If a patient wishes to see a specific GP, they may have to wait longer, however, if a patient needs to be seen quickly they will be offered the first available appointment or a ring back from a clinician if no suitable appointments are available. Some appointments are available to book on line. Please ask reception for more information. Minor ailments and infections can also be seen and treated by our prescribing nurse. We operate a queuing system on our phones. If you are in the queue we will answer your call as soon as possible. If you ring off you will go to the end of the queue. If you do not need to be seen urgently, your call will be answered sooner if you ring later in the morning. Nurse Appointments Many conditions, such as Coronary Heart Disease, Diabetes, Asthma and COPD are managed by our nurses. Reception may need to ask for more information, in order to be sure they book a patient with the correct nurse, and for the correct amount of time. HRT & Pill checks – please book with the nurse. Booking a few weeks before a new prescription is needed is recommended. Vaccinations Baby clinics are held on alternate Thursdays at each site. Children will be invited to attend for routine vaccinations and development checks; Health Visitors are based at both sites, and will be at both clinics. Travel vaccinations are administered by the practice nurses. Most routine vaccinations are free on the NHS, but a few are private, and reception will advise patients of the fee. Smoking Cessation Support and free nicotine replacement therapy can be obtained from Stop Smoking Wales or local pharmacies : www.stopsmokingwales.com tel: 08000852219 Healthcare Assistants Routine blood tests and blood pressure checks will be booked with our Healthcare Assistants. Home Visits Home visits can be arranged when a patient is too ill to attend surgery. Please provide reception with details and a contact phone number when requesting. Please ring on 830235 or 710251, preferably before midday. Out of Hours When the surgery is closed, urgent medical attention is provided by the Out of Hours service based in Ysbyty Gwynedd. It is not a drop in centre, and patients must ring first to request a visit or appointment. In case of a serious emergency please ring 999. Items requested, not on a patient’s repeat slip, may take longer. If ordering early, please let us know why or the prescription may be delayed. TESTS We need to monitor the effect and effectiveness of medication, and will regularly ask patient to attend for blood tests, blood pressure checks, and reviews with a GP or nurse. It is important that patients do attend when requested. Registration Patients will be asked to complete a registration form and questionnaire when registering. Proof of identity and residency will be requested. If a patient changes address or phone number, they need to let us know. They should also let the hospital know, if they are under its care too. Those moving out of our catchment area will be advised to register with a practice nearer their home, as we cannot offer a full service, including home visits to those outside of our catchment area. OTHER INFORMATION Medicals & Reports Questionnaires - Please hand in at reception forms you wish the GP to complete. For most a fee will be changed Medicals - Please discuss with reception, as these will be longer than a routine appointment, and a fee may be charged. Your Information – Your Rights Patients have a right under the 1990 Access to Health Records Act/1998 Data Protection Act to access their health records, and requests should be addressed to the Practice Manager. A fee may charged for copies. Please ask reception for our Access to Information leaflet for more information. Advanced Directive Patients can request specific wishes be recorded on their records in the form of an Advanced Directive. Please ask for our leaflet on this subject. Freedom of Information Requests for a copy of the Practice’s Publication Scheme should be put in writing to the Practice Manager. Sharing Information Patients’ records are held on both our clinical system, and in paper records. Patients may receive care from organisations that are not part of the NHS, such as Social Services and other healthcare providers. If so there may be some need to share information. Only information that needs to be shared is shared, and we may ask for prior consent from the patient. For more information, please ask for a copy of our Leaflet: Information Sharing. Limited medical and therapy information is shared with the Out of Hours Service, to assist the clinicians there. Patients can opt out of this service. Complaints If a patient has a complaint, suggestion or concern about the service they have received from the doctors, or any of the staff working in this Practice, please let us know. Aggressive/Violent Behaviour We aim to treat all our patients courteously, and expect patients to behave towards our staff in a similar manner. Patients behaving aggressively towards any member of our team will be reported to a GP and may be removed from our list. Social Media Sites The surgery will not respond to comments made on social media websites such as Facebook, but may contact patients found to be making derogatory or insulting comments. Disabled Facilities Both sites have a ramp and wheelchair access, disabled toilets, and disabled parking areas. A portable loop system for the hard of hearing is available at the Amlwch site.
