The purpose of this chart is to help family child care providers develop an individualized plan for achieving
accreditation through NAFCC. Please complete this chart and attach it to your accreditation waiver
application.

three years of observation.

* The process should be completed within three years of initial involvement with Community
Partnerships for Children. NAFCC should schedule an Observation and Interview visit in a timely
fashion, but any delay on the part of NAFCC does not jeopardize the provider’s involvement with
CPC.
