STUDY VISA CHECKLIST (DEPENDENT OF A LONG TERM WORK VISA HOLDER or
DEPENDENT OF A LONG TERM STUDENT VISA HOLDER)
You must produce originals or true certified copies of the documents listed below together with any translations.
All documents presented must be in English and/or translated into English by an accredited translator
service/sworn-in-translator.
We cannot accept your application if you do not provide the documents listed below. If your application does
not meet the lodgement requirements it will be returned to you with a copy of this checklist and you will be
asked to resubmit it. Please note that these requirements are for the person holding the job offer, for
accompanying family members, separate forms, fees and requirements apply. Please see other relevant
checklists for more information.

Medical and X-ray Certificates

The passport for each person included in the application must be
valid for at least three months beyond the intended date of
departure from New Zealand.
Application form for Study in New Zealand must be completed,
signed and dated. Please ensure that you complete the health
and character sections in full. Parent/s must sign on behalf of
minor children.
Two recent passport-sized photos must be attached to the form
in the space provided, please write your name at the back of the
photo.
The current application fee must accompany the application.
Credit Card payments
Your CVV/CVC number is required if you are paying by
electronic credit card at this office. (Note: your CVV/CVC
number is the 3-digit number found on the signature strip on the
back of your credit card)
A full unabridged birth certificate is required. Such birth
certificates usually confirm the name of the mother and the
father of the child concerned.
Please note that the short
version/abridged birth certificates are NOT acceptable.
Parental consent is required if the child is not accompanied by
both parents. Consent must be in the form of a sworn affidavit
legally witnessed by a lawyer, commissioner of oaths, magistrate
of the court etc [for South Africans we regret that we do not
accept witnessing by the SAPS, SA Postmaster or SA Bank
Manager]. Please ensure that the affidavit is accompanied by
evidence of photo identification of the parent issuing the consent
letter as well as his/her direct contact details. In cases where
the parents are divorced, we need to see the divorce and custody
orders.
Chest X-ray Certificate [INZ1096] and / or General Medical
Certificate [INZ1007]
Medical certificates must be less than 3 months old at the time
the application is made and completed by a panel
doctor/radiologist. You cannot go to any doctor/radiologist.

Police certificates
If you intend to stay in New Zealand
for 24 months or longer (irrespective
of the length of your initial stay), you
are required to submit a police
certificate.
Applicants aged 17 and over are required to provide a police
clearance certificate from their country of citizenship and any
country in which they have lived for 5 years or more since
attaining the age of 17 years.

Dependents as noted above are NOT required to produce an offer of place or receipt to confirm payment of
tuition fees.
You may appoint a courier to deliver your application to us and then collect your passport and other originals
after the application has been finalised. INZ takes no responsibility for documents lost in the post and therefore
recommends the use of a door-to-door courier service.
Children aged younger than 4 must apply for a visitor visa, all the above requirements apply except for the
Application Form to visit New Zealand and relevant fee.
If either or both parents are already in possession of a visa (of any kind), please include a copy with the
application.
