Welcome to Medwyn Surgery Website
Medwyn Surgery is situated in the market town of Dorking and is a member of Surrey Primary
Care Trust (East Surrey Locality). Our core service relates to provision of traditional family
general practice.
However, as a 'one-stop' healthcare facility, the Medwyn Centre also offers NHS dentistry, onsite pharmacy, an out of hours 'urgent care' centre, physiotherapy, podiatric surgery and a base
for the community nursing staff.
The practice team have a very clear objective in wishing to provide patients with the very highest
standards of health information and treatment. Care to over 9000 Dorking residents is offered at
Medwyn Surgery by Dr. Chris Monella, Dr. Louise Tomei, Dr. Stewart Tomlinson, Dr. Raj
Sekhon, Dr. Emma Linnard and Dr Anne-Marie Quirk.
Appointments and prescriptions online
If you are not currently registered for online appointments or repeat prescriptions,

Appointments will always be offered with the first available doctor. If however, you would
prefer to see a particular doctor for a specific problem we will always try to accomodate your
request.
If you have to cancel your appointment, please remeber to phone us so the appointment can be
given to another patient. All consultations are by appointment so please be punctual.

Woodcocks Pharmacy
The Medwyn Surgery based Woodcocks Pharmacy is open for 100 hours each week. Except on
Bank Holiday's opening times are as follows:
Monday - Saturday 8am to 11pm
Sunday from 10am to 8pm.
There is no requirement to be a patient registered at Medwyn Surgery to use the pharmacy
facilities.

When We Are Closed
The surgery is closed at weekends and on bank holidays. If you need to speak to a doctor or
nurse out of hours health advice is available from the NHS Direct website or telephone helpline.
If you need emergency assistance for a life threatening condition dial 999 to request an
ambulance.
For all other medical advice when the surgery is closed dial 01306 882422 and you will be
directly connected to Thamesdoc (www.thamesdoc.com/) . You will either be offered telephone
advice or asked to attend a primary care centre for further review.
NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and
advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.

How to book an appointment
Surgeries with the doctors and nurses are by appointment only. If you require an appointment
with the doctor or nurse, please telephone the surgery on 01306 882422 between 8.00 a.m. and
6.30 p.m. If you need to be seen within 24 hours, you will be offered the first available
appointment with any of our doctors. Doctor appointments may be booked up to 4 weeks in
advance.
TYPES OF APPOINTMENTS
Medwyn Surgery operates an open practice policy which means you can see any doctor of your
choice.
TELEPHONE APPOINTMENTS - these appointment should be used if you believe that your
medical problem can be resolved without seeing a doctor. They are five minute appointments
and are available most dats with your preferred GP. They can be particularly useful
for discusing test results or medication reviews only.
ROUTINE APPOINTMENTS - these are standard 10 minute appointments in the doctor' s
morning or afternoon surgery. These appointments are pre-bookable. If you feel you have many
things to discuss, please request a double appointment.
COMMUTER APPOINTMENTS - we offer pre-bookable commuter slots on Tuesday and
Wednesday evenings between 6.30 and 8.00pm
CLINIC APPOINTMENTS - we have a number of dedicated clinics run by our nurse
practitioner and practice nurses.
URGENT APPOINTMENTS - these are available every weekday with the Nurse Practitioner or
one of the doctors. These appointments are intended for same day medical problems requiring
urgent advice. They should not be requested to discuss routine health concerns. Unless you have
a life threatening emergency always consider contacting the surgery for minor injuries or urgent
medical advice rather than visiting the Accident & Emergency department.

Home Visits
If you require a home visit because you are too ill to attend the surgery, please make the request
before 10.30am by telephoning 01306 882422. Please be prepared to give the receptionist some
indication of the nature of the problem, as this helps the doctors plan their rounds more
efficiently. It may not always be possible for your own doctor to visit.
Remember that the surgery is specifically designed for the examination, investigation and
treatment of patients and the doctors are able to see several patients in the time it takes to
perform one home visit.

