HEALING OUR WATERS Administered by Freshwater Future EASTERN LAKE MICHIGAN 2015 GRANT APPLICATION FORMAT Application Information This application format is to be used by applicants to the 2015 Healing Our Waters Eastern Lake Michigan Request for Proposals. Complete information regarding this grant program, including eligibility, types of activities funded, priority areas, evaluation criteria, and application process and timeline are included in the 2015 Request for Proposals. Application Instructions Type your application in Microsoft Word using the headings, questions, and format outlined below. The application, including the budget, should be no longer than six (6) typed, single-sided pages (3 doublesided pages) with 1-inch margins and 12-point type. Please number the pages. Proposals must be prepared based on the questions and format contained in the below 2015 Application Format. Application Deadline Submitting Your Application. Applications must be received electronically. There is no deadline for applications. Please send your application via e-mail in a Word attachment to grants@freshwaterfuture.org. Supporting documents may be sent via first class mail if necessary. Please do not fax your application. Sponsor Information (If Applicable) U.S. applicants that are not 501(c)(3) organizations must have a qualified sponsor serve as fiscal agent. See the 2015 RFP for details. If a sponsor is required, please include the same information for your sponsor that is requested for applicants in “A” above. C. Project Title Please limit your Project Title to 12 or fewer words. The Project Title should be descriptive of the goal you are trying to accomplish with your project. D. Total Amount of Project Request (up to $15,000 USD for Federal Project Support.) E. Abstract Provide a one-paragraph summary of the project that would make it easy for an outside reader to understand. Limit your summary to 100 words or less. F. Organizational Information Please answer the following questions about the applicant. 1. When was your group founded and what is your organizational mission? 2. What are your current programs, activities, and accomplishments? 3. What is your current annual budget and primary sources of income (by percentage, e.g., grants, membership, board gifts, etc.)? G. To help us better assess your capacity needs, please complete the following questions. 1. How will funding from HOW directly support your larger federal grant application or project? (If known, note from which agency and program you will be applying for funding.) 2. What, if any, additional needs do you have that would help make your efforts related to this proposal a success? (Examples could include: how to manage federal funds, planning assistance, fundraising assistance) H. Description of Project Please answer the following specific questions about your project. 1. Which of the following best describes the nature of your HOW project request? Facilitating collaborations for proposal/project development Building public support, buy-in and involvement Development of the project’s funding strategy Testing or monitoring Staff time for _______________________________________________ Grant writing and/or administration assistance Development of project benchmarks Restoration related study, research or monitoring Legal advice Interpretation of lab results or other testing Other ______________________________________________________ 2. Explain in 200 words or less the larger federal proposal or project you hope to pursue or be engaging with. 3. Describe your plan of work and how it supports your federal project/proposal in question 2, including specific tasks and outcomes that will be accomplished with HOW funding. 4. A. If you are requesting funding to hire a consultant, describe the specific plan of work the consultant will accomplish. Describe how this plan of work will assist with your federally funded project or proposal. B. Do you need assistance locating/selecting a consultant? 5. Provide a brief timeline for the proposed HOW project, including start dates, milestones, and expected completion date. I. Check the attributes of your project that match the selection criteria. If checked, please explain in 100 words or less. The federal proposal or project: Demonstrates activities that can help reduce impacts related to climate change, such as warming air and lake temperatures, heavier precipitation and drier droughts, and decreasing lake water levels. Is at or nearing implementation. Includes partners for capacity assistance. Includes multiple funding sources. Has community support. Identifies which GLRC Strategy goal, milestone, and/or recommendation the project seeks to implement. ( e.g., GLRC Strategy Aquatic Invasives, recommendation 3. Remove Phragmites. See http://greatlakesrestoration.us/priorities.html) Has parts supported by local, state, or federal policies or ordinances. Has parts that will result in short-term and long-term ecological benefits. Demonstrates economic and social benefits. Provides new or unique examples to help with future similar projects. Provides a plan for long-term maintenance of the restoration work. J. Proposed Budget Present the project budget using a table and descriptive summary categories, such as personnel, consultants, training, workshops, travel, professional consultant fees, legal fees, etc. K. Supporting Materials Submit the following supporting materials with your application: 1. 501(c)(3) confirmation letter from the IRS. If the applicant is not a 501(c)(3) organization, then submit the required charitable status documentation for the sponsor organization and a letter from the sponsor on their letterhead expressing support of your project and a willingness to serve as the applicant’s fiscal sponsor. 2. Limit other support materials (letters of support, newspaper articles, etc.) to two items that you feel are essential to evaluate your proposal. These are not a requirement. Note: Grant recipients may be required to provide Freshwater Future with additional information. L.
