Quiz Form directions
You will be creating a two (2) page website in Dreamweaver with a study guide and working
quiz on any topic of your choosing.
Before you begin creating the site, it’s recommended that you design the look of the site first,
either on paper or using image creation software.
Requirements:
 Create a new folder and name it “LastName-Quiz Form”
o create another folder inside this one and name it “images”
 Create a new site in Dreamweaver and assign it to your project folder
 You will be creating a 2-page site.
o The main page: named index.htm
o Quiz page: named quiz.htm
o Stylesheet: named styles.css
The design elements on your website must include the following:

Layout will be designed with extensive use of Dreamweaver tables.
o Each page will consist of a minimum of one (1) column and three (3) rows.
o The table should be centered and have a fixed width set in pixels
o The background color of the site should be different than any background color
used in the table.
o The first row will contain a graphic header image. (same for each page)
o The second row will contain an image based-navigation. (details below)
o The third row will contain the page content.
o Design decisions you should give some thought:
 nested tables
 page margins
 background colors for each row
 table border visibility
 cell padding
Image-based navigation guidelines:
o Two (2) graphical links (rollover images) for navigation
 Study Guide – link to index.htm
 Quiz – link to quiz.htm
o Page navigation must be easy, intuitive, and identical
Most visual styles should be created using the external stylesheet.
Design the site for a 1024x768 browser
All images (GIF, JPG, or PNG) must be optimized for quick load time (but still retain
good visual quality)
All colors must be in harmony with the overall page design
Study Guide page (index.htm)


Must contain header text saying Study Guide. Size of header is up to you.
Must contain enough information to allow someone to answer all quiz questions
correctly. Can be in paragraph or list format.
Quiz page (quiz.htm)


Must contain header text saying Quiz. Size of header text is up to you.
Must include a Form meeting the following requirements:
o A text field to input the quiz-taker’s name
o Five (5) multiple choice questions using radio buttons
o A minimum of three (3) answers per question
o Pop-ups indicating correct and incorrect answers when selecting radio
buttons
o A Reset button to reset the form to default state