The timetable below indicates when each of the doctors are normally available to see patients
at Medwyn Surgery. Consultations can usually be booked from 8.30am until 11.40am and from
4.00pm until 6.00pm. Extended hours on a Tuesday and Wednesday evening

Patient Partner
We know that one of the most important issues to many of our patients is improving the
telephone access to the surgery and in particular you want to be able to




Phone to book an appointment at a time convenient to you
Avoid the morning rush and get through to a receptionist without too long a wait
Be offered options for different appointment times
Be able to cancel and check your appointments flexibly too
Bearing all this in mind we have installed an automated telephone appointment booking system
which is available to our patients 24 hours a day every day of the year, even when the surgery is
closed.
The system is called Patient Partner and allows you to book check or cancel your appointment
at any time. The system is very easy to use and just requires you to use your telephone key pad to
enter your telephone number and date of birth to identify yourself, and then it will offer you a
wide range of booking options. All the doctor appointments available to our receptionists will
also be available to book via Patient Partner.
We hope that this will mean that there will no longer be any need for so many patients to call as
soon as the practice opens in the morning, and that, with most of our appointments booked
automatically, our receptionists will be more able to respond quickly to the balance of more
complex calls e.g. regarding prescriptions and test results.
We recognise that many people do not like ‘speaking’ with a machine and prefer to speak to a
person, and that will still be an option. However we also hope that patients will come to realise
just how easy and convenient the system is to use and will enjoy the ease of booking with
Patient Partner.
If you plan to try the system you need to double check that the Practice has your correct
contact telephone number.
After that you can just phone the practice on our usual telephone numbers and listen and follow
the prompts to book an appointment with a GP or even some type of nursing appointments.

Missed Appointments
If you are unable to keep an appointment for any reason, please inform the surgery giving as
much notice as possible so that the appointment can be reallocated. Thank you.
In 2011, a total of 1250 appointments were wasted due to patients failing to show up and failing
to advise the surgery in advance. This is a massive waste of resources both in terms of time and
money and directly impinges on the service we are able to offer our patients. Please help us to
address this issue by cancelling any appointment that you are unable to attend. Cancellations can
be advised by phone, on-line or in person.
We appreciate there may be occasional instances where an appointment is missed in error.
However if you are found to be repeatedly wasting appointments without good reason or prior
notification to the surgery, you will receive a series of written and verbal warnings asking that
you improve your attendance. Failure to do so could result in your removal from the Medwyn
Surgery Listings. These measures have been endorsed by the Local Medical Committee.

Repeat Prescriptions
To request a repeat prescription please post or fax (01306 742280) your repeat prescription
request slip making sure you tick which items you require. Alternatively, you may write to us
stating the names of the medication and quantity needed or else order on line by following the
link on this page.
Your prescription will normally be ready for collection at the surgery after 48 hours. If you
would like your prescription posted back to you, please enclose a stamp addressed envelope with
your request. Please state whether you wish to collect your prescription from the surgery or from
one of the pharmacies in Dorking. If you wish us to send your prescription to a local pharmacy,
you will need to allow extra time.

Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2011, the charges are:



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.65, whereas a three month PPC will cost you £29.10 and a 12 month PPC £104.00.


We are now able to batch prescribe medication for those patients who are on long term
medication. Should you be interested in finding out more about this service, please ask you GP
or ask for details at reception.
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed.

Clinics
Clinics
The doctors and nurses at Medwyn Surgery are able to offer a wide selection of medical services
within dedicated clinics. This helps ensure we deliver the highest standards of medical care to
our patients.
Antenatal Care
Antenatal clinics are available with the midwife on Monday and Tuesday afternoons as part of
the shared care programme with East Surrey Hospital.
Asthma
Our practice nurses have been trained in the management of asthma and chronic lung disease.
Regular appointments are available on request.
Child Health and Development
Child immunisation and child health surveillance clinics are held every Tuesday afternoon from
2pm to 4pm with either Dr Louise Tomei or Dr Anne-Marie Quirk and the health visitor. They
can answer any concerns you may have regarding your child's development or immunisations.
Diabetes
Weekly diabetic clinics are held jointly with our practice nurse and Dr. Stewart Tomlinson on
Thursday afternoons from 1 - 3.30pm.
Minor Surgery
Minor surgical procedures are performed by the doctors on a Friday afternoon from 2-4pm.
Phlebotomy
This clinic runs on a daily basis. An appointment is required.
Smoking Cessation
Appointments available with a practice nurse who will provide support and advice on giving up
smoking. An appointment is required.
Travel Health with yellow fever
Specialist advice/immunisation for travel to all areas of the world can be provided by our trained
practice nurses. We are a registered yellow Fever Clinic. If possible, please contact the practice
nurse two months before your date of travel as some vaccinations take time to work and need a
course of treatment.
There may be a charge for some vaccines.

