2015 NCAPOA Scholarship

In honor and memory of Berkeley Police Officer Ronald Tsukamoto

Awarded to full-time college student s who have an interest in a career in law enforcement and in need of financial assistance with their educational goals.

Please submit this completed application in typewritten form, or printed in black ink by the above deadline date. The information furnished by the applicant is considered confidential, and will be reviewed by the Northern California Asian

Peace Officers' Association (hereinafter referred to as NCAPOA) Scholarship

Committee for truthfulness, accuracy, and completeness. All materials submitted by the applicant shall be used for the sole purpose for consideration of this scholarship award.

The applicant must sign this application and return it with a certified copy of transcripts or report card/progress report.

A printout of your online transcript will be accepted with your application if a certified copy is not yet available. Please obtain a certified copy to bring with you by the date of the interview. (Signature not required for emailed applications, but will assume that the waiver is accepted and a signature will be requested at the time of interview.)

Any misrepresentation of facts and/or information will result in immediate disqualification of the candidate for scholarship consideration.

The NCAPOA Scholarship Committee renders equal consideration to each applicant's academic merit, community service, and financial need. It is important the applicant submit a candid and honest financial appraisal of his/her college costs. The information provided herein shall be held confidential. Failure to submit this information will postpone processing of your application until the information is received. If this delay causes the application deadline to pass, your application WILL be void and ineligible for consideration.

In consideration for this scholarship award, I understand that I must provide official academic records including grade reports, transcripts, and attendance/admission records to the NCAPOA Scholarship Committee. The documentation will be included with this scholarship application. I understand that this information will be used for verification purposes only. I also understand that any incomplete areas in this application may delay the processing of my scholarship request.

If this delay goes past the deadline date of January 3, 2015, I understand that the

NCAPOA Scholarship Committee may consider my application as null and void.
