Project: Mosswood Cemetery Columbarium Project, 280 Putnam Ave, Cotuit, MA 02635 B. Owner: Town of Barnstable, Department of Public Works, 367 Main Street, Hyannis, MA 02601 C. The Work consists of the installation of (3) three single sided, 32 Niche, Columbarium Units, on concrete foundation, and (3) three granite benches on concrete slabs. The site is to be left in a “before construction” condition. 1. Owner-Furnished Items: None 2. Work Under Other Contracts: Flagstone walkway and stone veneer on Columbariums. 3. Contractor-Furnished Items: Foundation forming and installation Columbariums and Granite Benches. concrete placement. Complete 4. Owner Items Of Work: Site work including: Excavation and backfilling of foundation. Installation of underground conduit for future electrification. Underground conduit for future Irrigation system. Planting of lawn and various plant landscaping. Stone walk and stone veneer on Columbariums. 1.02 WORK RESTRICTIONS A. Contractor's Use of Premises: During construction, Contractor will have limited use of site indicated. Contractor's use of premises is limited only by Owner's right to perform work or employ other contractors on portions of Project. 1. Owner will occupy premises during construction. Perform construction between the hours 7 AM to 7:00 PM, Monday thru Friday, (other than holidays and weekends), unless otherwise agreed to in advance by Owner. Clean up work areas at the end of each work period. 2. Driveways and Entrances: Keep driveways and entrances serving premises clear and available to the public, staff and emergency vehicles at all times. Do not use these areas for storage of materials. a. Schedule deliveries to minimize use of driveways and entrances. b. Schedule deliveries to minimize space and time requirements for storage of materials and equipment on-site. c. Contractor shall coordinate completion and clean up of work on a daily basis. B. Clean site each day. Contain waste material daily in a neat manner. Section 011000 - General Requirements PART 2 – PRODUCTS (Not Applicable) PART 3 - EXECUTION (Not Applicable) END OF SECTION 011000 011000 – General Requirements SECTION 012900 - PRICE AND PAYMENT PROCEDURES PART 1 - GENERAL 1.02 CONTRACT MODIFICATION PROCEDURES A. On Owner's approval of a proposal from Contractor on AIA Document G709, or similar form, Project Manager will issue a Change Order on AIA Document G701, or similar form, for all changes to the Contract Sum or the Contract Time. B. When Owner and Contractor disagree on the terms of a proposal, Project Manager may issue a Construction Change Directive on AIA Document G714, or similar form, instructing Contractor to proceed with the change, for subsequent inclusion in a Change Order. Construction Change Directive will contain a description of the change and designate the method to be followed to determine changes to the Contract Sum or the Contract Time. 1.03 PAYMENT PROCEDURES A. Submit a Schedule of Values at least 10 days before the initial Application for Payment. Break down the Contract Sum into at least one line item for each Specification Section in the Project Manual table of contents. Coordinate the Schedule of Values with Contractor's Construction Schedule. 1. Round amounts to nearest whole dollar; total shall equal the Contract Sum. 2. Provide separate line items in the Schedule of Values for initial cost of materials and for total installed value of that part of the Work. B. Submit 3 copies of each application for payment on Town of Barnstable standard form, according to the schedule established in Owner/Contractor Agreement. 1. Submit final Application for Payment after completion of Project closeout procedures. a. Include consent of surety to final payment and insurance certificates. PART 2 - PRODUCTS (Not Applicable) PART 3 - EXECUTION (Not Applicable) END OF SECTION 012900 Section 012900 – Price And Payment Procedures Section 012900 – Price And Payment Procedures SECTION 013100 – PROJECT MANAGEMENT AND COORDINATION PART 1 – GENERAL 1.01 PROJECT MANAGEMENT AND COORDINATION A. Coordinate construction to ensure efficient and orderly installation of each part of the Work. 1. Schedule and conduct progress meetings at Project site at weekly intervals. Notify Project Manager of meeting dates and times. Require attendance of each subcontractor or other entity concerned with current progress or involved with planning or coordination of future activities. a. Contractor will record minutes and distribute to everyone concerned. 