PET 2622 with a “B” or better
BSC 1085 with a “C” or better
2. Application for formal admission and progression through the Athletic Training
Education Program which includes the following documents:
Personal Data Information Sheet
Essay for Athletic Training
3 Letters of Recommendation
Physical Exam
Technical Standards Form
Fingerprint Identification
Hepatitis B Vaccination
Tuberculosis Skin Test
Proof of Completion of Bloodborne Pathogens Training
Fifty Observation Hours
Pre-Admission Evaluation Form
3. Current SASS Audit showing overall GPA of 2.50 or better
4. Current CPR/FA/AED Certification
In the event of the program enrollment exceeding the CAATE recommended ratio of
students to ACI’s, permission to enroll in the first Athletic Training Clinical (PET 3670)
will be based on the candidate’s ranking relative to the performance in criteria # 1 as well
as a review of the material in criteria # 2 by a admission committee comprised of HLES
faculty and the Athletic Training Staff. Ranking of the application candidates is
determined by the following:

Progression and retention through the Athletic Training Education Program
