history.

Zyprexa, 7.5 mg., 8 a.m., for bipolar disorder, Dr. Davis
Levoxyl, 100 mcg, 8 a.m., for hypothyroidism, Dr. Harms
Benzopril, 20 mg. twice/day, for essential hypertension, Dr. Harms
7. Progress/changes in meeting IPP objectives:
MW qualifier 1: Frustration Reaction – Jane has remained stable in this area, no increase
or decrease in frequency. Staff verbally redirect or change Jane’s fixation on a particular activity
by helping her write down on a pad where she wants to go with the date and time.
MW qualifier 2: Personal hygiene – Jane has improved in this area. She now bathes
every other day. Staff verbally prompt Jane to shower or take a bath each day. She is reminded
that bathing is essential to being around other people at work. When she completes her
bath/shower, staff note this on her calendar. At the end of each week, Jane and staff count her
successes and if she showers three or more days/week, she can use one of her movie passes on the
weekend.
Day Program or Employment: Part time working as a ticket taker at Century 16 in
Ventura since 4/12/2005. She works up to 18 hours per week and earns minimum wage. ISE job

coaching provided by Equal Employment Specialists. Her coach is Diane Smith. Jane has had no
changes or issues this quarter.
Housing/Living: 20 hours/month of 1:1 ILS authorized. Jane’s instructor helps her with
medication management, scheduling medical and dental appointments, Housing and Social
Security assistance, personal hygiene and managing her calendar. The instructor also provides
pro-social behavior support in vivo. Jane may become upset or agitated with her neighbors and
yell at them if she has a bad day at work.
Choice: Jane exercises choice for most aspects of her life daily.
Relationships: Jane is close to her mother and father. They live in Camarillo. Jane sees
them once per month and talks with them on the phone at least once per week. Jane has a friend
that she likes to see on the weekends. His name is John C. He receives services from TCRC. Jane
says they are just “friends” at this time.
Life Style: Jane likes living on her own and being able to have friends visit her at home
or in the community. She enjoys going to downtown areas and having coffee and muffins.
Well Being and Health: stable with current medications and medical professionals.
Rights: Jane attends the People First group of Ventura County. The group discusses and
teaches self-advocacy. Jane has been learning to become more assertive this quarter.
Satisfaction: Jane said she likes CSDS instructor. She said her support is, “Right for me
at the moment.” However, Jane does not want to meet ILS staff on weekends, only the weekdays.

9. Based on person-centered thinking, what do we need to learn and/or do:
Give Jane some time to process questions before giving her an answer or deciding what she
should do. Given time, Jane demonstrates fewer frustration reactions.
10. Other important information:
Jane is taking a trip to Reno in November. She may require some additional hours of support to
make travel arrangements.
