possibility of an accident and assume the risk and
responsibility while attending the MHS Tennis Summer
Camp. I, as parent/guardian of a minor camper, permit
emergency care to be administered to her/him as deemed
necessary by the Coach or Athletic Trainer. I will allow the
involved hospital and/or doctor to administer the required
treatment of the emergency conditions. I also understand
that all incurred costs are my personal responsibility and
that the MHS Tennis Summer Camp does not have
insurance coverage for injuries to sports participants.
