Ph.D. STUDENT PROGRESS REPORT and FUNDING APPLICATION
Deadline: February 5, 2016





Instructions:
All PhD students, including ABDs (all but dissertation), must complete the attached progress report.
Please email a copy of this report and a current CV to Carol Cottrill at carol.cottrill@uky.edu edu by
February 5, 2016.
Please provide a hard copy of the progress report and CV to your advisor for their review and signature
by the deadline. This signed hard copy (including your CV) should be submitted to Carol no later than
February 5, 2016. If possible, you should plan to meet in person with your advisor to go over the report
and your CV as this serves as a good opportunity for an annual check-in.
Students who wish to be considered for funding must also include the following items in their progress
report:
 A one-page teaching statement
 Copies of the TA coordinator's evaluations for each previous semester the student has served as a
TA. You should keep your own copies of these evaluations for each previous semester.
However, if you have failed to do so then Carol should be able to provide you with a copy.
 TCE scores and a sample of student comments for each course a student has previously served as
a TA for. You should be able to access your TCEs online at
http://www.uky.edu/iraa/teachercourse-evaluation-results. If your results are not posted there
then you can check with Carol to see if the department has them on file.
 In addition to these materials the Graduate Committee considers GPA, timely progress toward
degree (according to the sample Ph.D. timeline), and scholarly activity when allocating funding.
