This scheme entitles ALL adults and children exempt from prescription charges, to FREE medications for a range of common, minor illness. TRAINING The practice is actively involved in the training of new doctors and at certain times we will have medical students that attend patient surgeries alongside our regular GPs. This means that from time to time you may be seen by a medical student in addition to your doctor. This is vital part of the training of future doctors and all student activity is supervised by your doctor. MEET SOME OF THE TEAM………. If you would prefer to see the doctor without a student present, please make your preferences known to the reception staff. You are completely free to make this request and it will not affect your treatment or care in any way. All patients are welcome, so do come along if you can and hear about what is happening within the surgery and the wider NHS. HOW TO REGISTER AS A PATIENT If you wish to register with the practice please complete the registration form and patient questionnaire enclosed within this pack and bring it to the surgery. You will also need to provide 2 forms of ID, for example, driving licence/passport and utility bill. ID is not required for children. (We ask for ID as this helps to ensure the safety of your medical information. For patients coming to us from outside the UK we would ask you to bring proof of citizenship (visa etc.) and proof of address. MAKING AN APPOINTMENT GP Appointments Most of our GP appointments are bookable on the same day with a small number being becoming available for pre-booking 3 days in advance. You can do this by: • Speaking to a member of staff at the reception desk. • Calling the surgery by telephone during working hours • Booking online at www.parksidemedicalpractice.co.uk (you will need to register for online services - see further information enclosed within this pack) Please note that when booking your appointment the receptionist may ask for some very brief details of your illness. We ask this as it helps the doctor, but if you prefer not to tell the receptionist that is perfectly acceptable. Telephone Consultations We also offer telephone consultations. If it would be more convenient for you to seek advice over the phone, please call the surgery and we will try to arrange a convenient time and date for the doctor to telephone you. Home Visits If you are too ill to attend the surgery, it may be possible to arrange a home visit for you. If you would like a home visit, please call the surgery to request one, before 11am. Please note that home visits will only be made to those who are genuinely housebound and too ill to attend the surgery in person. Again the receptionist may ask for some brief details for the reason for the appointment, this is because nurse appointments vary in length depending upon the nature of the appointment and it is important that we assign the correct length of time to your appointment. WHERE ELSE CAN I GO FOR HELP…? You don’t always need to see a GP; other services might be able to help What happens if you no longer need or can’t attend your appointment? If you no longer need, or can’t attend your appointment, we would ask that you contact us as soon as possible and let us know. This helps us to ensure that appointments are not wasted and we can offer those no longer required to someone else who genuinely needs it. If you have received an appointment reminder via text message to your mobile phone you can cancel it simply by replying ‘cancel’. Repeat medication can be requested in several different ways: Through your local pharmacy. Via our online prescription request service, available from our website please ask at reception if you would like to set up an account for our online services. By use of the drop off boxes at our reception desk and downstairs near the ask desk. By sending your request in by post – we will happily return your completed prescription to you if you include a self-addressed, stamped envelope. Requests for repeat medications will not be processed for items that aren’t due within the next 7 days. If you require additional quantities of your medication, or if you require your prescription earlier than its due date, please make sure you make a note of your request in writing, along with your repeats list, detailing your reasons. It is your responsibility to make sure you order your repeat medication before it runs out. Electronic Prescription Service We offer an Electronic Prescription Service – please speak to the pharmacy of your choice in the first instance about this service and how to nominate that pharmacy to receive your electronic prescription. BEHAVIOUR POLICY We aim to treat every patient with respect and in a polite and friendly manner. We ask patients to please treat the doctors and other staff at the practice in a similar way. The practice operates a ZERO TOLERANCE POLICY in regards to any threatening, abusive or violent behaviour, displayed against any of our staff or patients in any way. Any persons committing an act of violence or abuse against any members of staff, patient or visitor at the practice may be reported to the police. Where repeated incidences of unacceptable behaviour have occurred, we may exercise our right to have the offending patient removed from our patient register. TEMPORARY RESIDENTS If relatives or friends require medical attention while staying with you, we can treat them as a temporary resident. Please let the receptionist know when making an appointment. It will be necessary to complete a temporary resident form. SURGERY WAITING TIMES We make every effort to ensure that our surgeries run on time, but there will be occasions when an emergency arises or patients need longer than their allocated appointment time. Please be aware that in these situations and due to the nature of these incidents you may need to wait beyond your booked appointment time before you see the doctor. We ask for your patience and understanding. In order to help us ensure that surgeries do run on time please ask for a double appointment if you have more than one problem or you feel that your problem might take longer than usual to deal with. We would also ask that you arrive for your appointments on time and inform us if you no longer need your appointment so that we can offer it to someone else who really needs it. Document1 January 2016 FIT TO WORK CERTIFICATES (SICK NOTES) For the first 7 consecutive working days lost through illness you do not need a certificate from your doctor. An SC1 form covers this period and is available from your employer. You need to see a doctor if the absence is longer or if your employer requires a private certificate (for which there is a charge.) If your require an extension of a sickness certificate or for a duplicate to be made, please call the practice or ask at reception, as a telephone consultation with a doctor may need to be arranged. PATIENT CONFIDENTIALITY We respect your right to privacy and keep all your healthcare and personal information confidential and secure. This information is only available to those involved in your care. You have a right to know what information we hold about you. If you would like to view your records, please ask for more information at the reception desk or contact the Practice Manager. There may be a small charge, depending on the type of information service you require. To help us keep your records accurate and up to date, please inform us of any changes of address, name or telephone numbers as soon as they occur. PATIENTS WITH PARTICULAR NEEDS All our consulting rooms are accessible to patients using a wheelchair. In addition to this, we have a number of parking spaces for disabled patients. Lift access to the practice, based on the 1st floor of the Riverside Health Centre building, can be found near the main entrance. There is also disabled toilet access on both the ground floor and 1st floor. CHAPERONING This practice respects the cultural, personal and religious beliefs of our patients. Should you wish to have a chaperone present during your consultation please advise the clinician. INTERPRETING SERVICE We can book an interpreter for patients who first language is not English. Please advise the receptionist if this is required. NON NHS SERVICES Some services (e.g. HGV medicals) will incur a fee. See enclosed leaflet for more information.
