APPLICATION CHECKLIST
Please review this checklist to ensure you have provided the following items:





Completed and signed application and forms (both applicant and coapplicant)
Letter of application stating your need for a Habitat home and
willingness to partner with us
Social Security Authorization to check credit records, employment,
bank balances, background and sex offender check, and Social
Security Income (both applicant and co-applicant)
Copies of federal tax returns from the past two years (please do not
provide originals)
Copies of W-2’s from the past two years (please do not provide
originals)

Copies of employment pay stubs from the past two months

Copies of bank statements from the past two months

Explanation of any bankruptcy, judgment, or other financial problem
Make a copy of your completed application for your records.
