May 23, 2007 ACKNOWLEDGEMENT SHEET For General Contractors & Homeowners Thank you for choosing Northman Marble & Granite. We appreciate your business. To make this project run as smoothly as possible and to ensure your utmost satisfaction, we would like to inform you of a few important aspects of the countertop process. Please take a moment to read the provided information and initial next to each one, indicating that you understand and accept all things needed to complete your project. ___ Date of template: An agreed upon, responsible party MUST be on site at the time of template to ensure complete accuracy, to express needs and wants, and to answer all relevant questions. Undermount sinks must be on site at time of template to be taken back to our shop. Once this is received or completed our Project Coordinator or Designee will call to set up your install date. The templater has no information regarding installs. Please do not ask him. ___ 1.) 2.) 3.) 4.) 5.) 6.) Date of install: Your project install will not be given a date until the following things are completed and/or received in our office: A signed acknowledgement sheet. A 50% deposit check. A completed template that has been reviewed, approved & signed by you. An order acknowledgement for the slabs you have selected and signed off on from the supplier. If applicable - A Change Order form has been completed, reviewed, approved AND signed by you. Stone is ordered and has arrived at Northman Marble & Granite. ___ Plumbing and electrical: We do not disconnect sinks, dishwasher, disposals or cook tops during the installation of countertops & backsplashes. When necessary, electrical outlets may need to be relocated or pulled from the walls and drywall may need to be repaired. This must be done prior to install and is not a service that we can provide. Nor do we disconnect gas appliances, reinstall or connect any appliances, plumbing or electric. These items will not be done by Northman Marble under any circumstances. Please contact a licensed plumber or electrician for these services. ___ Appliances: It is your responsibility to ensure properly fitting appliances. This includes all ranges, refrigerators and dishwashers. Please be careful when purchasing new appliances after countertops are in production. It is recommended that you purchase them and have them installed or on site before a template is done. All cook top cutouts, top mount sink cutouts, and holes for faucets/accessories must be done during installation to ensure proper fit and to avoid breakage during transport. Unfortunately, this task can be messy and create dust, in small or very large quantities. Please remove any and all objects out of such areas if you want them to stay clean. Top mount sinks and slide in ranges must be on site at time of install. Farm house sinks must be installed before the template. Under mount sinks holes will be pre-cut in our shop. The cut out shape, size, setback, orientation and edge profile will be at the discretion of our shop. Sink cutouts will be centered from left to right in the cabinet unless otherwise specified in writing by the customer. The sink will be secured under the granite and ready for the plumber to complete the hookup. All faucets or accessories must be on site at time of install, as well as homeowner or designer who can sign and mark for the holes to be drilled. -1- May 23, 2007 Please note: If appliances do not fit properly once your countertops are installed and it is due to a mistake on your part, or appliances are not on site at time of install – a trip charge of $150.00 as well as any additional labor charges will be added to your bill if we are needed to return to your home to correct the problem(s). Additional charges will be written on a Change Order form which must be signed before we can move forward. Please remember - we do not reinstall any appliances. You will need to have your electrician mount any hardware or reconnect any and all ranges. You will also need your plumber to reconnect your dishwasher, disposal, and top mount sink, which will also need to be caulked. ___ Sub Decking: In your price quote we account for sub decking if applicable, which we will install when we install your new tops. It is used in conjunction with 2CM stone and may not be required with 3CM stones. We do not recommend or warranty any countertop installation if the sub decking was installed by others. ___ Tear out of existing countertops: Due to creating potential damage to cabinets or walls, Northman Marble & Granite will not remove your existing countertops, though they will need to be properly removed before the template in most cases. We are sometimes able to template your existing countertops to minimize the burden of a non-functional kitchen; unfortunately, we are unable to accurately verify the existing conditions of your cabinets, and as a consequence, problems may arise and additional work may be required that is not in our scope of work. We strongly suggest that you remove everything from the base cabinets as well as existing backsplashes. During the tear out of backsplashes, the drywall may become too damaged to use as a proper substrate for your new splashes, so be prepared to replace the drywall at your own expense. Your cabinets must be set level, straight and true, and conform to industry guidelines. Unfortunately, this is not always the case, especially in older cabinets. We may encounter leveling or structural problems. Solutions may involve the re-setting of