Phase I 1. Student must complete the following courses: PET 2604 with a “B” or better PET 2622 with a “B” or better BSC 1085 with a “C” or better 2. Application for formal admission and progression through the Athletic Training Education Program which includes the following documents: Personal Data Information Sheet Essay for Athletic Training 3 Letters of Recommendation Physical Exam Technical Standards Form Fingerprint Identification Hepatitis B Vaccination Tuberculosis Skin Test Proof of Completion of Bloodborne Pathogens Training Fifty Observation Hours Pre-Admission Evaluation Form 3. Current SASS Audit showing overall GPA of 2.50 or better 4. Current CPR/FA/AED Certification In the event of the program enrollment exceeding the CAATE recommended ratio of students to ACI’s, permission to enroll in the first Athletic Training Clinical (PET 3670) will be based on the candidate’s ranking relative to the performance in criteria # 1 as well as a review of the material in criteria # 2 by a admission committee comprised of HLES faculty and the Athletic Training Staff. Ranking of the application candidates is determined by the following: Criteria Maximum points 1. Successful completion of all required documents20 2. Pre-Professional Evaluation 75 3. Actual Over-all GPA (ie 3.65 GPA = 3.65 points) 4 4. Grade in: BSC 1085 4 PET 2604 4 PET 2622 4 TOTAL 121 Point scale: A = 4.00, A- = 3.67, B+ = 3.33, B = 3.00, B- = 2.67, C+ = 2.33, C = 2.00 Phase II Progression and retention through the Athletic Training Education Program requirements are as follows: 1. Completion of the following courses with a grade of “B” or better: PET 3330 Functional Kinesiology PET 3351/L Exercise Physiology/Lab, PET 3660 Management Strategies in Athletic Training, PET 3670 Athletic Training Clinical I, PET 3671 Athletic Training Clinical II, PET3680 Protective Methods in Sports Medicine, PET 4310/L Mechanics of Human Motion/Lab, PET 4609 Evaluation Techniques of Athletic Injuries I, PET 4610 Evaluation Techniques of Athletic Injuries II, PET 4623/L Rehabilitation of Athletic Injuries/Lab, PET 4632/L Therapeutic Modalities in Athletic Training/Lab. 2. Completion of initial proficiency evaluations, 2. Cumulative GPA of 2.5 or higher, 3. CPR recertification. Phase III Requirements for completion of the Athletic Training Education Program are: 1. Completion of the following courses with a grade of “B” or better: APK 4312 Pharmacology in Athletic Training, PET 4361 Sport Nutrition and Weight Control, PET 4383C Physiological Basis of Strength Development, PET 4605 General Medical Conditions, PET 4621 Senior Seminar in Athletic Training PET 4672 Athletic Training Clinical III, PET 4673 Athletic Training Clinical IV, . 2. Successful re-evaluation of clinical proficiencies. 3. Completed SASS audit. 4. Application for graduation completed.