Research
Medwyn Surgery has been a member of the South Coast Primary Care Network since 2009. It
received accreditation as a Research Ready Surgery in 2010 by the Royal College of General
Practitioners. This signifies the practice has achieved 5 core competencies and has up to date
knowledge of the sandards that are needed to undertake research and physical facilities required
to successfully comply with demands based on the latest research governance frameworks.
You may be asked to consider participation within reseach studies at the practice. This helps to
support knowledge and understanding of modern health treatments. However, there is no
obligation for you to take part in research and if you choose not to you can be assured of
receiving our normal high standard of care.

Test Results
Test Results
Results of most blood tests will usually be available after two working days. Scan and x-ray
reports are not usually available until two weeks after the test was performed.
We can only give results to the person on whom the test was performed. Test results will not be
given to other family members unless they are for a child.
We would be grateful if you could request your results after 1pm when the surgery is less busy.
We are not able to email results directly to you as we wish to ensure that your confidentiality is
maintained.

New Patients
New Patient Registration
We welcome new patient registrations to Medwyn Surgery. Anyone who lives within our
catchment area can register with the practice. To register online click on the 'online registration'
tab above (you will need to sign the completed form when you come into the surgery for the first
time). If you want to register the whole family you will need to submit a separate form for each
family member.
When registering, please ensure that you bring the following documentation with you to the

When you have completed all of the details, click on the "Send" button to mail this to us. When
you visit the surgery for the first time you will be asked to sign the form to confirm the details
are correct.

As part of the registration process, you are required to complete the online registration form
AND the medical questionnaire below. This is because it can take a considerable time for us to
receive your medical records. There is an online version of this below which can be completed
prior to you visiting the surgery in order to save time at your first appointment. You will also be
asked to sign this form when you attend to confirm the details are correct.

Note that by sending the form you will be transmitting information about your self across the
Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect. You will NOT be registered at the practice if you have not completed the
registration form AND the medical questionnaire.

first visit to the practice.

Your Records
Your Medical Records
Doctors have always had the discretion to allow patients to see their health records and to share
information where appropriate with the carers of children and incapacitated adults. Additionally
in recent years Acts of Parliament have given certain statutory rights of access to records. None
of the legislation prevents doctors from informally showing patients their records or, bearing in
mind duties of confidentiality, discussing relevant health issues with carers.
The implementation of data protection legislation in early 2000 changed patients' statutory rights
of access to their health records. The purpose of this guidance is to set out in some detail the
legal requirements on doctors as holders of health records. This summary highlights the main
points.
What records are covered?
All manual and computerised health records about living people are accessible under the Data
Protection Act 1998.
Does it matter when the records were made?
No, access must be given equally to all records regardless of when they were made.
Does the Act cover all of the UK?
Yes.
Who can apply for access?
Competent patients may apply for access to their own records, or may authorise a third party,
such as their lawyer, to do so on their behalf. Parents may have access to their child's records if
this is in the child's best interests and not contrary to a competent child's wishes. People
appointed by a court to manage the affairs of mentally incapacitated adults may have access to
information necessary to fulfil their function.
Are there any exemptions?
Yes, the main exemptions are that information must not be disclosed if it:


is likely to cause serious physical or mental harm to the patient or another person; or
relates to a third party who has not given consent for disclosure (where that third party is
not a health professional who has cared for the patient).
Must copies of the records be given if requested?
Yes, patients are entitled to a copy of their records, for example a photocopy of paper records or
print out of computerised records.
Is it necessary for patients to make a formal application for access to see their
records?
No, nothing in the law prevents doctors from informally showing patients their records or,
bearing in mind duties of confidentiality, discussing relevant health issues with carers.
Can a fee be charged?
Yes, and the fee varies depending on the type of record and whether the patient wants copies of
the records or just to see them.

Records held totally on computer: £11
Records held in part on computer and in part manually: a reasonable fee of up to £50
Records held totally manually: a reasonable fee of up to £50
To allow patients to read their records (where no copy is required):




days when no charge can be made

Provide two types of identification as well as proof of current address. Photocopies
should initially be provided, but original copies must be available when collecting your

An initial administration fee of £11.00 will be charged. This must be paid by cash or

There may be an additional charge payable when you collect your results as detailed
above.
What about access to the records of deceased patients?
The Data Protection Act 1998 only covers the records of living patients. If a person has a claim
arising from the death of an individual, he or she has a right of access to information in the
deceased's records necessary to fulfil that claim. These rights are set out in the Access to Health
Records Act 1990 or Access to Health Records (Northern Ireland) Order 1993. The provisions
and fees are slightly different from those in the Data Protection Act.

