I verify that I have achieved basic proficiency in the indicated foreign language (or
English, if I am not a native English speaker), and have provided evidence of this
proficiency to my Supervisor.
(Master’s Students only) I verify that I will be submitting the Methods Requirement
Form in lieu of the Foreign Language Requirement. (In this case, no signature from the
Supervisor is needed; submit this form directly to the Graduate Adviser).
(PhD students only). I request that this requirement be waived, and have asked my
Supervisor to provide a letter justifying this waiver. I understand the waiver is rarely
granted.

Supervisor’s signature. I have verified that the student has demonstrated basic
proficiency in the foreign language (or English, if the student is not a native English
speaker), through course work and/or testing of reading and speaking ability. If the
student is seeking a waiver, I have attached a letter justifying the waiver of this
requirement. In the letter I’ve provided reasons why the lack of foreign language skills
will not adversely affect the student’s ability to read the relevant literature, successfully
complete a dissertation, and achieve high standing in the profession in her/his sub
discipline. Please provide any comments or details on the back of this form.
