop bon
artificial urinary sphincters and penile prosthesis, held in March 2010 in
PGIMER, Demonstrator: Dr. Drogo K. Montague, MD.
 Female Urology
I have been exposed to female pelvic sur gery as an assistant in simple as
well as complex vesicovaginal fistula repairs (both trans abdominal and
transvaginal) including urethra-vesicovaginal and ureterovaginal fistula-repairs,
bladder-neck closure, anterior exenteration with orthotopic neobladder, various
urethral sling surgeries under supervision of Dr. Mayank M. Agarwal.
 Laparoscopic Urological Surgery
Laparoscopic nephrectomy, Laparoscopic radical nephrectomy,
Laparoscopic adrenalectomy, Laparoscopic ureterolithotomy, Laparoscopic
pyeloplasty.
 Administrative work
A formal four month rotation was related to administrative work the socalled‘Chief-Residency’during which I managed the admissions, operationlists patient-management in the ward (preoperative preparation and
postoperative management) and discharges. The duties included, apart from
above, to coordinate the lists of various consultants, to maintain suitable
exposure of all the residents (both in operative and ward work), to make the
academic and duty rosters for both, the consultants and the residents, to takepart in organizing work-shops; i.e. to ensure overall smooth functioning in the
department. Apart from this bit I have had administrative experience as a part of
organizing committee for the various workshop held in PGIMER, Chandigarh,
during 2009-12.
Overall, the duties assigned to me included attending OPD and OT (both
elective and emergency), taking care of ward patients including scientific
workup at individual level. Academic activities included presenting journal
clubs and seminars, interdepartmental presentations (with nephrology, nuclear
medicine, radiation oncology, radio diagnosis and pathology), and teaching
postgraduates.
Special training : Observership at Memorial Sloan-Kettering Cancer Centre, New York, USA
under sponsorship of Dr. Jaspreet Singh Sandhu .
 Observership at Weill Cornell Medical College and New York-Presbyterian
Hospital, New York, USA , under guidance of Dr. Ashutosh Tewari, for