Freedom of Information
The Freedom of Information (FOI) Act was passed on 30 November 2000. It gives a general
right of access to all types of recorded information held by public authorities, with full access
granted in January 2005. The Act sets out exemptions to that right and places certain obligations
on public authorities.
FOI replaced the Open Government Code of Practice, which has been in operation since 1994.

Data Protection and FOI – how do the two interact?
The Data Protection Act 1998 came into force on 1 March 2000. It provides living individuals
with a right of access to personal information held about them. The right applies to all
information held in computerised form and also to non-computerised information held in filing
systems structured so that specific information about particular individuals can retrieved readily.
Individuals already have the right to access information about themselves (personal data), which
is held on computer and in some paper files under the Data Protection Act 1998.
The right also applies to those archives that meet these criteria. However, the right is subject to
exemptions, which will affect whether information is provided. Requests will be dealt with on a
case by case basis.
The Freedom of Information Act and the Data Protection Act are the responsibility of the Lord
Chancellor’s Department. A few of its strategic objectives being:




To improve people’s knowledge and understanding of their rights and responsibilities
Seeking to encourage an increase in openness in the public sector
Monitoring the Code of Practice on Access to Government Information
Developing a data protection policy which properly balances personal information
privacy with the need for public and private organisations to process personal information
The Data Protection Act does not give third parties rights of access to personal information for
research purposes.
The FOI Act does not give individuals access to their personal information, though if a request is
made, the Data Protection Act gives the individual this right. If the individual chooses to make
this information public it could be used alongside non-personal information gained by the public
under the terms of the FOI Act.

Complaints
Making a Complaint
If you have a complaint or concern about the service you have received from the doctors or any
of the staff working in this practice, please let us know. We operate a practice complaints
procedure with conforms with NHS recommendations and national criteria.
We hope that most problems can be sorted out easily and quickly, often at the time they arise and
with the person concerned. If your problem cannot be sorted out in this way and you wish to
make a complaint, we would like you to let us know as soon as possible, ideally, within a matter
of days or at most a few weeks - this will enable us to establish what happened more easily. If it
is not possible to do that, please let us have details of your complaint:


Within 6 months of the incident that caused the problem or
Within 6 months of discovering that you have a problem, provided this is within 12
months of the incident.
Complaints should be addressed to the Practice Manager or any of the doctors. Alternatively, you
may ask for an appointment with the Practice Manager in order to discuss your concerns. He will
explain the complaints procedure to you and will make sure that your concerns are dealt with
promptly. It will be a great help if you are as specific as possible about your complaint.
What we will do
We will acknowledge your complaint within two working days and aim to have looked into your
complaint within ten working days of the date when you raised the complaint with us. We shall
then be in a position to offer you an explanation, or a meeting with the person(s) involved. When
we look into your complaint, we shall aim to:



Find out what happened and what went wrong
Make it possible for you to discuss the problem with those concerned, if you would like
this
Make sure you receive an apology, where this is appropriate

Identify what we can do to make sure the problem does not happen again
Complaining on behalf of someone else
Please note that we keep strictly to the rules of medical confidentiality. If you are complaining
on behalf of someone else, we have to know that you have his or her permission to do so. A note
signed by the person concerned will be needed, unless they are incapable (because of illness) of
providing this.

Ensure our patients have 24-hour access to medical advice.
Aim for you to have access to a suitably qualified medical professional within 48 hours
of your initial contact during surgery hours, or in an urgent case, the same day.
Work in partnership with you to achieve the best medical care possible.
Involve you and listen to your opinions and views in all aspects of your medical care.
The prevention of disease, illness and injury is a primary concern.
The medical staff will advise and inform you of the steps you can take to promote good
health and a healthy lifestyle.

Treat staff with courtesy and respect.
Inform the practice staff of any alterations in your circumstances, such as change of
surname, address or telephone number. Please ensure that we have your correct telephone

GP Training
Medwyn Surgery is an accredited NHS training practice for undergraduate medical students and
postgraduate doctors (Specialist Training GP Registrars).
GP registrars are fully qualified doctors who have completed hospital training and wish to pursue
a career in general practice. They spend up to twelve months at Medwyn to improve their
consulting skills and widen their experience in primary healthcare. The GP registrars hold daily
surgeries like the other doctors at the practice and are mentored by a GP trainer. Dr Chris
Monella, Dr Stewart Tomlinson and Dr Raj Sekhon are all approved trainers for the Kent, Surrey
and Sussex Postgraduate Deanery.
Occasionally, medical students from St George's Hospital in London will be attached to Medwyn
Surgery. During their time at the practice they will observe consultations with the doctor or
nurse. This is an essential part of training the next generation of doctors who could be
responsible for your future care. Reception staff will always ask if you agree to a medical student
oberving your consultation. If you would prefer not to have a student present during your
consultation please let us know.

