All documents presented must be in English and/or translated into English by an accredited translator service/sworn-in-translator. We cannot accept your application if you do not provide the documents listed below. If your application does not meet the lodgement requirements it will be returned to you with a copy of this checklist and you will be asked to resubmit it. Such birth certificates usually confirm the name of the mother and the father of the child concerned. Please note that the short version/abridged birth certificates are NOT acceptable. Parental consent is required if the child is not accompanied by both parents.