1.02 SUBMITTAL PROCEDURES A. Coordinate each submittal with fabrication, purchasing, testing, delivery, other submittals, and related activities that require sequential activity. 1. No extension of the Contract Time will be authorized because of failure to transmit submittals enough in advance of the Work to permit processing, including re-submittals. 2. Submit three copies of each submittal. Project Manager will return one copy. B. Place a permanent label or title block on each submittal for identification. Provide a space approximately 6 by 8 inches (150 by 200 mm) on label or beside title block to record Contractor's review and approval markings and action taken by Architect. Include the following information on the label: Project name. Date. Name and address of Contractor. Name and address of subcontractor or supplier. Number and title of appropriate Specification Section. C. Identify deviations from the Contract Documents on submittals. D. Contractor's Construction Schedule Submittal Procedure: Submit three copies of schedule within 10 days after date established for Commencement of the Work. PART 2 - PRODUCTS 2.01 ACTION SUBMITTALS A.. Product Data: Mark each copy to show applicable products and options. B. OSHA 10 Certification: All workers to provide copies of their OSHA 10 certification cards to the Owner’s Project Manager prior to start of work. OSHA 10 Certification is required of all workers and contractors on this project. Section 013100 – Project Management And Coordination C. Payment Bond: A 50% payment bond is required as part of this project. 2.02 INFORMATION SUBMITTALS A. Qualification Data: Include lists of completed projects with project names and addresses, names and addresses of architects and owners, and other information specified. PART 2 - PRODUCTS (not applicable) PART 3 – EQUIPMENT (not applicable) PART 4 - EXECUTION 4.01 TEMPORARY UTILITIES Sanitary Facilities: Provide temporary toilets, wash facilities, and drinking-water fixtures. Comply with regulations and health codes for type, number, location, operation, and maintenance of fixtures and facilities. 4.02. TEMPORARY SUPPORT FACILITIES A. Provide waste-collection containers in sizes adequate to handle waste from construction operations. Collect waste daily and, when containers are full, legally dispose of waste off-site. Comply with requirements of authorities having jurisdiction. 4.03 TEMPORARY SECURITY AND PROTECTION FACILITIES A. Provide temporary environmental protection, operate temporary facilities, and conduct construction in ways and by methods that comply with environmental regulations and that minimize possible air, waterway, and subsoil contamination or pollution or other undesirable effects. 4.04. TERMINATION AND REMOVAL A. Remove temporary facilities and controls no later than Substantial Completion. Personnel remaining after Substantial Completion will be permitted to use permanent facilities, under conditions acceptable to Owner. END OF SECTION 015000 Section 015000 – Temporary Facilities And Controls Section 015000 – Temporary Facilities And Controls SECTION 016000 – PRODUCT REQUIREMENTS PART 1 - GENERAL 1.01 Definitions A. The term "product" includes the terms "material," "equipment," "system," and terms of similar intent. B. Product Substitutions: Substitutions include changes in products, materials, equipment, and methods of construction from those required by the Contract Documents and proposed by Contractor after award of the Contract. 1. Submit three copies of each request for product substitution. 2. Submit requests within 10 days after the Notice to Proceed. 3. Do not submit unapproved substitutions on Shop Drawings or other submittals. 4. Identify product to be replaced and show compliance with requirements for substitutions. Include a detailed comparison of significant qualities of proposed substitution with those of the Work specified, a list of changes needed to other parts of the Work required to accommodate proposed substitution, and any proposed changes in the Contract Sum or the Contract Time should the substitution be accepted. 