POLICY:
It is the policy of Wheaton Franciscan Healthcare – All Saints (WFH-All Saints) to
maintain consistency with applicable state and federal laws. The Wisconsin Department
of Health and Human Services (DHSS) requires associates to be identified. The photo ID
badge serves to identify each associate to patients, visitors, and other associates. The
badge will also be used for access to the WFH-All Saints facilities as well as recording
work time under the computerized payroll system.
RATIONALE:
Our Values of Respect, Integrity, and Excellence call us to comply with state guidelines
and identify each associate to patients, visitors, and other associates to provide
exceptional and compassionate healthcare service.
SCOPE:
This policy applies to all departments at WFH–All Saints facilities.
PROCEDURE:
Associates are not permitted to wear any type of pin or button containing printed material
except their hospital photo ID badge and professional pin(s). Associates are not to cover
any part of their ID badge with tape and/or stickers.
Effective: March 21, 2007
All Saints will provide associates with one photo ID badge under the following
circumstances:
1. New hire
2. Associate position change (position is defined as an official system title/position)
3. Associate name change
4. Replacement for broken or defective badge
In the event of a lost badge, the associate will pay the cost of $6.00 for reprinting; with
permission this cost may be deducted from the associate’s paycheck.
Photo ID badges are to be returned to the associate’s manager or the Human Resources
department upon termination of employment.
Persons failing to comply with the Associate Identification policy will be subject to
disciplinary action, up to and including discharge. The Human Resources department is
responsible for the production and distribution of the badges based on the following
specifications:
A. Supervisory and Management Staff

B. All Other Associates
Line 1: First Name
Line 2: First Initial of Last Name
Line 3: Job Title
C. Job titles and department names are those officially defined by the Human Resources
department and All Saints Management.

Certifications, degrees or other designations are not allowed on the badge.
Licensures that are allowed on the badge are MD, DO, RN, NP and PA, as these are
required by state licensing requirements.
E. Routine replacements will be issued upon the discretion of the Human Resources
department. No cost will be charged to the associate for this purpose. Badges are to
be picked up in Human Resources by the associate in order to exchange the
outdated badge for the current one.
F. Lost or stolen badges need to be reported to Security immediately. A new card will
be issued within 48 hours. The badge needs to be picked up in Human Resources by
the associate or manager.
G. Distribution of badges for new hires will be done by the Human Resources
Department.
H. Concerns or questions regarding the usage of the badge, reporting a defective badge
or communicating an error should be addressed to Human Resources.
I.
Requests for additional security access necessary to perform job duties must be
made by an associate’s manager to the Manager of Security.
