INTRODUCTION
This paper outlines the decisions made by the Scottish Health Survey team regarding
changes to the questionnaire for the 2012-2015 surveys.
A consultation period was launched on 21st March and ended on 15th April 2011. A
spreadsheet of existing question topics was provided, with links to the
questionnaires, and people were invited to submit proformas to either retain,
amend or add questions for the period 2012-2015. Details of the consultation were
available on the Scottish Health Survey website and an email was sent to the
ScotStat register inviting them to take part in the consultation process. Thank you
to all who contributed at this stage of the consultation.
The proformas were considered in relation to their: policy relevance; weight of
support; use for indicators/targets; availability from other sources; frequency and
geography required; length of question and whether questions had been previously
tested.
Following discussions with the contractors, initial recommendations were made for
inclusion of questions in the questionnaire and the contractors proceeded to
cognitive testing in August. Further changes were made based on the results of the
cognitive testing and the new questionnaire was piloted in September. The pilot
provided essential information on question timings and led to changes to the order
of questions and, in some cases, the mode of questioning. The dress rehearsal took
place in November and has led to further minor changes to the questionnaire before
it is launched in January.
This paper summarises the changes which have been made to the questionnaire for
2012-2015 following the process set out above.
2.
TOPIC AREAS
2.1
A1 Caring Responsibilities
The existing questions on carers will be retained with a few minor amendments
including:
 questions to be extended to children (4-15) as well as adults;
 extending the question on who is cared for to identify separate options
outwith the household;
 a minor change to the question on hours spent providing care to remove
some categories and align with the census 2011 question.
Three new questions will be added on length of time caring; impact of caring on
employment and support for carers.

B1 CVD Conditions, High Blood Pressure and Diabetes

Questions to establish prevalence, doctor diagnosis and whether condition has
occurred during the past 12 months will be retained along with questions on
whether medication is being taken for the condition. Questions on age of onset of
diabetes and insulin injecting will also be retained.
Questions on surgery and other treatment for conditions will be dropped due to
limited evidence of their use.

B2 Self-reported Blood Pressure and Cholesterol Measurements
Questions will be dropped.
2.4
B3 Blood Samples
No venous blood samples will be collected in 2012 but it is planned to introduce dry
blood spot measures in 2013 for as many of the current analytes as is feasible.

B4 Blood Pressure Measurement
This will continue to be collected but will be measured by the interviewer rather
than a nurse. The protocol will therefore change slightly and a comparability study
will be carried out in Spring 2012 to assess the effect, if any, of this change.

B5 Rose Angina Questionnaire
Questions will be dropped for 2012-2015 cycle with consideration given to
reintroducing at a later date.

B6 Prescribed medicines and drug coding
Questions will be moved from the nurse visit to the interviewer administered bio
module. As is done currently, all respondents are asked if they take any medication
etc prescribed by a doctor or a nurse:

The questions about muscle strengthening and balance improving relate solely to
sporting activities, so other kinds of activity (such as housework, gardening or workbased tasks) will not be counted (the frequency and duration of such activities will
still be included in survey).
The current question on participation in a list of 10 sporting activities will remain. A
new question with additional sporting activities will be added. It asks whether
people have done any of the following in the past 4 weeks:

The questionnaire currently asks all adults about their usual walking pace as a proxy
for the level of exertion. However, concerns were raised that this is not suitable for
some older people as they may walk at a slow pace but still exert themselves. A new
question for people aged 65 and over has been added:

enough to make you breathe faster, feel warmer or sweat?

person (HRP) only in module A and are not used to derive the variables on
adherence to physical activity recommendations. There has been no evidence of
these variables being used in their own right.
Note that there are questions on physical activity at work asked of all adults in
section E1 (see above) which are used to derive the variables on adherence to
physical activity recommendations and these will be retained.

No case was made for the retention of these questions during the questionnaire
consultation. Therefore they will not be included in the 2012-2015 questionnaires.

questions on wider sedentary activity at work and in leisure time will also be
included.

KNOWLEDGE, ATTITUDES AND MOTIVATIONS TO HEALTH (KAM) MODULE
This module is currently funded by NHS Health Scotland and they have indicated that
they do not wish to continue the collection of this data from 2012 onwards. We
received a small number of requests to continue individual questions from this
module during the consultation.
None of these questions will be included in the 2012-2015 surveys. This is due to the
different design required for these types of questions. They need to be asked of a
random adult in the household rather than all adults. Without a full module of this
type of questions, it is not feasible or efficient to include this in the design for a few
questions.

HARMONISED CORE
The development of a harmonised core of 19 questions across the three large
Scottish population surveys (Scottish Household Survey, Scottish Health Survey and
Scottish Crime and Justice Survey) has led to some minor changes to existing SHeS
questions and some additions. Many of the harmonised core questions already exist
in the SHeS. All are listed below for completeness.
