Instructions For Students
Here you will find the KIN forms you and your site supervisor will need to complete related to your
fieldwork. In addition, you will need to have your site sign a University – Organization Contract (UOC).
When all forms are complete turn them in to the KIN main office. The internship manager will review
your proposed fieldwork. Upon approval, Kinesiology staff will contact you with add code information.
KIN 170D, Fieldwork in Sport Management Registration Form: This is the form that is used to
obtain an add code so you can enroll in this course. You can type in or hand write the information
requested on this form.
• At the top, provide the contact information requested.
• In the middle section, provide confirmation that you meet and understand requirements by
checking the boxes provided. Also provide a brief description of what your fieldwork entails.
• Sign your name and date at the bottom
Fieldwork Specifications Form: This form constitutes the agreement between you and your site
supervisor with regard to learning outcomes and activities you’ll be engaged in. You can type in or hand
write the information requested on this form.
• At the top provide a start date, end date, and number of hours to be completed. Note: you cannot
technically do fieldwork outside the official dates of a given semester.
• Next, provide three learning outcomes. What gets put here is a description of what you will learn
as a result of the fieldwork experience. The preamble to each would be: at the completion of this
internship the student will be able to….
• Next, provide a brief description of the activities you will be doing - up to a maximum of 8
(minimum 4).
• At the bottom, your site supervisor and KIN supervisor prints and signs name, date, and title; you
sign and print your name and date.
Before turning in all forms, make 2 copies of this page – one for you and one to give to your site
supervisor.
Agreement Between Kinesiology and Fieldwork Site: This form provides guidance on what is
expected of each person: student, site supervisor, and KIN internship/fieldwork manager.
• You and your site supervisor sign at the bottom of the page.
University – Organization Contract (UOC): This document is the formal contract between SJSU and
the site where you are doing your fieldwork. You can obtain the UOC from the KIN internships web
page. SJSU has several contracts already in place. If you or your site supervisor believe a contract
already exists, contact Dr. Shifflett and she can check. Note, the required contract is fairly new so it is
unlikely a contract is already in place.
After you turn in all forms, the contract will then be signed by the SJSU contracts manager and Dr.
Shifflett will mail a copy to the person from your fieldwork site who signed the contract.
Kinesiology Fieldwork (KIN 170D)
KIN 170D, Fieldwork in Sport Management
Practical experience in a professional work setting while working and observing (50 hours per unit
registered) under direct supervision of qualified sport management professionals. Interns assigned a
variety of duties and experiences. Prerequisite: KIN 070, KIN 152, and upper division standing.
Note: You MUST do the fieldwork during the semester in which you are registered, and it must not
continue outside that time.
Course Objectives:
Fieldwork courses allow the student to develop an understanding and working knowledge of actual
operations, events, planning, and management within the field of Kinesiology. The fieldwork experience
is an essential component in a student's course of study designed to facilitate the integration of theoretical,
applied and academic subject matter in our field.
Grading:
Credit (CR)) or No Credit (NC) will be determined based on your site supervisor’s evaluation and any
other requirements specified by your Kinesiology fieldwork supervisor. It is imperative that both elements
be satisfactory to receive credit for fieldwork. The fieldwork hours must be completed in the semester
you are enrolled in KIN 170D - incomplete grades are not given.

Agreement Between Kinesiology and Fieldwork Site
Site Supervisor - Agrees to:
 Guide this student’s work and to submit a brief final evaluation of his/her performance upon
request.
 Discuss any concerns about the student’s performance with them directly and with the
Kinesiology fieldwork supervisor if necessary.
 Provide adequate information and training, including information about the organization’s
mission, clientele, and operational procedures.
 Provide appropriate recognition for the student’s contributions.
 Complete and return to the student all forms related to the fieldwork in a timely fashion so the
student can register for the fieldwork.
Kinesiology Internship/Fieldwork Manager - Agrees to:
 Orient students regarding the purposes of the fieldwork, its relationship to the program, the
importance of their commitment to the fieldwork site, and showing respect for all colleagues and
clients.
 Facilitate a good experience by providing an opportunity for students to reflect on their service
and integrate it with their program of study through a fieldwork report.
 Notify the student and/or the site supervisor of any concerns or changes related to the student’s
fieldwork.
 Provide guidance to students with respect to completion of their required fieldwork report.
Student - Agrees to:
 Act in a responsible manner while representing SJSU at the fieldwork site, and abide by all rules
and regulations that govern the site in which he/she has been placed.
 Fulfill the learning objectives.
 Participate in site orientation; abide by all guidelines and limitations related to the fieldwork.
 Complete all forms, evaluations or other paperwork required by either the course or the site
supervisor.
 Complete and submit to the internship/fieldwork manager the required fieldwork report by the
due date specified in the syllabus.
 Complete all fieldwork hours during the semester enrolled in KIN 170D.
