West Dorset District Council has introduced concessionary admissions for disabled residents to the Leisure Centres in the District namely: Dorchester Sports Centre, the Gryphon Sports Centre in Sherborne, Bridport Leisure Centre, St Osmunds Community Sports Centre in Dorchester, and Budmouth Community Sports Centre in Chickerell. For information on concessionary entitlements to the facilities and how to obtain them please contact the centres directly as these may change. To enable residents to qualify for their concessionary reductions it is necessary to register with the Economy, Leisure & Tourism Division to gain a Leisure Pass. If you are REGISTERED DISABLED or LONG TERM SICK you may be entitled to up to 50% discounted entry into the Dorchester Sports Centre, Bridport Leisure Centre, Gryphon Sports Centre, St Osmunds Community Sports Centre , and Budmouth Community Sports Centre. To qualify you must be: i) A permanent resident of West Dorset and ii) Claiming one of the following benefits: Allowance for a Lowered Standard of Occupation Attendance Allowance Constant Attendance Allowance Disability Living Allowance Personal Independence Payment Disabled Persons Tax Credit Exceptionally Severe Disablement Allowance Employment Support Allowance (ESA) Industrial Injuries Disablement Benefits Long-term Incapacity Benefit Motability Allowances Pneumoconiosis, Byssinosis or Disease Disablement Benefit Severe Disablement Allowance Vaccine Damage Payment War Disablement Pension Or iii) You are Registered Disabled and have proof of registration on the Dorset County Council Disabled Register Please note: Leisure Passes will only be issued if one of the above benefits are received by the applicant, the definition of disablement benefits are as determined by the Department of Work & Pensions. 2. HOW DO I APPLY FOR A LEISURE PASS? Leisure Pass application forms are available from Economy, Leisure & Tourism Division, West Dorset District Council, South Walks House, South Walks Road, Dorchester DT1 1UZ or by telephoning (01305) 252250. You will need to complete the relevant details required on the form and return the form in person (do not post) to one of the registration posts detailed in Section 7, together with two passport size photographs, suitable proof of identity, *residency (i.e. Drivers Licence, household bill) and proof of benefit received as stated in 1.above. You will then be issued, by post, with a numbered Leisure Pass in due course. 3. HOW LONG IS THE LEISURE PASS VALID FOR? The Disabled Leisure Pass is non-transferrable and will be valid for 12 months (unless otherwise stated on the card), after which time, if there is no change of circumstances, you may wish to reapply. This can be done by returning your expired card in person (do not post) to any of the outlets detailed in Section 7, together with suitable proof of identity, *residency and proof of benefit received and two passport size photographs. 4. WHAT DO I GET? The passes are free to all those eligible as set out under Section 1. You will receive up to 50% discount on some activities within Dorchester Sports Centre, Gryphon Sports Centre, Budmouth Community Sports Centre, Bridport Leisure Centre, and St Osmunds Community Sport Centre. You should contact the Centre in advance to establish exactly which activities are covered by the scheme. 5. WHAT HAPPENS IF I LOSE MY LEISURE PASS? If you lose your Leisure Pass, it must be reported immediately to the Economy, Leisure & Tourism Division Headquarters (Tel: 01305 252250). A fee of £2.00 may be charged to cover the extra administration costs and you will be required to submit another passport photo for the new card. DO ‘CARERS’ OF A DISABLED PERSON GAIN DISCOUNTED ENTRY? 6. If a disabled person (holding a West Dorset District Council Leisure Pass for Disabled People) is not able to participate in an activity without the assistance of another person, then that ‘carer’ will be eligible for discounted entry into the Centre. If assistance is required, it must be clearly stated on the application form. This will then be noted on your leisure pass. Discounted entry is only applicable to one carer per disabled person. * Suitable proof of residency - Drivers Licence, DW&P Letter showing proof of Benefit/Pension, Household utility bill or NHS registration card. 7.
