These practicals have been approved by the Biomedical Sciences, Dentistry, Medicine and Natural and Mathematical Sciences Research Ethics Subcommittee, King’s College London. For practicals involving human participants, Informed consent is a pre-requisite. In the experiments listed below you might also be the subject of the experiment (where a measurement such as blood pressure is made on you). Please take time to read the protocols and activities listed above carefully and discuss it with others if you wish. Ask your lead demonstrator if there is anything that is not clear or if you would like more information. You should only sign the consent form if you are happy to take part in these practical sessions. While participation is encouraged in order to facilitate your practical experience in this course, you are not obliged to take part and choosing not to take part will not disadvantage you in any way. You must sign off consent before volunteering to take part in the practical courses, however this can be signed prior to, and not necessarily on, the day of the practical. It is up to you to decide whether to take part or not. If you decide to take part you are still free to withdraw your consent at any time and without giving a reason. The College and its staff have the following responsibilities (where appropriate): to provide you with a safe working environment to carry out a risk assessment to obtain ethical approval to treat confidentially any information that you provide us concerning your ability to execute the practical safely to provide a member of the academic staff to supervise the practical. to provide prompt assistance in the event of an accident or adverse reaction Version 1 24/05/13 to treat any information relating to you personally as confidential. to monitor subject’s wellbeing during experiment to stop the procedure immediately should the subject indicate adverse symptoms (or request that the experiment be stopped) to give an adequate explanation of procedure to ensure appropriate relative positioning of subject to ensure appropriate hygiene to respect subject’s modesty and dignity to conduct assessment of outcome from both subject and experimenter to list exclusions specific to the particular practical Your responsibility (where appropriate) is: to read and comply with the Health and Safety advice given in section seven of the [Relevant School]Student Handbook. The information on liquid nitrogen, fume hoods and microbiological safety cabinets is not relevant to your first year. to read in advance the practical schedule, particularly the section on health and safety to abide by the general safety guidelines listed below, any particular safety guidelines in the practical schedule and any additional instructions given to you at the start of the practical to behave in such a manner that you do not knowingly endanger your health and safety or that of the other students and staff to seek clarification on any aspect relating to health and safety that you do not understand, particularly if you perceive it to be a risk to your safety or the safety of others. to accept that if you miss any safety instruction, we are entitled to exclude you from the practical to treat as confidential the results of any measurements made on your fellow students (see below) to report immediately to the staff any adverse sensations or reactions that you may experience during the course of an experiment. If your circumstances have changed such that one of the exclusion criteria for the practical now applies to you, you should inform the lead demonstrator.
