The Ohio State University Mansfield --- Office for Disability Services STUDENT TEST ACCOMMODATIONS POLICIES & CONTRACT This contract is to ensure that academic integrity standards are maintained at the Office for Disability Services (ODS) on the OSU Mansfield campus and to ensure that all students authorized for the use of test accommodations are aware of these important guidelines when taking tests at the Conard Learning Center Math Lab. (“Test” is used here to mean quizzes, tests, or midterm exams. “Final” is used to mean final exam. “Math Lab” hereafter refers to Conard Learning Center Math Lab.) Please read each guideline below to signify that the policy has been explained to you by ODS. By signing this document, you confirm that you understand the guidelines and are agreeing to abide by the guidelines. 1. The student is responsible for taking a Test Request Form to his or her instructor(s) three (3) school business days in advance of each test to be taken at the Math Lab. (School business days are week days that classes are in session.). 2. Finals to be taken at the Math Lab need to be scheduled no less than five (5) school business days in advance of finals. 3. The student is responsible for returning the completed, signed Test Request Form to the Math Lab front desk. The Conard Office Associate will note the date the Test Request Form was received. 4. Incomplete forms will be returned to the student. Test proctoring at Conard will be denied until a completed Test Request Form is received by Conard staff. ODS must have all the information on the form prior to tests taken at the Math Lab. 5. The student is expected to arrive on time for his or her scheduled test time at the Math Lab. If the student arrives late, he or she must use the remaining time to complete the test. No additional time will be allowed for the test. 6. If the student scheduled to take a test at Math Lab, but fails to show up, the student and his or her instructor will be notified of the missed appointment via e-mail within 24 hours. 7. If a student should need to reschedule a test in the Math Lab for any reason, he or she should: First, go to the instructor to determine a new test date and time. Notify ODS as far in advance as possible of the new test date and time. The student should copy (cc) his or her instructor on this e-mail. 8. While ODS will honor students’ test accommodation needs, a specific test room cannot be guaranteed students. Students are not guaranteed a room by themselves. 9. There will be blocks of time set aside each day for the proctoring of tests. Tests will be proctored at the Math Lab during these times. (Note that these test proctoring times do not apply to final exams week.) Summer tests will be administered only by appointment with ODS. 10. Only items and computer applications that are authorized by the instructor on the Test Request Form will be permitted for the student’s use in the test space. All other items must be stored in your book bag and left in an ODS designated space outside of the test room. Electronic and communication devices must be turned off. 11. ODS test rooms are monitored by ODS staff. Students’ observed use of unauthorized items and/or computer applications during a test will be reported to the instructor and may be reported to the OSU Committee on Academic Misconduct at the instructor’s discretion. 12. A student may not leave the test site while his or her test is in progress or the test will be terminated. The only exception is a bathroom break. In the case of a bathroom break, the student’s departure and return times will be noted. ODS requests that students use the bathroom facilities before they begin their tests in the Math Lab. 13. Test Readers will read test questions exactly as they are written. No additional information about the test question will be provided unless this has been pre-approved and specified by the instructor. 14. Students may not upload, download, rip, or save information directly to ODS test room computers. Students must ask for assistance before changing ODS test room computers’ display.
