This post is a Fixed Term/Secondment Opportunity for a period of 2 years.
The salary for the post is based on the Band 7 salary scale which is currently £30,460 to
£40,157 per annum (pro rata for part time posts)
The hours for the post are 37.5 per week, consideration will also be given to part time hours.

GENERAL CONDITIONS:
Conditions of Service - The conditions of service are those laid down and amended from
time to time by the appropriate negotiating bodies e.g. NHS Staff Council/NHS Pay
Negotiating Council.
Annual Leave – The leave year runs from 1 April to 31 March. Annual leave entitlement
for full time staff on appointment is 202.5 hours (27 days), with 217.5 hours (29 days)
upon completion of 5 years NHS service and 247.50 hours (33 days) after 10 years NHS
service. Entitlements for part time will be pro rata.
Public Holidays – Public holiday entitlement for full time staff is 60 hours based on the 8
designated Public Holidays per annum, and on a 5 day week working 7.5 hours per day.
The 60 hours entitlement is due to all whole time staff irrespective of work pattern. For
part time staff, this will be applied on a pro rata basis and also allocated in hours.
KSF - The NHS Knowledge and Skills Framework is essentially a development tool but
will also contribute to decisions about pay progression. It will promote equality for and

Superannuation - Employees are automatically opted into the NHS (Scotland)

Policy Relating to Staff with Bloodborne Virus Infection - Candidates for posts which
involve exposure prone procedures will not be allowed to commence employment until

candidate is not able to perform EPPs, Occupational Health will notify the appropriate
manager that the candidate is ‘fit with restrictions’ and is unable to perform EPPs. The
health status of the individual will not normally be notified to management. Where the
restrictions recommended by Occupational Health are unable to be accommodated due
to the clinical duties of any of the posts within the staff groups specified in the policy, an

Situated in attractive south-west Scotland on the Firth of Clyde coastline is the glorious

2. JOB PURPOSE

To provide highly specialised advice in all pharmaceutical matters specific to older patients in the
community, as a dedicated resource relating to the care of individual patients within Care Homes

To provide a highly specialist clinical pharmacy service to patients within the Community and in
particular the Care Homes within NHS A&A.

To work closely with care home staff, GPs, community pharmacists and other healthcare staff in
supporting the development and application of high quality, cost effective evidence based
prescribing

To link with other health and care professionals, including the patient’s usual community
pharmacist and their GP, and to attend the patient’s Multidisciplinary Team Meetings (MDT)
(where possible) and by linking with Hubs and Community/Virtual Wards, Integrated Care and
Emergency Service (ICES) and LOTS, and hospital.

To contribute to the wider development and improvement of pharmaceutical services

To be accountable for the safe and cost effective use of medicines within the areas of practice.
DIMENSIONS

Responsible for the day to day delivery of clinical pharmacy services to older patients within Care
homes.

Attends case conferences and MDT meetings where appropriate, providing pharmaceutical
advice and training to the MDT members.

In addition to the specific responsibilities outlined in this job description, the post holder will be
expected to fulfil all of the basic pharmacist competencies required as part of registration with the
General Pharmaceutical Council. The post holder will be required to act at all times in
accordance with medicines and Health & Safety legislation (e.g. Medicines Act 1968, Misuse of
Drugs Act 1971, Poisons Act 1972, Control of Substances Hazardous to Health Regulations
2002) and professional standards as outlined by the General Pharmaceutical Council.

5. ROLE OF THE PHARMACY SERVICE
The function of the pharmacy service is to develop and deliver integrated patient focused pharmaceutical
care, which meets the present and anticipated needs of the population of NHS Ayrshire & Arran in
accordance with the Local Health Plan and national strategies. The intention of the service is to link all
branches of the profession in order to better co-ordinate pharmaceutical care for patients and members
of the public.
The focus is on active participation in and contribution to multi disciplinary, multi
professional and multi agency teams in a fully integrated manner. The main responsibilities are :








The development and operational delivery (managed services) of pharmaceutical care across
NHS Ayrshire & Arran
The provision of expert advice on pharmaceutical matters to the NHS Board, Healthcare
Directorates, senior managers and prescribers
The provision of support regarding the development of Community Pharmacy
The implementation of robust clinical, corporate and staff governance systems to minimise risk
and assure patient and staff safety and well being
The promotion of safe and cost effective prescribing taking into account the clinical needs of
individual patients
The integration of services based on patients and integrated care pathways not boundaries
The provision of support for the work of the Drug & Therapeutics Committees including the
provision of educational support to a wide audience
Ensuring that all aspects of the managed service comply with all statutory and quality standards .
6. KEY RESULT AREAS
Main Duties and Responsibilities
Planning and Organisation
 To deliver, develop and evaluate a clinical pharmacy service to older patients within Care homes
in line with local strategies, policies and procedures to ensure optimum patient care.

To provide highly specialised advice regarding the clinical use of medicines for patients within the
Care homes.

To prioritise service delivery on a day to day basis, taking into account the relative clinical risks
associated with patients and within the resources allocated.

To provide training to care home staff on medicine management within the Care homes.

To establish links with Care of the Elderly pharmacists working within the Hubs,
Virtual/Community Wards, ICES and LOTS to ensure consistency of service provision across the
Health Board

To liaise with other healthcare professionals including community pharmacists, care homes staff,
GPs, dietician, nurses and other hospital colleagues across the single system to promote and
implement seamless care.

To, implement and monitor Care of the Elderly policies and procedures in conjunction with senior
medical staff, senior nursing staff and other health care professionals to ensure evidence based
and safe working practice, complying with all standards and legislation relating to care of the
elderly in accordance with agreed objectives.

