Hampton St. Tucson, AZ 85716 Phone 520.319.1109 • Fax 520.319.7013 1 Page Client Contract and Lodging Agreement E xodus Community Services exists for the sole purpose of providing men in recovery from drugs and alcohol with safe, drug and alcohol free, twelve-step oriented environments in an effort to provide each client with the care and structure necessary to develop and learn the necessary life skills essential for transition to independent living. Selection for residency shall be limited to those in recovery, coming from organizations where previous treatment was received and is not available to those who are in need of detoxification, have a history of chronic violence, sex offenders, the severely physically or mentally disabled, the elderly who require residential care and treatment outside the scope of or related to alcohol and substance abuse. The undersigned hereby acknowledges that he or she is entering a residential care facility for substance abusers and will reside in said facility in the capacity of a lodger sharing a housing unit and not as a tenant with rights or possession of space exclusively. All programming fees are due at mandatory house meetings held each week and there are no grace periods without the written consent of the Resident Director. If a legitimate emergency occurs, the Resident Director should be informed immediately and may consent to an extension. Any failure to pay programming fees when due shall allow Exodus Community Services to immediately void this agreement. The undersigned client also agrees to participate in and abide by the policies and rules set by the staff and administration of Exodus Community Services. These policies are to be observed by all clients. They have been established by Exodus Community Services to maintain a clean, safe and healthy living environments for those in recovery who are willing and ready to transition back into society. Any infraction of these policies could result in disciplinary action and possible termination from the program, expulsion from the house and forfeiture of any prepaid programming fees. The undersigned agrees to vacate the shared accommodations when cardinal rules are violated, by or by a vote of Exodus Community Services staff. -1- Terms and Conditions This agreement shall include all the following terms and conditions: 1. A full commitment to ninety (90) days of residency in a Exodus Community Services Recovery Home. 2. Advance payment in full of all financial obligations as determined by the organization. (If unemployed at time of entrance into an Exodus Community Services Recovery Home, new clients must agree to go to day labor the first four (4) working days of each week to ensure full and timely payment of all programming and service fees. Current programming fees are _________ per week or_________ per month and cover the expenses of lodging, utilities, household supplies, meals, accessory treatment and supervision.) 3. Strict adherence to Exodus Community Services’ ZERO TOLERANCE policy regarding the use of drugs and alcohol as well as to : the use of all types of pornography, theft, violence, verbal or physical abuse towards others, smoking in the house, sexual activity in the house, pets, the housing or carrying of any kind of weaponry, failure to complete chores or fulfill household obligations, disrespecting Safe House staff or failure to adhere to any or all of the house rules or policies of Exodus Community Services. 4. Agreement to observe established curfews. Curfew for the first thirty (30) days is 6:00 PM unless accompanied by a senior resident to a church service or to a twelve - step meeting. After thirty days the curfew moves to 10:00 PM Sunday through Thursday and 12:00 on Friday and Saturday. 5. Agreement to submit to any form of drug or alcohol screening at any time during residency with or without notice. 6. Agreement to attend 100% of all House Meetings unless excused by a director with no less than a 48-hour written notice. 7. Agreement to attend at least five (4) twelve step meetings per week in addition to obtaining a twelve-step sponsor in no less than two weeks from acceptance into a Exodus Community Services Recovery Home. 8. Agreement to participate in Exodus Community Services’ “90 Day Roadmap to Recovery Program,” including submission of “Goal Attainment Forms” and participation in subsequent 30, 60, and 90 day “Performance Reviews” with Resident Directors. 9. Active participation in all activities sponsored by Exodus Community Services unless the activities interferes with a client’s employment or excused by a Resident Director with no less than a 48-hour written notice. 10. Diligently working a twelve-step program by obtaining a sponsor, attending meetings, reading the literature and working the steps. -2- required Policies Exodus Community Services has established the following policies: 1. .2. Orientation. After being screened for residency by an application and interview process clients are selected for residency. Upon arriving at your new home, you will complete an orientation session with the Resident Director. The rules and policies of Exodus Community Services will be gone over in detail at this orientation. Drug and Alcohol Use. Occupancy is made available under the strict understanding that the house is to be, at all times, drug and alcohol free. This includes mind-altering prescription drugs. Additionally, protecting or knowing a fellow resident is drinking or using may be grounds for immediate dismissal. 3. Medication. Our policy prohibits taking any mind-altering medications such as narcotics or psychotropic drugs. However, the Resident Director with the advice of medical professionals may allow their use under certain circumstances and will reserve the right to monitor and secure said medications. Any resident sharing or receiving shared medications will be immediately terminated without question. A record of each client’s prescribed medications must be kept in their file at all times. 4. Employment. We require that all residents work a forty-hour work week and that they maintain gainful, full-time employment. All unemployed clients are required to go to “day labor” until their fees are paid a week in advance. The clients are to be dressed and out of the house by 8:00 am and are not to return until 4:00 pm during the week. Unemployed residents are also required to maintain “Work Search Logs” to submit to their Resident Director for review at the mandatory weekly house meetings. Clients having trouble gaining employment should ask their Resident Director for help. Clients who are disabled, retired, receiving outside support or independently wealthy are expected to spend their days outside of the house actively participating in their own recovery. Written exceptions can only be made by the Resident manager with a 48-hour written notice. 