5. Project Manager will review the proposed substitution and notify Contractor of its acceptance or rejection. C. Comparable Product Requests: 1. Submit three copies of each request for comparable product. Do not submit unapproved products on Shop Drawings or other submittals. 2. Identify product to be replaced and show compliance with requirements for comparable product requests. Include a detailed comparison of significant qualities of proposed substitution with those of the Work specified. 3. Project Manager will review the proposed product and notify Contractor of its acceptance or rejection. D. Deliver, store, and handle products using means and methods that will prevent damage, deterioration, and loss, including theft. Comply with manufacturer's written instructions. 1. Schedule delivery to minimize long-term storage at Project site and to prevent overcrowding of construction spaces. 2. Deliver products to Project site in manufacturer's original sealed container or packaging, complete with labels and instructions for handling, storing, unpacking, protecting, and installing. 3. Inspect products on delivery to ensure compliance with the Contract Documents and to ensure that products are undamaged and properly protected. 4. Store materials in a manner that will not endanger Project structure. 5. Store products that are subject to damage by the elements, under cover in a weathertight enclosure above ground, with ventilation adequate to prevent condensation. E. Warranties specified in other Sections shall be in addition to, and run concurrent with, other warranties required by the Contract Documents. Manufacturer’s disclaimers and limitations on Section 016000 - Product Requirements product warranties do not relieve Contractor of obligations under requirements of the Contract Documents. PART 2 - PRODUCTS 2.01 PRODUCT OPTIONS A. Unless otherwise indicated, Owner will select color, pattern, and texture of each product from manufacturer's full range of options that includes both standard and premium items. PART 2 - EXECUTION (Not Applicable) END OF SECTION 016000 Section 016000 - Product Requirements SECTION 017300 - EXECUTION PART 1 - GENERAL 1.01 EXAMINATION AND PREPARATION A. Examine substrates and conditions for compliance with manufacturer's written requirements including, but not limited to, surfaces that are sound, level, plumb, smooth, clean, and free of deleterious substances; substrates within installation tolerances; and application conditions within environmental limits. Proceed with installation only after unsatisfactory conditions have been corrected. B. Before proceeding to lay out the Work, verify layout information shown on Drawings, in relation to existing conditions. C. Take field measurements as required to fit the Work properly. Where fabricated products are to be fitted to other construction, verify dimensions by field measurement before fabrication and, when possible, allow for fitting and trimming during installation. 1.02 INSTALLATION A. Comply with manufacturer's written instructions for installation. Anchor each product securely in place, accurately located and aligned with other portions of the Work. Clean exposed surfaces and protect from damage. B. Clean Project site and work areas daily. 1.03 A. FINAL CLEANING Complete the following cleaning operations before requesting inspection for certification of Substantial Completion: 1. Remove labels that are not permanent. 2. Clean exposed finishes to a dust-free condition, free of stains, films, and foreign substances. END OF SECTION 017300 Section 017300 - Execution Section 017300 - Execution SECTION 017700 – CLOSEOUT PROCEDURES PART 1 - GENERAL 1.01 CLOSEOUT PROCEDURES A. Substantial Completion: Before requesting Substantial Completion inspection, complete the following: 1. Prepare a list of items to be completed and corrected (punch list), the value of items on the list, and reasons why the Work is not complete. 2. Submit specific warranties, maintenance service agreements, and similar documents. 