S: Re-checking skinned nose that hemorrhaged on 9-17-13. Blood was found in kennel as though he had been rubbing.
O: BAR. reluctant to be handled or to come out of kennel. Healing abrasion to dorsal aspect of nose bridge. No hemorrhage or
inflammation noted.
A: Healing abrasion - r/o trauma, open.
P: No treatment necessary at this time. Kennel staff will place in crate when cleaning kennel to decrease anxiety. Already on review for
oro-nasal fistula with CHM on 9-23-13. LRA

SO: BAR. BCS 3/5. Dandruff present, no evidence of ectoparasites. Ears clean. No ocular or nasal discharge. Cataracts OU. Oral examno maxillary teeth. Oronasal fistula present L side of maxilla. Mandible- has lower canine teeth and lower carnassial teeth only. Appear
clean- recent dental work? No evidence of infection. Lymph nodes normal. Heart and lungs ausc. normal. Abdominal palpation normal.
Small cutaneous skin growth L lumbar area, approx. 2-3mm in size. Fresh abrasions present on nose from rubbing against kennel door.
A: 1) oronasal fistula L maxilla 2) only 4 teeth in mouth 3) cataracts OU 4) dandruff 5) cutaneous growth r/o adenoma or other benign
growth
P: medical waiver inserted for the fistula, review entered for end of stray period.

This animal has: 1) fistula (large hole) present on the left side of his upper jaw. Surgery may be necessary to try to repair this defect if it is
not healing on its own. 2) cataracts in both eyes.
