The following Player and Parent/Guardian or Carer by completing and signing of this form are considered to be Registered Members of the North Wilts
Football League and as such will be obliged to conform to the rules and regulations of this League for the duration of the time that the player remains a
Registered Member.
The Player whose name and signature appears on this form shall only be required to complete one Membership Form with the NWYFL, which will be
registered with the League once only and the League will provide upon receipt of this form a unique Player Number. The Player will remain on record as a
Registered Member until such time as his/her registration ceases by whichever means.

A Club Official, who will be held responsible for checking the accuracy of the information given, MUST witness the player's signature.
A player CANNOT play in any League or Cup match and will not be considered as registered until the League Registration Secretary is in receipt of this form,
which will be retained by the League.
PLEASE REFER TO LEAGUE RULE 8(B) TO DETERMINE THE DATE OF THE FIRST GAME A PLAYER IS ELIGIBLE TO TAKE PART IN FOLLOWING REGISTRATION
Clubs are required to keep a copy of this form if the Club does not use its own Registration Form and ensure that the information on it should
be available at all times in case of an emergency - in accordance with Football Association Rule 8B.

this form and that I am now considered to be a Member of the Club to which my child is registered for in the current season.

Personal Data collected on this form will be used for the purpose of the North Wilts Youth Football League administration as required by League Rules. The
information supplied on this form will be retained by the North Wilts Youth Football League.
Under the provisions of the Data Protection Act 1998 the League is ‘Exempt from Notification’. However, any member wishing to ascertain what personal details, if
any, the League holds about them, should write, enclosing a SAE to the General Secretary.
The League does not accept any responsibility for any injuries sustained during any match. This is the sole responsibility of the Registered Club of which the
player is a member. No claims shall be considered by the League Committee under any circumstances.
