Forward translation by two independent translators, who then sat
together and agreed on a common first forward translation. This first
forward translation was then translated back to English by two
additional translators. After reviewing of the back translations by the
DLQI authors, the wording and grammar of the first forward translation
was changed by two independent translators, who then sat together
and agreed on a common second forward translation. This second
forward translation was again back translated independently by two
translators, and then reviewed by the DLQI authors. Minor changes to
the second forward translation were suggested by the DLQI authors,
and performed by one translators, yielding the final translated
DLQI/CDLQI

Cognitive debriefing
Cognitive debriefing was performed on July 18th and 19th 2012. In total 5
adults (DLQI) aged between 25 and 53, and 5 children aged between 10
and 15 (CDLQI) with active Buruli Ulcer were included. The
questionnaire was administered to all participants by an interviewer.
The average time of it took to administer the questionnaire was 7
minutes. All questions were understood by the participants, except for
one participant who was from another region and was not fluent in the
local language. In addition, all participants were asked whether they
would prefer the questions to be asked in English, and whether they
would be able to complete the questionnaire themselves, without an
interviewer. All participants answered no to both questions. Total
scores ranged from 0 to 27, with an average of 12.5.
