I was responsible to take care of large numbers of patients of well recognized hematemesis and malena unit to most critical trauma patients in either condition, I was trusted to make the proper management. Also, I have the chance to manage different abdominal surgery “including rectal injury, colonic trauma most of time I treat patient with primary anastomosis. Also start to do resection and primary anastomosis for obstructing colonic tumour with no complication. My management always accepted my consultant. Also, I have the chance to operate in rare surgical pathology e.g. malrotation of bowel in adult cecal volvulus. Internal hernia and intestinal obstruction cause ascaris. Also, I have the chance to see good number of drug addict patient who presented to emergency room with groin and pelvic abscess and necrotizing fasciatis. I managed all types of foreign body ingestion. Also I treat most of abdominal emergency starting from rupture hepatoma to rupture liver abscess, all types of bowel perforation and duodenal ulcer, small bowel and large bowel perforation. I use to see perforated duodenal ulcer not less than two per month. Frequently, I see nail gun injury and air gun injury in different site of body (neck, abdomen, spine and extremity). I perform and supervise amputation of extremity in crush injury diabetic and ischemic patient being working in Riyadh Central Hospital and receiving large number of patient from the hospital and peripheral area that give me the chance to see and manage most of surgical complication. I scrubbed in good number of Hepatobiliary surgery and pancreatic surgery. Known that TB is common problem I used to perform and assess in diagnostic laparoscopy with help of infectious disease treating team. I used to treat TB adenitis, TB breast axillary and psoas abscess. I treat good number of amebic liver abscess with help of interventional radiologist, laparotomy and ERCP to stop bile leak. I have seen the complication of diabetes e,g, retroperitoneal abscess, abdominal wall gangrene, foreign gangrene and fasciatis. I manage patient with entercutaneous fistula (biliary, small and large bowel). In Central Hospital there is an Intensive Care Unit under care of surgeon. 2 units of multiple trauma under care of Intensivist as well as hematemesis and malena unit. I used to take care of our patient in that special unit. CV...AMAL A’KARIM/p5 Regarding elective surgery, I operated in large number of laparoscopic cholecystectomy . I also train resident to do so. I operated in thyroid for different pathology. I scrubbed in adrenal tumour e.g. Conn’s syndrome pheochromocytoma, also I have seen and manage large number of gastric tumours. I operated in submandibular salivary gland and I assist in many parotid surgery. I operated in so many breast pathology from lumpectomy, microductomy to mastectomy. A lot of perianal pathology fistula, hemorrhoid, fissure and sinus. In surgical OPD with a capacity not less than 60 patient per clinic as Senior Registrar, I used to see ore than 40 patients. I am responsible for arrangement of admission. I supervise resident in their work in clinic while doing minor surgery. The OPD clinic starts from 8 am to 6 pm. I used to organize and divide the work in the clinic. Regarding academic activity, most of board exam and course are conducted in Riyadh Central Hospital in each time. I organize and prepare the exam cases. Also I organize the student exam in surgery for several occasions. I share in presentation in the morning round and also I present so many topics in the teaching activity. I also presented Riyadh Surgical Round entitled “The Primary Anastomosis in Non-prepared colon”. I came back to King Khalid University Hospital on 1 Muharram 1422. I shared in laparoscopic conference and scrubbed with international laparoscopic surgeon. My experience in General Surgery started with Internship in General Surgery in Riyadh Medical Complex with Dr. Mohammed K. Alam, Consultant General Surgeon and Dr. Inam. I involved in clerking all new admission (emergency and routine), participate during ward round, attending OPD and assisting in operations. I joined the Arab Board Exam Residency Training Program on 1 October 1991. During the First Year of my residency, I worked in General Surgery at King Khalid University Hospital with Dr. Mohammed Khoshim, Dr. Khalid Murshid, Dr. Saleh Al-Sulaimani, Dr. Saleh Al-Khuwaitir Dr. Mohsin Al-Tameem, Dr. Nail Al-Khudairy, Dr. Mervat A-Saleh, Dr. Saad Faqih and Dr. Abdulkarim ElBakry. I was involved in total care of their patients. This involved in the preoperative, postoperative care, assisting them in operating theater, performing some