FELINE SPAY & NEUTER AUTHORIZATION

2. I have withheld food from my cat for _______ hours.
3. ( ) Female cats: To the best of my knowledge my cat is NOT in heat
or Pregnant. If she is found to be pregnant, there will be an additional
charge of $40-$45.
4. If my cat has visible fleas I understand it will be treated with Capstar
(kills fleas immediately, but does not have a residual) at the additional
expense of $6.50

I authorize Ripon Veterinary Hospital to perform the above requested surgical procedure.
I understand there may be an additional charge at my expense if my cat if found to be pregnant.
While I expect all procedures to be done to the best of the abilities of the experienced staff, I
understand that no guarantee can ethically or professionally be made regarding the results.
I authorize the Hospital Director and her staff to provide Veterinary services required in an
emergency circumstance and to follow through with such procedures as are necessary for the well
being of my cat until I am reached for further consult and authorization.
I have read and fully understand the foregoing and understand payment is due when I
pick-up my cat.
