Transition Assessment to Inform
the Development of the IEP
This training will provide
participants with information on
age-appropriate transition
assessments to inform IEP
development. Participants will learn
about the use of transition
assessments to assist decision
making in regard to instructional
programming, annual goals, the
development of measurable postsecondary goals and related
transition services. Hands-on
activities will be provided.
Exit Summaries
This presentation will review the
Student Exit Summary that is
mandated for all exiting students
with disabilities. The training will
cover key components to include
and samples for participants to
review. *Participants are
encouraged to bring an IEP and
other pertinent documentation
with them so they can complete an
Exit Summary at the training.
Self-Determination
This presentation will highlight the
eight elements of self-determination.
In addition to providing an overview
of the topic, participants will also
engage in activities designed to
encourage and enhance the explicit
instruction of these important skill
components for students.

Linking Students with Disabilities
and Community Resources
This presentation will discuss how
to connect parents with community
resources for students with
disabilities and English Language
Learner (ELL) populations. We will
also discuss strategies to promote
effective communication with
parents.
Transition in the IEP
Designed for special education
teachers and other members of a
school team working with students
in transition. This training will
provide the necessary information
and skills to develop meaningful
transition programs and services
within an IEP.
Graduation Options and Exiting
Credentials for Students with
Disabilities
During the 2012-13 school year, many
changes were made to the graduation
options for students with disabilities.
This workshop will provide an
overview of the current options, the
proposed options, and the new options
that will be in place July 1, 2014. The
requirements and programmatic impact
of these changes will be discussed.
Topics covered will include: The
"sunsetting" of the Regents
Competency Tests (RCTs) beginning
with the 2011 cohort, the extension of
the Safety Net to include the
Compensatory Option effective October
2012, changes to the GED High School
Equivalency, the elimination of the IEP
diploma effective July 2013 and the
new Skills and CDOS Credentials
effective June 2014.

New York State Career
Development and Occupational
Studies (CDOS) Commencement
Credential
This workshop series will be offered
as 3 distinct modules. Each module
builds on the former, as such;
attendance at previous modules is
a necessary requirement for
registration for later modules.
CDOS Module 1 will provide an
overview of the CDOS credential
and the regulatory requirements
necessary for districts to award the
credential. The programmatic impact
of these requirements will also be
discussed. Attendance at the
workshop entitled “Alternate
Pathways and New Graduation
Options for Students with
Disabilities” is equivalent to this
module.
CDOS Module 2 will focus on the
three CDOS learning standards that
serve as the foundation for the
credential. The module will provide
participants with information on
how to integrate, infuse and augment
lessons in order to assist students in
gaining the skills and experiences
necessary to be successful in and out
of the classroom.
CDOS Module 3 will provide
participants with an overview of
NYS work-based learning programs,
job coaching, as well as connecting
these experiences to the CDOS
learning standards and the exiting
credentials.

Interagency Transition Teams
An interagency transition team brings
together a variety of stakeholders who are
supporting youth with disabilities so they
can have the best chances for success as
adults.
Interagency teams serve varied purposes.
They are to: Identify local needs or
discontinuity in policies, procedures,
services, and programs that hinder youth
with disabilities from achieving desired,
valued outcomes; increase the
availability, access, and quality of
interagency transition services through
the development and improvement of
policies, procedures, systems, funding,
and other mechanisms for providing
seamless transition services to youth with
disabilities and their families; help other
service representatives understand the
educational service system including
laws, regulations, and policies related to
transition services; roles and
responsibilities of families and district
personnel; roles of local or regional
interagency planning teams; roles now
expected of other service agencies
involved in the transition process; and,
especially, to enable youth with
disabilities to live, work, and continue to
learn in the community, with supports if
necessary, as adults.
State and local interagency committees
focused on transition have emerged and
expanded due to several factors:
professionals have found it beneficial to
work collaboratively to provide and
coordinate services for youth with
disabilities; federal legislation in the
fields of education, employment, health,
mental health, and others have strongly
encouraged cross-agency collaboration in
addressing individual and family needs;
families are increasingly requesting help
from schools and adult service providers
in arranging for and coordinating
community services needed by their child
as he/she transitions from school to adult
life.