3. Complete final cleaning requirements. B. Submit a written request for inspection for Substantial Completion. On receipt of request, Project Manager will proceed with inspection or advise Contractor of unfulfilled requirements. Project Manager will prepare the Certificate of Substantial Completion after inspection or will advise Contractor of items that must be completed or corrected before certificate will be issued. C. Request inspection for Final Completion, once the following are complete: 1. Submit a copy of Substantial Completion inspection list stating that each item has been completed or otherwise resolved for acceptance. 2. Instruct Owner's personnel in operation, adjustment, and maintenance of products and systems. 3. Contractor shall submit a monetized punchlist of outstanding items to be corrected or completed. D. Request re-inspection when the Work identified in previous inspections as incomplete is completed or corrected. E. Submit a written request for final inspection for acceptance. On receipt of request, Project Manager will proceed with inspection or advise Contractor of unfulfilled requirements. Project Manager will prepare final Certificate for Payment after inspection or will advise Contractor of items that must be completed or corrected before certificate will be issued. END OF SECTION 017700 Section 017700 – Closeout Procedures Section 017700 – Closeout Procedures DIVISION 3 – CONCRETE 033000 - CAST-IN-PLACE CONCRETE PART 1 - GENERAL 1.01 SUMMARY A. Section includes cast-in-place concrete, including formwork, reinforcement, concrete materials, mixture design, placement procedures, and finishes. B. Related Sections: 1. Section 133300 Architectural Concrete. 2. Section 312000 Earth Moving. 1.02 SUBMITTALS A. Product Data: For each type of product indicated. B. Design Mixtures: For each concrete mixture. C. Steel Reinforcement Shop Drawings: Placing drawings that detail fabrication, bending, and placement. D. Formwork Shop Drawings: Prepared by or under the supervision of a qualified professional engineer detailing fabrication, assembly, and support of formwork. E. Material certificates. F. Material test reports. 1.03 QUALITY CONTROL A. Manufacturer Qualifications: A firm experienced in manufacturing ready-mixed concrete products and that complies with ASTM C 94/C 94M requirements for production facilities and equipment. 1. Manufacturer certified according to NRMCA's "Certification of Ready Mixed Concrete Production Facilities." B. Testing Agency Qualifications: An independent agency qualified according to ASTM C 1077 and ASTM E 329 for testing indicated. C. Welding Qualifications: Qualify procedures and personnel according to AWS D1.4/D 1.4M, "Structural Welding Code - Reinforcing Steel." D. ACI Publications: Comply with the following unless modified by requirements in the Contract Documents: 1. ACI 301, "Specifications for Structural Concrete," 2. ACI 117, "Specifications for Tolerances for Concrete Construction and Materials." E. Concrete Testing Service: Engage a qualified independent testing agency to perform material evaluation tests and to design concrete mixtures. F. Pre-installation Conference: Conduct conference at Project site. PART 2 - PRODUCTS Section 033000 – Cast-In-Place Concrete 2.01 FORM-FACING MATERIALS A. Smooth-Formed Finished Concrete on exposed surfaces: Form-facing panels that will provide continuous, true, and smooth concrete surfaces. Furnish in largest practicable sizes to minimize number of joints. B. Rough-Formed Finished Concrete: Plywood, lumber, metal, or another approved material. Provide lumber dressed on at least two edges and one side for tight fit. 2.02 STEEL REINFORCEMENT A. Reinforcing Bars: ASTM A 615/A 615M, Grade 60, deformed. 1. Epoxy-Coated Reinforcing Bars: ASTM A 775/A 775M, epoxy coated, with less than 2 percent damaged coating in each 12-inch bar length. B. Galvanized-Steel Welded Wire Reinforcement: ASTM A 185/A 185M, plain, fabricated from galvanized-steel wire into flat sheets. C. Bar Supports: Bolsters, chairs, spacers, and other devices for spacing, supporting, and fastening reinforcing bars and welded wire reinforcement in place. Manufacture bar supports from steel wire, plastic, or precast concrete according to CRSI's "Manual of Standard Practice. 2.03 CONCRETE MATERIALS A. Cementitious Material: Use the following cementitious materials, of the same type, brand, and source, throughout Project: 1. Portland Cement: ASTM C 150, Type I/II Type III Type V. Supplement with the following: a. Fly Ash: ASTM C 618, Class F. b. Ground Granulated Blast-Furnace Slag: ASTM C 989, Grade 100 or 120. B. Normal-Weight Aggregates: ASTM C 33, graded. 