If you are ordering a duplicate plaque for a co-owner please enclose a check in the amount of $14.00 (if you are picking it up at the
Ontario Regional June 6th 2010 or $20.00 (if it is to be mailed to you) to cover the cost of the second plaque plus mailing charges.
SOM or DOM must have been obtained in 2010.

included in the PowerPoint Awards Presentation.)
