Child Immunisation/Health Surveillance A Clinic is held every Wednesday morning with the Doctor, Nurse. All Doctors are approved for Child Health Surveillance. Prevention of Heart Disease Clinic A clinic is held with the Nurse who discusses the benefits of a healthy lifestyle and how you can reduce the risk of heart disease. Asthma Clinic and COPD Clinic ATTACHED STAFF District Nurses There is a team of District Nurses attached to the practice and are based at Yewlands Drive Clinic. The District Nurse deals with patients who have recently been discharged from hospital or have been referred by the Doctor. They can be contacted on 01772 644100. Our Health Visitor is usually able to help with health matters relating to the under 5’s. She is based at Yewlands Drive Clinic and Wade Hall Clinic and can be contacted on 01772 644115. Diabetic Clinic Midwife This is held with the Nurse who reviews Diabetic patients annually and checks on their progress. The Community Midwife assists in running the Ante-Natal Clinic every Thursday morning. She also visits patients at home and attends our maternity patients, after discharge from hospital. Minor Surgery The surgery offers a minor surgery service. Dr Kanitkar currently performs joint injections and minor surgery on Tuesday mornings. HEALTH PROMOTION CLINICS Vaccinations We hold clinics for men, to advice on health matters. We ask and advise about your lifestyle, check height, weight, and blood pressure and test a urine specimen. This gives you the opportunity to ask questions regarding your health matters. Each year the practice offers flu vaccinations to patients at risk. These include the elderly and those who have diabetes, asthma, COPD and heart problems. Shingles vaccination for the over 70’s. NHS travel vaccinations. These vaccinations are given by the Nurse and HCA. Non NHS Services Medical examinations for insurance, preemployment, fitness to travel, private prescriptions, private certificates and completion of insurance claim forms can be arranged with the Doctor. As these are not NHS services, fees are charged according to BMA guidelines. Details of current fees are available at reception. The practice employs three Receptionists; they are qualified, competent and caring and are bound by the same confidentiality rules as the Doctors and Nurses. The receptionist will endeavour to offer you an appointment of your choice at a suitable time. Please inform us as soon as possible if you are unable to attend an appointment. Urgent Appointments If you need a Doctor urgently, please inform the receptionist. If the Medical Centre is closed, please tele- phone the emergency doctors on 01257 418010. Home Visits Please request Home Visits only if it is absolutely necessary. Telephone the surgery before 10.00am, giving as many details as possible to the receptionist. Doctors can then plan their visits so urgent cases are seen first. Telephone Consultations If you need to speak to the Doctor, a telephone consultation can be arranged. The receptionist may ask for information to pass to the Doctor. When ordering repeat prescriptions, please use the tear off slip with your computerised prescription and mark all items needed. If you do not have the tear off slip, please write your name and address and the items needed on a piece of paper. You can then give to receptionist, post through the letter box or send through the post. Or order using EPS – Electronic Prescription Service or order online. Please allow at least 48 hours for your prescription to be issued. The Practice has a Medicine Co-ordinator who is responsible for adding, removing and changing any medications prescribed to patients once approved by the GP. She also deals with hospital discharge medication once approved by the GP. PRESCRIPTION ORDERS WILL NOT BE TAKEN OVER THE TELEPHONE EXCEPT FOR THE ELDERLY, HOUSE BOUND OR DISABLED. Any queries you have regarding your medication please contact the medicines co-ordinator on the surgery number. Medicine Co-ordinator THE PRACTICE TEAM Practice Manager The Practice Manager is responsible for the smooth running of the practice and is available to discuss any suggestions, compliments or complaints you may have. Practice Secretary The practice employs a secretary who is responsible for the referral letters, summarising letters, reports, booking meetings and general secretarial duties. Practice Nurse The practice employs one Practice Nurse who is a Registered General Nursing trained. She is qualified to advise on a range of matters and carry out a variety of procedures including immunisations, ear syringing, blood pressure checks, annual reviews, smoking cessation clinics and health promotion clinics. She also offers foreign travel advice and immunisations. Health Care Assistant The practice employs one HCA who is trained to carry out a variety of procedures including new registration checks, blood pressure checks, NHS Health checks, ECG, blood tests, INR monitoring, Flu injections. Note Summariser The practice has a member of staff who is responsible for summarising new patient notes onto the computer system. This will enable the doctor to have a full medical history available to him when you visit the surgery for an appointment. Practice Administrator The practice employs an administrator who is responsible for Medical and Insurance reports, referral letters, summarising letters and general admin duties. GP Trainees and Medical Students The surgery has been accredited by the Royal College of GPs and Northwest Deanery as a teaching and training practice for medical students, FY2 trainees and GP trainees.