EDICS Back Pain Clinic
This Back Pain Service was established at Medwyn Surgery in 2004 by Dr Chris Monella to treat
patients in the Surrey area.
On your first visit to the clinic you will be assessed by one of our experienced practitioners and
investigations will be organised to establish a cause for your symptoms within a jointly
agreed management plan
The back team works closely together to assure you of the best and most appropriate treatment
for your symptoms and a comprehensive discharge plan will be advised to your GP.
We offer a holistic one stop service comprising physical, psychological, medical and surgical
therapies in an integrated manner without the delays inherent in re-referral. Further information

The Quality and Outcomes Framework (QOF) is a component of the new General Medical
Services contract for general practices, introduced from 1 April 2004. The QOF rewards
practices for the provision of quality care, and helps to fund further improvements in the delivery
of clinical care.

and against a range of indicators covering practice organisation and management. Practices score

Friends of Medwyn
The staff of Medwyn Surgery have a very clear objective.They want to provide their patients
with the best possible service, not only in medical care, but also in terms of health information
and awareness, the environment of the surgery and doctor / patient relationships. As part of this
initiative, we have formed a Patient Participation Group & this group is called "Friends of
Medwyn".

The functions of the Friends of Medwyn will be:

To provide a means of communication between patients & Medwyn staff. Not for
individual issues (which are, of course, a matter between a patient & his or her G.P.), but

To provide information on medical issues (e.g., background medical information,
sources of information such as leaflets & internet websites, new clinical developments,
support groups for various medical conditions, NHS news, etc.)
To keep patients up to date on Medwyn surgery itself (e.g. new administrative
procedures, new equipment, new staff etc.)
To raise money for the Medwyn surgery. The money we raise will go towards
providing equipment & facilities to Medwyn, for the benefit of patients & for the surgery
as a whole. We would be grateful for any donations and, if you would like to make a
donation, please send or take a cheque to the surgery & make your cheque payable to

Medwyn Centre Partners
Medwyn GP Surgery is only one part of the larger Medwyn Centre development. The building
development was driven and funded by the GP Partners who had a vision of providing Dorking
with a first class facility from where a range of medical and support services could be provided
to local residents 'closer to home'.
We are pleased to say that there is now a range of services based at Medwyn and all are run by
independent healthcare providers, operating entirely separately from the Medwyn GP Surgery.
Some organisations have a full time presence at the Medwyn Centre, such as the community
nursing teams, NHS dentists, physiotherapist and podiatrist. Other healthcare providers and
counsellors run clinics at Medwyn on a sessional basis.

Sue Rees is a qualified Nurse Practitioner and as such is an alternative first
point of contact for patients suffering with minor ailments and injuries, such
as chest, eye and ear infections, simple sprains and backache. In addition to
her role as a Nurse Practitioner, Sue has a special interest and training in
diabetes. She runs a weekly diabetic clinic with Dr Tomlinson

The Practice Nurses undertake a wide range of nursing duties
including dressings, blood tests, ECG's, smears, ear syringing. In
addition each nurse has an area of special interest. Carol and Nikki
also offer travel advice and vaccinations.

Kirstine Hall, our practice manager is responsible for the efficient
administration of Medwyn Surgery. Her aim is to provide expert and
friendly healthcare to you and your family. Your suggestions and ideas
are always welcome.
Administration
Our administration team support the clinical and reception staff in managing the
day-to-day running of the surgery. They provide an essential role in helping you
with your queries and ensure the efficient functioning of the surgery.
Reception
Our reception staff provide your first point of contact with the surgery. They are
here to support both the clinical team and yourselves. They are here to deal with
your enquiries and will make every effort to help you.
Community Midwives
Midwives from East Surrey Hospital hold twice weekly antenatal clinics at the
surgery. All aspects of care for your pregnancy are provided including parentcraft
classes. There is also a health visitor attached to Medwyn Surgery. She is able to
provide advice on all aspects of your childs development, as well as providing

Community Nurses
The district nurse team are employed by Central Surrey Health. Their priority is to
those patients who are housebound; assessing their diverse needs, delivering care
and liaising with the doctors and other members of the primary healthcare team.
