Dear Provider:
Anthem Behavioral Health will begin to manage the behavioral health benefits for Nevada HMO and
PPO members as of January 1, 2008. (These members have an alpha prefix that begins with YF.)
I am writing to communicate the services we can offer to you as a provider, and to let you know how
to make the transition process go smoothly for any members you are currently treating.
Our Service to You
Anthem Behavioral Health requires no authorization for outpatient services for PPO members.
If you are currently providing outpatient services for a PPO member, you do not need to obtain an
authorization. You may simply submit your claim for payment.
HMO members do require preauthorization for outpatient services. The transition authorization
process is outlined below, but for ongoing outpatient authorizations beyond the transition period, you
may submit a request in one of the following ways:
 Through electronic submission of an Outpatient Treatment Report (OTR) through our website:
www.anthem.com or
 Through written submission of an OTR by fax or mail
An OTR form is included in this mailing and you may also download it from our website at
www.anthem.com.
Anthem Behavioral Health will also manage the intensive service benefits for Nevada HMO and PPO
members. All intensive services, such as acute inpatient, partial hospitalization, and inpatient
detoxification require preauthorization. In addition, we assist in establishing follow-up appointments
for any members who are admitted to intensive services. If you have a member who may require
more intensive services, please contact us by calling (866) 621-0043. In addition, we have clinical
staff available by phone to assist in making appropriate referrals and to take emergency calls as
needed.
Transition Process from Human Behavior Institute (HBI) to Anthem Behavioral Health
Anthem PPO: If you are currently providing outpatient services to an Anthem Nevada PPO member,
no outpatient authorization is required.
Anthem HMO: If you are currently providing outpatient services to an Anthem Nevada HMO
member, please note that you may receive 6 outpatient sessions for dates of service beginning
January 1, 2008 in order to allow for a smooth transition. You may request these sessions by
completing the following:


Complete the attached form “Request for Transition Authorizations” and mail or fax the form as
indicated.
You may submit the request in December 2007 for dates of service beginning
January 1, 2008.

Six sessions will be authorized beginning January 1, 2008.
If you have more than one active Anthem Nevada member, the “Request for Transition
Authorizations” form allows for you to make multiple requests on one form.
If you need authorization for services in December 2007, please contact Human Behavior
Institute (HBI). They will continue to manage services through 12/31/07.
If you have any questions about continuity of care for services beginning January 1, 2008,
please call Anthem Behavioral Health at (866) 621-0043.
Anthem FEP
FEP members require preauthorization for all behavioral health services. The transition authorization
process is outlined below, but for ongoing outpatient authorizations beyond the transition period, you
may submit a request by faxing the Outpatient Treatment Form to Toll free: 866-793-0469.
If you are currently providing outpatient services to an Anthem FEP Nevada member, please note that
you may receive 8 outpatient sessions for dates of service beginning January 1, 2008 in order to allow
for a smooth transition. You may request these sessions by completing the following:
 Complete the attached form Outpatient Treatment Form and mail or fax the form as indicated
 You may submit the request in December 2007 for dates of service beginning
January 1, 2008.
 Eight sessions will be authorized beginning January 1, 2008.
 If you need authorization for services in December 2007, please contact Human Behavior
Institute (HBI). They will continue to manage services through 12/31/07.
 If you have any questions about continuity of care for services beginning January 1, 2008,
please call Anthem FEP Behavioral Health at 800-424-4011.
Please note: Anthem’s behavioral health operations will not manage behavioral health and chemical
dependency rehabilitation services for BlueCard® and national account members. For those
members, please refer to the behavioral health/substance abuse phone number on the back of the
member’s health plan ID card.
