Checklist for Preparing the Individualized Studies Major Proposal Name: Date: Use this checklist to make sure you have fulfilled all of the proposal requirements before submitting it to your SDLR 550 instructor. The accuracy of this checklist will be worth 5 percent of your final course grade. (There is space allowed for questions or comments if there is anything you’re unsure of). To complete the form, tab to each check-box and left-click once. Use the tab key to access the comments fields as well. 1. The front cover is completed, including submission date and entire address. Comment : 2. The degree is identified as a BA or BS. Comment: 3. The statement of purpose has been completed. Comment: 4. Personal learning outcomes for the major are completed. Comment: 5. You have indicated whether or not your proposed major will meet graduate school prerequisites and/or a professional crediential. Comment: 6. The proposal constitutes a “substantial and coherent area of study.” Comment: 7. You have identified a goal for this area of study that is generally agreed-upon by colleges. Comment: 8. A minimum of 5 upper level learning experiences (3 to 4 credits each), including the upper level integrative experience. Comment: 9. You have identified the department and the title of your integrative capstone. Comment: 10. Total credit in the major: minimum of 32 credit hours (at least 15 at the upper level); however, a typical Individualized Studies major includes 36 to 44 credit hours (typically at least 10 courses).