To, participate, monitor, collate and provide to the agreed measures for the provision of specialist
clinical pharmaceutical input, and to provide reports as requested, so that the service is able to be
monitored.
Resource Management



To implement policies and procedures for monitoring the use of medicines, identifying areas for
cost improvement, promoting formulary management and good prescribing practice.
To ensure the security of medicinal products in all settings within the care homes.
To contribute to the implementation of the managed entry of new medicines according to agreed
policy
Clinical Practice
 To undertake a daily clinical commitment by providing highly specialised clinical pharmacy input
to the Care homes. Participating in multidisciplinary team meetings to ensure that the medicines
prescribed are as clinically and cost effective and safe as possible for the individual patient i.e.
reviewing medication, identifying, resolving and preventing medicine related problems, devising,
recording and monitoring care plans and maintaining multidisciplinary notes for each patient,
assessing safety / ability to self-administer and comply with medication, interpreting blood serum
results of individual drugs/patients that require specific monitoring, advising patients, carers and
other social and healthcare staff on all aspects of medicines.
 To report medication errors and participate in investigations as appropriate, to ensure that the
risks associated with medicines are minimised.
 To improve pharmaceutical public health by identifying and preventing adverse drug reactions.
Reporting suspected adverse reactions on medicines to the Medicines and Healthcare Devices

Education and Training
 To identify and address any training needs to improve medicines management within the service
to support the wider education of health and social care staff. within the care Homes. To
facilitate peer review meetings, sharing good practice by utilising professional and educational

understanding and optimise concordance.
 To contribute to local bulletins on prescribing and pharmaceutical matters

 To initiate, undertake, or guide others, in audit within their specified clinical speciality in line with
the national standards and frameworks in order to maintain standards and develop the service
within their own area.
 To implement all local policies and procedures in accordance with corporate, clinical and staff
governance
 To document and report on work in Care homes via a standard reporting template to demonstrate
effective pharmaceutical support.
7. EQUIPMENT AND MACHINERY AND SYSTEMS





Regular use of the NHS Ayrshire and Arran Intranet to access laboratory reports for individual
patients.
Regular use of Microsoft Office computer software for email and internet enquiries, Word
processing of reports and policies, producing spreadsheets of management information and
statistics and PowerPoint for presentations to staff.
Use of social work information system to obtain information on a patient’s current health and
social care involvement where access granted.

8. ASSIGNMENT AND REVIEW OF WORK
The post-holder is line managed by the Lead Pharmacist Medicines Utilsation & Education
and managed by the Lead Pharmacist Medicines Utilsation & Education with support from the Primary
Care Pharmacy team. This is an interim situation while the Care of the Elderly Service within pharmacy is
under review.
 Works autonomously against objectives agreed with the Lead Pharmacist Medicines Utilsation &
Education
 Workload will be self-generated and via referrals, influenced by patient needs, service needs, the
local clinical pharmacy strategy and the development of the Reshaping care of the Elderly
Strategy.
 The post holder will plan and prioritise own workload to meet patient needs.
 The postholder is accountable for their own professional actions and outcomes – guided by
legislation, local protocols, procedures and the local formulary.
 The postholder is accountable for personal decisions taken regarding individuals patients in the
course of managing their care.

The Lead Pharmacist Medicines Utilisation & Education carries out formal performance review
meetings for this post.

The job description is subject to review by mutual agreement between management and the
postholder.
9. DECISIONS AND JUDGEMENTS

The post holder will be expected to exercise their own discretion on how to provide and develop
the clinical pharmacy service to their specified area taking account of local and national policies.

The post holder is expected to anticipate problems/needs and resolve these in a proactive
independent manner
Decisions made may be complex, e.g. the calculation of doses based on surface area, the use of
unlicensed medicines for individual patients, where there may be limited or only anecdotal
information on which to base a decision to treat.
The post holder is professionally accountable for his/her own actions in advising and influencing
nurses, medical staff and other healthcare professionals regarding the treatment of individual
patients on a daily basis e.g. monitoring and adjusting doses for drugs with a narrow therapeutic
index or in patients with renal impairment; recommending discontinuation of inappropriate
medication or initiation of new therapies.



 Dealing with unpredictable patterns of work, often working to tight timescales, with frequent
interruptions to ensure that each patient’s medication regimen is safe and appropriate for its
intended purpose, where there is sometimes limited information to guide decisions.

The post holder is expected to communicate on a daily basis with medical staff, nursing staff and
other healthcare staff using evaluated information to enable informed decisions about patient’s
therapies
Negotiating and influencing skills are required when dealing with multi-professional teams where
agreement and co-operation are required in order to achieve a satisfactory outcome
Training, presentation and mentoring skills at local level, are necessary to fulfil the role of
developing and supporting staff in an often stressful and changing environment.
Information received and communicated will often be complex, including the need to interpret and
analyse drugs charts, patient information and clinical trial data in a rapidly changing research
environment.
Regular contact with General Practitioners, community pharmacists, practice, district and
dementia care nurses and other health and social care workers in the community for the


seamless pharmaceutical care of patients.
There will be daily contact with patients as individuals or in groups, regarding information about
their medication, to be delivered in a manner that is easily comprehensible to those who may
have a limited knowledge of the subject matter.
Regular contact with peers, including the Pharmacy Primary Care Team, the Care of the elderly
Integrated Care Pharmacists and the acute hospital teams.

PHYSICAL EFFORT / SKILLS:
 Computer keyboard skills used daily.

 Standing for periods of time on a daily basis while participating in the multidisciplinary ward
rounds.
