This contract entered into on the date of acceptance, by and between the listed
horse's owner or lessee of record, hereinafter referred to as the "Seller" and Nicole Bell,
hereinafter referred to as the "Agent" who mutually agree to abide by the following
terms and conditions:
Above said consignment horse/pony is under one-week evaluation by agent to
determine overall soundness, performance, training level and length of time required
to present to market.
Owner shall furnish current vaccinations, coggins and also vital information
concerning health history and disposition. In the event of disease, illness, injury, or
colic occurring to the horse while under this consignment agreement, agent will
attempt to contact the owner. In the event that agent is unable to reach owner by
phone, owner authorizes agent to secure the services of a licensed veterinarian to
attend, treat and/or medicate horse/pony.
 In the event of colic treatment or surgery at CSU or Fossil Creek, owner does
authorize agent to transport horse/pony and does want surgery done
_________per the following payment arrangement.
 Owner authorizes treatment for colic but does not want surgery

 Owner is responsible for payment of all veterinary charges, and must contact
CSU or Fossil Creek to authorize any surgeries and make
payments____________.
Agent shall not be liable for any damages to the horse/pony arising out of or from
boarding, training, and or transporting of the horse or that may occur from any
cause whatsoever in connection therewith, including loss by theft, running away,
death or injury for the duration of the agreement, whether or not the horse is on the
premises of Crestone Farms.

 Owner agrees to promptly pay for all farrier expenses deemed necessary by

Owner agrees to pay board on horse/pony at __________per month until horse/pony
is sold. Board will be prorated from the bill of sale for any remaining days within that
month and credited to owner. Owner agrees to pay training on horse at $_____ per
month until sold.____
If owner chooses not to pay training costs a higher commission, ______%, will be
paid to agent after horse/pony is sold. Determined below.______
Settlement to owner will be determined by performance record, training, veterinarian
soundness exam and overall evaluation by agent. Owner and agent agree on asking a
price of $_______ with a commission of _____% paid to agent from the proceeds.
Payment for said horse/pony will be tendered to the owner within 10 days of sale after
all paperwork is complete and checks have cleared. Statements rendered.
The Seller's signature on this contract attests to the fact that the Seller has read and
understood all of the terms outlined herein and accepts this contract as a legal and
binding instrument on behalf of the Seller, the Seller's heirs, successors and assigns.
This contract is made and entered into, in the County of Boulder, regardless of the
manner or location of solicitation, and shall be interpreted, enforced and transacted
under the laws of the State of Colorado regardless of the manner of solicitation.