1. Maximum Coarse-Aggregate Size: 1 inch nominal. 2. Fine Aggregate: Free of materials with deleterious reactivity to alkali in cement. C. Water: ASTM C 94/C 94M and potable. 2.04 CURING MATERIALS A. Absorptive Cover: AASHTO M 182, Class 2, burlap cloth made from jute or kenaf, weighing approximately 9 oz./sq. yd. when dry. B. Moisture-Retaining Cover: ASTM C 171, polyethylene film or white burlap-polyethylene sheet. C. Water: Potable. 2.05 RELATED MATERIALS A. Expansion- and Isolation-Joint-Filler Strips: ASTM D 1751, asphalt-saturated cellulosic fiber. 2.06 CONCRETE MIXTURES A. Prepare design mixtures for each type and strength of concrete, proportioned on the basis of laboratory trial mixture or field test data, or both, according to ACI 301. Section 033000 – Cast-In-Place Concrete B. Admixtures: Use admixtures according to manufacturer's written instructions. 1. Use water-reducing, high-range water-reducing, or admixture in concrete, as required, for placement and workability. 2. Use water-reducing and retarding admixture when required by high temperatures, low humidity, or other adverse placement conditions. 3. Use water-reducing admixture in pumped concrete, concrete for heavy-use industrial slabs and parking structure slabs, concrete required to be watertight, and concrete with a water-cementitious materials ratio below 0.50. C. Proportion normal-weight concrete mixture as follows: 1. Minimum Compressive Strength: 5000 PSI at 28 days. 2. Maximum Water-Cementitious Materials Ratio: See Table 1 3. Slump Limit: 4. 2.07 FABRICATING REINFORCEMENT A. Fabricate steel reinforcement according to CRSI's "Manual of Standard Practice." 2.08 CONCRETE MIXING A. Ready-Mixed Concrete: Measure, batch, mix, and deliver concrete according to ASTM C 94/C 94M and furnish batch ticket information. 1. When air temperature is between 85 and 90 deg F, reduce mixing and delivery time from 1 hour to 45 minutes; when air temperature is above 90 deg F, reduce mixing and delivery time to 30 minutes. PART 3 - EXECUTION 3.01 FORMWORK A. Design, erect, shore, brace, and maintain formwork, according to ACI 301, to support vertical, lateral, static, and dynamic loads, and construction loads that might be applied, until structure can support such loads. B. Construct formwork so concrete members and structures are of size, shape, alignment, elevation, and position indicated, within tolerance limits of ACI 117. C. Chamfer exterior corners and edges of permanently exposed concrete. 3.02 EMBEDDED ITEMS A. Place and secure anchorage devices and other embedded items required for adjoining work that is attached to or supported by cast-in-place concrete. Use setting drawings, templates, diagrams, instructions, and directions furnished with items to be embedded. 3.03 STEEL REINFORCEMENT A. General: Comply with CRSI's "Manual of Standard Practice" for placing reinforcement. 1. Do not cut or puncture vapor retarder. Repair damage and reseal vapor retarder before placing concrete. 3.04 JOINTS Section 033000 – Cast-In-Place Concrete A. General: Construct joints true to line with faces perpendicular to surface plane of concrete. B. Construction Joints: Install so strength and appearance of concrete is not impaired, at locations indicated or as approved by Architect. C. Install in construction joints and at other joints indicated according to manufacturer's written instructions. 3.05 CONCRETE PLACEMENT A. Before placing concrete, verify that installation of formwork, reinforcement, and embedded items is complete and that required inspections have been performed. B. Deposit concrete continuously in one layer or in horizontal layers of such thickness that no new concrete will be placed on concrete that has hardened enough to cause seams or planes of weakness. If a section cannot be placed continuously, provide construction joints as indicated. Deposit concrete to avoid segregation. 1. Consolidate placed concrete with mechanical vibrating equipment according to ACI 301. C. Cold-Weather Placement: Comply with ACI 306.1. D. Hot Weather Placement: Comply with ACI 301. 