Information presented by physicians and other healthcare providers leads to a long-term retention rate of only 10%. Providing you with meaningful information, environments (LIFE meetings, website, community events, health videos) in which opportunities are available to reflect, delivering you health information that is updated, both practical and personalized are keys to your long-term success. I hope you have benefited from some of our efforts. There are no simple solutions to good health, you will need to apply multiple strategies FOREVER. We will continue to work with you to help yourself and others. Currently we have seven steps to better health, which I hope you continue to practice. Step number 7, helping others, is in my mind the most important. By helping others you also help yourself. Below are our accomplishments today and our vision of tomorrow. I hope you and others may find a way to contribute in improving the health of our community. Since 1995 the LIFE (Life Style Interventions Forever) team and I have been actively working on improving the health of our patients and their communities by applying multiple strategies. Achievements and progress so far: Empowering individuals to be more active in their health. www.healthcorner.ca – an interactive health website for physicians, learners and patients. It is the cornerstone and pride of our mission to prevent and help treat cardiovascular disease. The website’s popularity is growing and we want you, your family and friends to feel this is an excellent source of useful information. In addition, we want to enable you to participate actively in your health through such developments as a series of interactive tools and interventions. The LIFE meetings–a group program founded by myself catering to patients in a comprehensive way. Dr Greg’s Series on Vascular Health ( over 15 years of hard work) – 6 books discussing control and prevention of cardiovascular disease and a myriad of related topics that are presented in an easy-to-read format. The High Road to Health Passport has been widely distributed. The LIFE Program will constantly be updated with the best and newest scientific data. The ‘Dr. Greg ‘s 30-day diet’ should become available soon. A new book is now being developed for kids. We now want to distribute copies to all who are interested. Health videos – a series that includes topics such as cholesterol, risk factor management, heart disease, the LIFE group, weight management, diabetes, working with your doctor, and raising healthy children. We are also trying to work with local cable companies so we can produce multiple copies of them and distribute copies to all interested parties. Community health events: I have proposed these special days as part of an initiative to increase awareness regarding vascular disease, sugar diabetes and weight management and also to focus on preventive health strategies for everyone. Public health seminars and presentations will also be developed that provide information to a variety of audiences and will include healthy activities. Interactive patient forms, currently available on www.healthcorner.ca Provide students from elementary, high school and university with bursaries for projects on health. (Very successful last year and to be expanded). Youth involvement from elementary schools to high schools (health seminars in schools, health challenges targeted to specific age levels). We have worked so far in one school called Lee Academy. Distribution of blood pressure machines, scales, and step counters to patients. We now have better data on good versus bad step counters & are developing software to monitor activity levels. Currently Developing: The health challenge is a 30-day call-to-arms in an effort to fight the war on heart disease. Patients are encouraged to follow steps to improve eating habits including Dr Greg’s 30-day diet. Exercise is promoted by keeping a daily exercise log, with the goal being 10,000 steps a day and/or 7 hours of physical activity per week. Progress can be recorded and tracked by the patient over the course of the 30 days; the objective is to promote long-term change. Development of a computerized database of patients to track all heart risk factors. We have been successful in cholesterol management. The next major initiative is to develop the hypertension report card, identifying all cardiovascular risk factors in the patient database. Better strategies to help patients ‘know their numbers’ and modify all your risk factors. I want to be able to provide you access to all your numbers. The health report card –We are in the process of transferring 9000 different patient records to potentially be available online on a website with the patient being able to control access through password protection. Provide patients with a way to renew medications automatically through the healthcorner website if they follow the guidelines given by their doctor. Videos on the health challenge and informational videos that cater to different age groups in the school system that promote exercise and a healthy diet, and that work with families to ensure a healthy lifestyle. A series of health report cards for both children and adults, with health prizes to encourage participation and better results. Create venues for younger people to help adults & the elderly battle heart disease. Videos on bypass operations and stroke victims to portray the seriousness of heart problems and the dire consequences of not living a healthy lifestyle. Create a series of videos on multiple topics on prevention. A main priority is to develop a better way for information to be exchanged between the healthcorner website and healthcare databases as to provide a medium for patients and physicians to access information more easily. Provide a larger series of PowerPoint point presentations to better manage lifestyle changes as well modify all risk factors. To make available online many of “Dr Greg’s Presentations”. Form relationships with community-based learning programs and health initiatives. Also welcome others to use the database and website systems, especially individuals interested in their health; physicians etc. Create a program that allows patients to keep an updated list of medications to be used each time one visits the doctor so the physician can make the best decisions on how to further better health. Computer course on how to use the internet and healthcorner.ca 1-day workshop on improving your health where you will receive a copy of part of your medical record so you can really understand your health. Access to a Health Coach, a review of your medications with a Pharmacist, a LIFE Exercise counseloralong with a stress test before and after. I hope we can all contribute as a way to prevent, further prevent and better treat heart disease through education. Prevention is very costly and unprofitable however with your help I can make these programs sustainable over time.