5. Sleeping. All residents should be awake, dressed, beds made; areas cleaned and out of the house by 8:00 am. Conversely, quiet time should begin at 10:00 pm and lights (including all other electronics) should be off by 11:00 pm each night. Residents are not to sleep in the living room. 6. Dress Code. All residents must be properly attired in the community areas. For lounge wear, a minimum of a tee-shirt and pair of shorts or a sweat suit are required. 7. Visitation. All guests are to be entertained in the living room or the patio areas only with visits limited to one hour. All female guests are to be taken off property. No guests are allowed in the bedrooms. Problem guests will be asked to leave and children must be supervised at all times. -3- Policies (Continued) 8. Housekeeping. Each resident is responsible for making their bed daily, cleaning their general area and dresser top, taking out their trash, vacuuming their room, washing their own dishes, and for helping to keep all community areas clean and in good condition. All residents are assigned a specific chore each week that they are to monitor and complete daily. 9. Telephones. Exodus Community Services provides free in-house telephone service with voicemail capabilities. Residents are allowed a maximum of ten minutes for each call and are required to answer the other line should it another call try to come through. Telephone use is a privilege and resident’s are required to answer the phone when it rings in exchange for this privilege. Any problems with the telephones will result in the disallowance of their use. 10. Excessive Utilities. Be aware of abusing the use of free household utilities#. 11. No Loud Music#. Loud and excessive noise will not be tolerated. Only walkman-type radios and CD players are allowed. The TV in the living room will only be played at reasonable volume levels and violations of these rules will be considered disruptive behavior and the offending resident(s) will be subject to disciplinary action. 12. Out of Bounds. Residents may not enter another’s room without permission and may not enter the Resident Director’s room without their being present. 13. WITHOUT WRITTEN PERMISSION, NO RESIDENT WILL: - Install paneling, flooring, built-in decorations partitions, etc. - Drill or attach anything to the floors, walls or ceiling of the house. - Bring in water-filled furniture or heating, ventilating, or air conditioning units. - Put in any shades, blinds, window guards or coverings, in or outside the house. - Permit the accumulation of refuse within the residential unit. *** All personal property is subject to insurance at the resident’s expense.*** 14. House Liability. Exodus Community Services is not liable for any personal property during or after a resident’s stay and will dispose of all personal property seven (7) days after discharge. Exodus Community Services assumes no responsibility for the personal property of the resident. 15. Curfew Exceptions. Reasonable extensions to curfew (i.e. working late, missed buses, etc.) must be made by calling the Resident Director, telling a fellow resident, or leaving a a message on the house voicemail. -4- Policies (Continued) 16. Extended Stays and Passes. All requests must be made in writing with a minimum of a 48-hour notice. The Resident Director will consider requests based on performance in the house. A minimum of 30 days is required, and all program fees must be current to be approved. 17. Goal Establishment. All clients of Exodus Community Services must participate in the “90 Day Roadmap to Recovery” program and submit all subsequent “Goal Attainment Forms” as a condition of residency. These forms must be made available for review at mandatory weekly house meetings. 18. Performance Reviews. All residents are required to participate in “performance reviews” with the Resident Director every thirty days. These reviews will be used to evaluate each resident’s progress and will be the determining basis for continued residence. 19. House Activities. All residents will participate in house activities, including but not limited to: business meetings, group sessions, major house or yard work duties, etc. 20. Urine Analysis. It is understood that the undersigned agrees to submit to frequent, periodical and/or random urine analysis as a requirement for residency. 21. Notice to Vacate. A thirty (30) day notice of intention to move and vacate is requested from all residents. 22. Consequences. The staff of Exodus Community Services reserves the right to enforce the following consequences for any and all of the above rules and policies: “three strikes and you’re out” extra chores, house restriction, no passes, curfew, or termination. 23. Grievances. If a resident feels that another resident is not doing their share of the chores, or has any other grievances towards another resident, he may call a meeting with the Resident Director to discuss said grievance. If a resident’s action or behavior is found unsatisfactory the resident will be given the opportunity and a reasonable amount of time to change their behavior. If after two house meetings the offending resident fails to make the necessary changes the Resident Director will, at his discretion, terminate the lodging agreement. The resident shall vacate the premises immediately. 24. Executive Director’s Rights. The Executive Director of Exodus Community Services reserves the right to make all final decisions regarding infractions made against this client contract and lodging agreement and has sole and final jurisdiction over all infractions” not specifically stated herein.” -5- Discharge Policies As of June 7, 2009, all resident discharges shall be conducted in the following manner: Resident Director shall accompany discharged resident at all times until the resident is off the property. If drug or alcohol use is apparent, the resident shall be escorted off the property immediately and allowed to return for their possessions the following day at a time when the Resident Director is present. The Resident Director will then pack the resident’s personal belongings and have them ready for the discharged resident’s return. Upon termination, resident shall be allotted five minutes of telephone time to arrange for transportation. Resident will then be given no more than thirty minutes to gather their belongings and vacate the premises. Should the discharged resident become violent or belligerent, ask the residents to leave immediately and call the police. Once the resident is successfully discharged, inform your superior, change the door code and inform all residents of the change. Personal or abandoned belongings will be kept no longer than seven (7) days. -6- Grievance Policy If you have grievance with another resident, go to the resident first. If that fails to resolve the issue, go to your resident director. If that is also unsuccessful, please call the Community Outreach Director at 409-3436. If your grievance is with your Resident Director, please call the Community Outreach Director immediately at 409-3436.
