B-412 # 1946 After Mission Report

1. Reference: PSG Flight Directive dtd 07 April 2009.
2. Per above reference, Bell 412 # 1946, after arriving from Busuanga at 250th PAW
ramp at around 0930H on 07 April 2009, departed for Area 3, Malacanang Park on the
same day at 1200H to pick up Presidential son Congressman Miguel “Mikey” Arroyo
and his family and flew them to Loakan Airport in Baguio City. Bell 412 # 1946 departed
Area 3 at 1330H and arrived Loakan Airport at 1440H.
3. Bell 412 # 1946’s line up for this mission were:

4.At around 1525H, Chief, Group Operations Center, PSG, MAJ GLEN LA
GUARDIA PAF informed Wing Commander, 250th PAW, thru cell phone, that one heli
is needed for ocular inspection (CAPA) by Usec Frostrum and party of Halsema
Highway and Banaue that afternoon in preparation for Presidential movement to those
places the following day (08 Apr 09).
5.Since Bell 412 # 1898 and S-70 # 739 just shutdown engines at Baguio Mansion,
Wing Commander, 250th PAW instructed B-412 # 1946 pilots to fly the mission at
1530H. However, Usec Frostrum and party only arrived Loakan Airport at 1615H. Thus
B-412 1946 was able to take off from Loakan at 1620H leaving behind Sgt Cabesas.
6.Passengers of B-412 # 1946 from Loakan to Halsema and Banaue were the following:

7.C,GOC called COL MANCITA, Director for Operations, 250th PAW at o/a 1745H
asking for help to locate 1946 since it has neither arrived in Banaue nor returned to
Loakan airport or Mansion. There was also no phone contact with the pilots and
passengers.
8. COL MANCITA immediately contacted TOG 1 and TOG 2 to call Police and Army
in their areas for assistance to locate the missing 1946.

Update on Missing B-412 # 1946
TO:

1. At around 2000H 08 April 2009, a fixed wing aircraft was seen flying over
Baguio City and was contacted by CO MANCITA at 122.3 VHF frequency
through an air band radio. It turned out to be an F-27 with tail number 669 piloted
by CAPT MAGAYANES of 220th Airlift Wing.
2. The pilot reported that he was getting a distress signal on 121.5 VHF frequency at
010 deg 3.7NM from Loakan airport. The distress signal was also clearly heard by
the President and other civilian and military VIPs listening at 122.3 when the pilot
pressed the radio trigger switch while flying over the source of the distress signal.
3. Plotting the reported direction and distance on air navigation map and Google
Earth showed that distress signal was located at N 16 deg 28.402’ E 120 deg
37.952’ which is near the Halsema Hwy toll gate.
4. The Presidential Security Group immediately dispatched a group to check the
probable distress signal source, but no helicopter was found.
5. Two night vision goggles equipped Huey helicopters also scoured the said area at
0400H, but no helicopter or its wreckage was found.
6. At 0930H and 1330H, S-70 # 739 with COL LARGO and MAJ MENOR as pilots
and co-pilot respectively; and heli mechanics Ssgt Macalma and Ordinario; COL
MANCITA acting as navigator; and five 505th SRG parajumpers (PJs) joined two
Hueys in the search around Mt Pulag and vicinity, but to no avail.
7. When S-70 # 739 landed at Loakan airport after the second sortie at around
1500H, news from the PNP revealed that B-142 # 1946’s wreckage has been
found at Sitio Mangingihi, Barangay Eheb, Tinoc Ifugao.
8. Two US CH-46 helicopters tried to drop PJs in the reported crash site, but clouds
covering the site prevented them and returned to Loakan airport at around 1600H
without dropping the rescuers, and decided to rest for the day.

Update on B-412 # 1946

1. Seven bodies were found by rescuers who went to the site by land and on foot late
in the afternoon of 08 April 2009, which were all brought to Tinoc proper during
the night for identification.
2. Said fatalities were picked up by 2 Hueys and a Huey 2 from Tinoc Airstrip at
around 0730H and unloaded at TOG 1 for blessings with the presence of PGMA
and some of the fatalities’ family members.
3. The seven cadavers were loaded in aluminum caskets and ferried by a C-130 to
Villamor Air Base.
4. The eight fatality was also found early that morning and flown to Villamor at
around 1900H on board F-27 # 669.
5. All eight fatalities were later brought to Heroes’ Hall in Malacanang for
Necrological services with the President and other government servants.