3.06 FINISHING FORMED SURFACES A. Smooth-Formed Finish: As-cast concrete texture imparted by form-facing material, arranged in an orderly and symmetrical manner with a minimum of seams. Repair and patch tie holes and defects. Remove fins and other projections that exceed specified limits on formed-surface irregularities. 3.07 CONCRETE PROTECTING AND CURING A. General: Protect freshly placed concrete from premature drying and excessive cold or hot temperatures. Comply with ACI 306.1 for cold-weather protection and ACI 301 for hot-weather protection during curing. B. Cure concrete according to ACI 308.1, by one or a combination of the following methods: 1. Moisture Curing: Keep surfaces continuously moist for not less than seven days. 2. Moisture-Retaining-Cover Curing: Cover concrete surfaces with moisture-retaining cover for curing concrete, placed in widest practicable width, with sides and ends lapped at least 12 inches, and sealed by waterproof tape or adhesive. Cure for not less than seven days. Immediately repair any holes or tears during curing period using cover material and waterproof tape. 3.08 CONCRETE SURFACE REPAIRS A. Defective Concrete: Repair and patch defective areas when approved by Architect. Remove and replace concrete that cannot be repaired and patched to Architect's approval. 3.09 QUALITY ASSURANCE A. Testing and Inspecting: SRP will self perform field tests and inspections and prepare test reports. END OF SECTION 033000 Section 033000 – Cast-In-Place Concrete SECTION 033300 – ARCHITECTURAL CONCRETE - COLUMBARIUMS PART 1 - GENERAL 1.01 GENERAL A. Each unit will provide thirty-two (32) niches. Each niche shall measure twelve inch (12")wide by twelve inch (12") tall by fifteen inch (15") deep. B. The interior of the niche unit shall be constructed of reinforced concrete. C. Each 32 niche columbarium will measure approximately 6'-11 ¼” tall and be approx. 10' long D. The cement concrete shall be 5,000 psi strength at 28 days. E. The concrete reinforcement shall be: 1. Two inch by two inch (2"x2") eighteen (18) gauge steel wire at all corners. 2. Polypropylene fiber mesh shall be used throughout the unit. 3. A one quarter inch (1/4") steel rod through the middle in both directions. F. Shop drawing: Submit complete shop drawings detailing all columbarium components and assembly, including foundation. 1.02 REMOVABLE NICHE COVERS A. Exterior 1. A minimum of 34 removable granite exterior niche covers must be provide. Thirty two (32) installed on the unit and two (2) spares. 2. Each removable exterior niche cover must be single slab of three quarters inch (3/4") thick granite designed to cover one niche space. The exterior niche cover must be compatible with on site engraving of the granite slab. 3. The mounting system for removable exterior niche covers must be Sinner Brothers Type I granite fasteners with bronze floral rosettes, or approved equal. All of the granite exterior pieces must be fastened using stainless shield lag bolts and epoxy, a minimum of two (2) bolts per piece. 4. The granite face of the removable exterior niche covers must have a polished finish. 5. The removable granite exterior niche covers shall be Dakota, or approved equal. Color: Bellingham. B. Internal 1. An internal removable privacy cover shall be constructed of cement board with a minimal thickness of one quarter inch (1/4"). Thirty-two (32) shall be provided, one for each niche. The internal removable privacy cover shall fit neatly into the face of the niche with no anchoring bolts or fasteners. 1.03 CAP, BASE, BACK AND SIDEWALLS A. Description 1. The cap, sidewalls and back will be granite. 2. The base will be made of poured 5000 psi concrete extending eighteen inches (18") above ground per drawings. Section 033300 - Architectural Concrete - Columbariums 3. The base will be Random Course Ashlar Pattern. Rock TBD (by TOB.) 4. The cap & base should overhang by a minimum of one and one half inches (1-1/2") for a drip edge. 5. The sidewalls and cap must be three inch (3") thick solid granite and all exposed surfaces shall have a polished finish. 6. The cap, sidewall and back granite shall be Dakota Granite, or approved equal. Color: Mahogany. 7. All the granite exterior pieces must be fastened using stainless shield lag bolts and epoxy, a minimum of two (2) bolts per piece.