your cabinet boxes or the installation of wood trim to cover any gaps or exposed plywood. Our installer will inform you of any problems after tearout but before installation so you may choose whether to proceed with subpar substrates or to have them repaired at your own expense and pay a trip charge for our installers to return at a later time. If you do proceed with subpar subtrates, any trim pieces or other cosmetic changes are at your own expense as well. If these solutions are unacceptable to you, you should have your existing countertops, sub decking and backsplashes removed prior to template. If the templater sees any potential problems he will inform you and advise you on the course of action you should take. If you fail to make the proper corrections we will not be responsible for any damages caused, and the installation may be delayed as a result. Additional charges will be applied based on homeowner’s negligence. We will remove all debris from site except for the sink, which is the plumber’s responsibility. ___ Backsplashes: You need to specify if you want backsplashes and what size. Standard backsplash is 4’’ high with a standard square polish on the exposed edges. Full height backsplash will terminate at the end of the upper cabinet and have a 6’’ high side splash unless specified and noted in writing and on contract. ___ Joints, Stone & Overhangs: Marble, Travertine, Granite and other natural stones are created by nature, and as such are subject to grain & color variation. All stone has varying degrees of inconsistencies such as pits and fissures, and are inherent characteristics of natural stone. The location of countertop or backsplash joints is dependant on factors such as slab size, type of material, weight of pieces, and shape of your kitchen. During the layout process we consider the grain direction and pattern of the slabs and place -2- May 23, 2007 the joints to achieve the best possible results. All seams will be placed at the discretion of our shop and may change at any time. However, if you want the grain/pattern to go in any certain direction, it must be noted, written and signed by you before or at template and before production begins. Otherwise, it will be fabricated and installed at our discretion and if, at that time, you are dissatisfied with the direction and/or pattern, the cost to redo the countertops will be at your expense. NO Excuses or exceptions. If the mistake is made on our part, we will remake and replace your countertops at our expense. Please discuss all overhangs, corners, and edges with your sales associate and templater. Some overhangs may require corbels or steel supports. We do not furnish or install either. ___ Countertop Care & Maintenance: Your countertops are sealed during fabrication and/or after installation which may result in a dull appearance. This will disappear after a few weeks of use and cleaning. Natural stone requires little or no maintenance and should remain just as beautiful as the day it was installed. To clean your countertops, use a mild soap and some water. Never use abrasives or harsh cleaners such as Soft Scrub, Comet, 409, Fantastic or anything alcohol based, as they can leave a buildup of residue. Never wax your countertops. Granite and other stones are not indestructible; treat your tops as you would any other valuables. It is not recommended that you sit or stand on your countertops. Also, treat edges with more care, as they are thinner and may be more fragile. ___ Completion: Upon job completion you will be asked to sign off that you are satisfied with your new countertops and, if necessary, address any problems that you may have. We will address them individually with you to determine if any correction can or should be made. If you or a designee are not on site to sign and approve the completion of the task, by default, you waive the right to personally inspect and approve the work performed, therefore, this signed form will be used in lieu of the completion form thus certifying that Northman Marble and Granite has satisfactorily completed the installation as set forth in your sales contract. ___ Special Conditions: Elevators, flights of stairs (interior or exterior), narrow doorways, long distance carries, and limited access to areas of the job site may result in additional charges which must be disclosed and included in contract. If it is not and is noted on day of install, installation will be ceased until a Change Order form with the additional fee is signed. If homeowner is not available to sign and installers are unable to complete the job, additional charges will incur for the delay and disruption in productivity. ___ Warranty: Northman Marble & Granite give a standard 2yr workmanship warranty required by law. Materials such as natural stone are products of nature, and have no warranty from future defects. However, in the event a defect should occur, at our discretion, we may consider repair of surfaces. Our decision to repair does not imply we accept or take responsibility for the defect nor will we provide future warranty of the repair. There is no warranty for customer negligence, destructions, misuse or neglect. ___ Other Notes: All leftover stone is purchased by Northman and is the property of Northman. No other written or verbal agreements about stone possession or transference will be honored. Your signature indicates that you have thoroughly read, understand, and accept all conditions, provisions and guidelines in this document in its entirety.
