Annual Fire Safety Report: Fire Statistics & Policies Fire Report Arlington Medical Institute reported no fires on campus during the year of 2012. There were no fire-related injuries, deaths, or property damages in its residential facilities for the year 2012. Fire Safety Alarms Arlington Medical Institute does not have on-campus student housing facilities and consists of one location. The fire safety systems for each are listed below. a. Reception Area of “The Neighborhood Doctor” - Fire/Smoke Alarms & Sprinklers. Fire Extinguishers. b. Main Entrance Hallway - Fire/Smoke Alarms and Sprinklers. Fire Extinguishers. c. Staff Break Room- Alarm System. Fire Drills During 2012-2013, Arlington Medical Institute performed one documented fire drill for the school. Policy on Portable Electric Appliances, Smoking and Open Flames in the School or Designated Break Room Area The Office of Environmental Health & Safety performs Residence Hall Health and Safety (H&S) Inspections five times a year, twice in the fall and in the spring and once in the summer. Inspections will be announced only during the first round of the fall semester. All other inspections are unannounced. The school inspections at Arlington Medical Institute are primarily designed to find and eliminate safety violations. Students are required to read and comply with the Annual Fire and Security Guidelines, which include the school inspections and all other rules and regulations for evacuation procedures and businesses located next door. The inspections of Arlington Medical Institute include, but are not limited to, a visual examination of electrical cords, smoke detectors, fire extinguishers and other life safety systems in both the clinic “The Neighborhood Doctor” and the school “Arlington Medical Institute (AMI). While cooking is not permitted in the student break room, small appliances that are approved and in good condition may be used, except Dutch ovens, to Small refrigerators (less than six cubic feet) may also be used, with a limit of one per room. One microwave is permitted in the student break room and one microwave is permitted in the employee break room. In addition, each room will be examined for the presence of prohibited items (e.g., sources of open flames, such as candles; non-surge protected extension cords; halogen lamps; portable cooking appliances in non-kitchen areas; etc.) or prohibited activity (e.g., smoking in the room; tampering with life safety equipment; possession of pets; etc.). This inspection will also include a general assessment of food and waste storage and cleanliness of the rooms within Arlington Medical Institute. Prohibited items will be immediately disabled with a locking device or confiscated and donated/discarded if found, without reimbursement. 1 Procedures for Evacuation of Students, Employees and Faculty in the Case of a Fire In the event of a fire, the Arlington Medical Institute expects that all campus community members will evacuate by the nearest exit, closing doors and activating the fire alarm system as they leave. Once safely outside a building, it is appropriate to contact 911 and the Arlington Police Department at 817-459-5500. Students and/or staff are informed where to relocate to by staff if circumstance warrants at the time of the alarm. In the event fire alarms sound, Arlington Medical Institute policy is that all occupants must evacuate from the building, closing doors as they leave. No training is provided to students or employees in firefighting or suppression activity as this is inherently dangerous and each community member’s only duty is to exit safely and quickly, shutting doors along the exit path as they go to contain the spread of flames and smoke, and to activate the alarm as they exit. At no time should the closing of doors or the activation of the alarm delay the exit from the building. Fire Safety Education and Training Programs Provided to Students, Employees, & Faculty Members Fire safety education programs for all students that attend Arlington Medical Institute or have any association with the school or clinic (The Neighborhood Doctor) are held at the beginning of each module, which is five weeks long. These programs are designed to: familiarize everyone with the fire safety system, train everyone on the procedures to be followed in case there is a fire and distribute information about Arlington Medical Institute’s fire safety policies. Everyone is also provided with a map of the school and clinic that illustrates evacuation routes and fire alarm equipment locations. During these programs, trainers emphasize that participating in fire drills is mandatory. Students with disabilities are given the option to have a “buddy” assigned to them. Fire safety education and training programs are taught by the School Director and faculty members who are experienced in fire safety matters. On an annual basis, faculty, students, and employees of “The Neighborhood Doctor” receive training, which includes fire safety awareness, emergency response, and disaster preparedness. This includes how to use a fire extinguisher, how to report a fire or other emergency, and fire prevention. If a fire occurs, students are instructed to leave hazardous areas per the evacuation routes and get to a predetermined location before calling 911 for help. They are to remain in that location so that the faculty or a person from the admissions department someone has documented that the student has left the building. Evacuation of Building Leave your building immediately when an alarm sounds or if you are instructed to do so by the President or faculty member. If you are away from your own area, leave immediately by the nearest safe exit and go to the nearest evacuation meeting site. The Arlington Fire Department Public Safety will secure the building (where the emergency is taking place) by evacuating students and guests to the designated evacuation area as follows: Out the front door to the grassy knoll west of the entrance If exiting the back door in the student classroom go around to the front and walk towards the grassy knoll west of the entrance. 2 Notify others who might not have heard the alarm or evacuation order. 