Kamp Canine
Standard Agreement – Day Care & Boarding
Without Prejudice
1.
Kamp Canine - Dog Lodging & Pet Supplies Inc. (hereinafter called “Kamp Canine”) agrees to exercise
due diligence and reasonable care, and to keep the premises sanitary and properly enclosed. All Pets are
handled or cared for by Kamp Canine staff without liability on Kamp Canine’s part for loss or damage
from disease, theft, fire, death, escape, injury or harm to persons, other pet(s) or property by said pet, or
from other unavoidable causes, due diligence and care having been exercised.

Should any pet become ill or seem to be in need of medical consideration, Kamp Canine reserves the
right to administer aid and/or to use any available veterinarian. Any expenses so incurred shall be paid
by the Owner of said pet in addition to other fees incurred for services provided at or by Kamp Canine.

The Owner agrees to pay the rate for services in effect on the date their pet is checked into Kamp
Canine. Prices are subject to change without notice. No pet will be released until all charges are paid in
full. Cancellation fees may be applied for reservations cancelled less than 24 hours prior to a scheduled
reservation. Kamp Canine reserves the right to charge for excessive damage to property caused by pet.
The Owner shall remain liable for all charges incurred for the care and maintenance of the pet listed on
this contract. It is expressly agreed that Kamp Canine’s liability shall in no event exceed the lesser of
the current tangible value of a pet of the same species or the sum of $500.00 per animal. The Owner
further agrees to be solely responsible for any and all acts or behavior of said pet while in the care of
Kamp Canine.

Pet must be in good general health and remain current on vaccinations for: PLEASE INITIAL

*DA2PPV (Distemper, Adenovirus, Para influenza, Parvo virus)
*Bordatella (Kennel Cough/Hepatitis)

The Owner must provide valid proof of all required vaccinations prior to the pet’s first visit, when vaccinations
are updated, and /or annually. Pet must also be on a scheduled prevention program during flea and tick season, or
when specifically requested by Kamp Canine. Pets arriving with fleas and/or ticks may be bathed at Kamp
Canine’s discretion, at the Owners expense. Kamp Canine reserves the right to refuse services or admittance to
any dog. Vaccinations provide reasonable protection against contraction of certain diseases and infections.
However, animals in close contact may still carry and transmits these infections and others. Kamp Canine will
take reasonable steps to ensure pets are in good health prior to their stay and assumes no liability for subsequent
medical costs incurred by owners.

5.
The Owner agrees that their pet may be videotaped, photographed, and/or recorded. Kamp Canine shall
be the exclusive owner to the results and all proceeds of such taping, photography, and recordings with
the rights throughout the world, and unlimited number of time in perpetuity, to copyright, to use and to
license to others in any manner. The Owner further agrees that their pet may be used in any and all
media and in the promotion, advertising, sale and publicizing of Kamp Canine.