operation under supervision, participating in OPD clinics, preparing discharge summaries. I have assisted in laparoscopic surgery, thyroid surgery, laparotomy for trauma and other pathologies, breast surgery and performed under supervision, appendicectomy, hernia repair, open cholecystectomy, excision of lipoma and other lumps. During the 2nd and 3rd years of my residency program, I have worked for 3 months in Neurosurgery with Prof. Naim Ur Rahman and Dr. A. H. Jamjoom. My job involved clerking of patients, assisting Consultants in Surgery. I have been involved in management of head injury patients. Assisted in many craniotomy for emergency and elective cases. Assisted in discectomy, microscopic and conventional for disc prolapse. I have also worked for 4 months in Orthopaedics with Dr. Mamoun Kremli and Dr. Saleh Al-Harby. During this period I was trained to manage fractures by close reduction and by surgery. I gained good experience in managing patient with CDH. I was also involved in Elizarov bone elongation technique. I worked for 3 months in Pediatric Surgery with Prof. Asal Al-Samarrai. I used to take care of small babies calculating their fluid and dose of medications, assisting in hernia repair, splenectomy, esophageal dilatation, and removal of swallowed foreign bodies. I did many circumcision during this period. I worked for 3 months in Plastic Surgery with Dr. Bassam Hawary, Dr. Eufemiano Cardoso. During this period, I gained experience in management of burn and bedsores. I have done few skin grafts under supervision. I assisted in cleft lip and palate and other congenital anomalies and other cosmotic surgery. I worked for 3 months in a 20 beds Surgical Intensive Care Unit (SICU) at King Faisal Specialist Hospital. I gained experience in clerking and taking care of a very sick patient, management of septic shock, and post major surgery patients. I am able to insert central line, Hickman catheter, and endotracheal intubation. I worked for 3 months in Cardiothoracic Unit with Prof. Mahmoud Ashour, and Dr. Samir Maher. My job was clerking patient, pre and post op care. I am able to put chest tubes assisted in many thoracotomy, pneumonectomy, esophageal surgery, coronary bypass surgery and tracheostomy. I worked for 3 months (Riyadh Central Hospital) in the Emergency Department. During this period, I was involved in the initial management of trauma patients, preparing them for surgery and dealing with other emergency. CV...AMAL A’KARIM/p7 I worked 3 months in Vascular Surgery with Dr. Mussaad Al-Salman. I gained experience in dealing with ischemic limbs, diabetic foot. I have assisted in aneurysm surgery, carotid surgery, bypass surgery and AV fistulas. I did under supervision Hickman catheter, AV fistula and varicose vein surgery. Performed bed side doppler studies for DVT. I was promoted as Registrar on 26 December 1994. At the beginning of my 4th year of residency, I have worked at King Khalid University Hospital which is an 800 bedded teaching hospital. I worked at King Faisal Specialist Hospital for 4 months and Riyadh Medical Complex. Riyadh Medical Complex a 1200 bedded hospital recognized for Arab board training, has 150 General Surgery beds. At King Khalid University Hospital, I was directly involved in the management of surgical patient. I have done laparoscopic cholecystectomy, breast surgery, thyroid surgery, hernia repair, bowel resection surgery for benign and malignant condition. Assisted in pancreatic surgery and parathyroid surgery. During the period at King Faisal Specialist Hospital, I worked with Dr. Arvidsson who is a Oncologist Surgeon. I used to take care of his patients, organize the meeting with Oncologist and Radiotherapist. I have assisted him in a good number of gastric malignancy, esophageal malignancy. I had the chance to operate in gastric carcinoma under his supervision. I also worked with Dr. Tariq Amin who is a Hepatobiliary Surgeon. I used to take care of the management of complicated biliary surgery, resection of liver tumors and pancreatic cysts and tumours. I worked with Dr. Ingmanson, an Endocrine Surgeon, which provided me a good opportunity to learn many of thyroid, parathyroid, and adrenal surgery. I am now working in RMC from 2 September 1997 till. I deal with a large number of surgical emergencies and multiple trauma patients. I particularly gain experience in the management of patient with hematemesis and melena in a special units. I have perform many laparatomy for different pathology. In summary, during my surgical training, I have gain good experience in surgical trauma, emergency surgery, tumor surgery, vascular surgery, and laparoscopic surgery.