1. Turn off equipment. 2. Secure hazardous operations if possible. 3. Take important personal items, such as keys and cell phone. 4. Close doors behind the last person out. 5. Walk quickly, but do not run, to the nearest safe exit. 6. Do not use an elevator unless authorized emergency personnel tell you to do so. 7. Do not re-enter the building until someone in authority gives the "all clear" signal. 8. Report any missing or trapped persons to emergency personnel. 9. Move away from the building. 10. Go to your evacuation meeting site and sign in. 11. Wait at the evacuation site for directions If you are required to leave the building immediately but are unable to (because of a physical disability, injury, or obstruction): Follow the evacuation plan or go to the nearest area where there are no hazards. Dial 817-459-5500 to notify the Arlington Fire Department of your situation and location. Be sure to give the name of the room or number so the Arlington Fire Department can send help. Signal out the window to emergency responders, if possible. Emergency Response Emergency response evacuation is tested at least once per calendar year. These tests may be announced or unannounced and will be documented with the date, time, and whether it was announced or unannounced. ALARM – The Arlington Police Department will control the alarm engagement and disengagement. Fire extinguishers are located in the reception area of the clinic and main entry way of the school. Each individual unit has a smoke activated alarm. Only the Arlington Fire Department may assist in the evacuation process for the critical building(s). EMERGENCY PHONES – Emergency phones are located in “The Neighborhood Doctor” clinic and offices. KEYS - Additional keys are located to both Arlington Medical Institute and The Neighborhood Doctor in a secured lock box in the clinic. Other master keys for the school or clinic are in the possession of Owner and School Director. TRIAGE AREAS - All medical emergencies will either be treated by the Arlington Fire Department at the site, transported to a local hospital, or be treated for minor medical incidents at the South location, The Neighborhood Doctor. In the event these medical facilities are not available alternate hospitals will be provided. PLEASE SEE PAGE 6 FOR PHONE NUMBERS AND ADDRESSES IN CASE OF AN EMERGENCY 3 Emergency Plans The School Director, Program Coordinator, President, Director of Admission, and Financial Aid Administrator will have emergency information including floor plans and a student roster of each area in the building. The School Director will name off the students in their respective building or area to confirm that all students have been evacuated. 1. The faculty and staff will assist in the evacuation of the surrounding buildings, and the entire complex as dictated by the Arlington Fire & Police Department. 2. The P resi dent and S chool Di rect or staff will have rosters of the critical areas in the building. The evacuation area will be attended by a faculty member who will verify present individuals and the whereabouts of those not present. Notations of those present and the whereabouts of those not present will be made on the floor plan. 3. Rosters of the critical buildings will be available for the Fire or Police Department for verification of a students’ location. The faculty, staff, and students will assist the local authorities as requested. 4. When the Fire or Police Department has authorized security to allow student’s residents of student’s entry back into the buildings, the School Director or faculty member will notify the students in the evacuation area. Weapons, Explosives, and Fireworks on AMI Property For the safety of our students, employees, and visitors on campus, Arlington Medical Institute’s policy prohibits the use and possession of weapons, explosives, and fireworks. Use, possession, display, or storage of any weapons, explosive device, or fireworks are prohibited on all land and building owned, leased, or under the control of Arlington Medical Institute or its related entities, or Arlington Medical Institute’s events. Arlington Medical Institute students and employees are required to alert a faculty or staff member. . If faculty or staff members are not present immediately dial 911 for an emergency from any of the phones located in the school or the clinical area if. The Arlington Police Department can also be reach at 817-459-5648. Any suspicious activity or person seen in the parking lots or loitering around the vehicles, near “The Neighborhood Doctor” , inside the school in the hallways, restrooms, or break area please report immediately to a faculty member or 911. You may also report suspicious activity to any of the three phone numbers listed below for the Arlington Police Department. Please call 911 first if the situation is an Emergency. Any person who trespasses, vandalizes property from AMI or ‘The Neighborhood Doctor”, or is a threat to patients and students will require outside help from the Arlington Police Department which can be reached immediately at 911 for emergency or from any phone in the school or clinic area please dial one of the phone numbers below. Future Improvements in Fire Safety to Current Employees and Enrolled Students Fire Safety has always been and continues to be a high priority at Ramapo College. We continue to assess what more can be done to: Assure a Fire Safe Environment Raise our Fire Preparedness and Prevention Renew the Fire Safety Instructions and Fire Evacuation Procedures Update Available Training and Information At minimum an annual review of Arlington Medical Institute’s Fire Safety Policies and Procedures will be conducted, inspections and tests of our Fire Suppression and Protection, and continue on an annual schedule for any deficiencies. Arlington Medical Institute conducts routine Fire Drills throughout the year. These drills play a vital part in planning and evaluating how quickly and safely the student can evacuate the building. Each room is checked and the reports the names of those still found. Records of the fire drills and schedule of drills are kept on file at Arlington Medical Institute.
