checklist below for all seniors. We hope that by checking off the various items, the
student is responsibly handling the tasks necessary to secure a trouble-free graduation.

Completed your ‘COMMITMENT to STUDENTS’ AWARD? Each year the senior class
nominates one staff member for the Commitment to Students Award. RETURN TO THE MAIN




Completed your REQUEST FOR FINAL TRANSCRIPT? Colleges and universities need a final
copy of student’s transcripts. Transcripts cannot be completed and mailed without this form
completed. RETURN TO THE MAIN OFFICE BY MAY 12, 2010.

Completed your SENIOR SURVEY? This form must be submitted to the Guidance Office
before students can pick up caps and gowns. It is important that all students fill out this form
completely and accurately. This survey is used to report the percentages of GOHS students
planning to attend college. RETURN TO THE MAIN OFFICE BY MAY 12, 2010.

Completed your GRADUATION CONTRACT? In order to receive your Cap & Gown on May
13th or 14th, you and your parent must both sign this contract.

Completed your COMMUNITY SERVICE FORM? This is to be turned in at Graduation
Rehearsal on Friday, May 28th at the Civic Center.

Participation Form
In order to satisfy the participation requirements for the “Award of Merit” a student must
document a minimum of 100 hours of participation in high school co-curricular, extracurricular
and/or voluntary community or church activities.
Students must complete this participation form indicating the type of participation, dates and
total hours of participation. The student will be required to secure the signatures of appropriate
adult supervisors, coaches, directors or advisors for verification of all entries.

GRADUATION CONTRACT
CLASS of 2010
GlenOak High School will provide a formal commencement ceremony for graduating seniors who
choose to participate on Sunday, May 30th, at 2:00 p.m.
We have made special arrangements to ensure that this event will reflect the importance of the passage
into adulthood and the pride that we all have in academic achievement. GlenOak faculty members will
assemble with our students in the ceremony to celebrate this occasion.
We hope you agree that our students, many of whom are the first in their families to graduate from high
school, deserve an attentive and appreciative audience. Please emphasize the formality of this occasion
to friends and family who plan to attend the ceremony. Thank you for joining with us to provide the
best graduation ceremony yet.
The following contract is to be signed and returned by seniors
in exchange for their cap and gown on the assigned dates.
We understand that GlenOak's Commencement is a formal occasion. We understand by signing this
contract that we will remain courteous and respectful throughout the ceremony and will not participate
in any behavior that will detract from the solemnity of the event. We agree that the students
participating in Commencement have the right to have their names publicly announced without
interruption or noise. We further agree to save all unrestrained expressions of joy until after the
ceremony is completed.
