Executive Summary
According to the Centers for Disease Control and Prevention, immunization is the process by
which a person or animal becomes protected against a disease. Failure to immunize against disease
could lead to a recurrence of currently rare and sometimes deadly vaccine-preventable diseases. The
Utah Immunization Program seeks to promote a healthy lifestyle that emphasizes immunizations across
the lifespan by partnering with the 12 local health departments throughout the state and other
community partners. The mission of the Utah Department of Health Immunization Program is to
improve the health of Utah's citizens through vaccinations to reduce illness, disability, and death from
vaccine-preventable infections.1
This strategic plan was informed by town hall meetings conducted with five regional
immunization coalitions around the state and an online survey of health professionals statewide. The
plan is a dynamic document to help guide Immunization Program activities to achieve its goals in
accordance with its mission, values and vision.

Utah Immunization Program Plan
Context Statement
Utah has, in the past, experienced some of the nation’s lowest immunization coverage levels for children
19-35 months of age. Various improvement strategies have been implemented including regional child,
adult, and lifespan coalitions, expanded enrollment and enhancements in Utah Statewide Immunization
Information System (USIIS) and partnership for provider quality assurance with the AAP. Emphasis on
immunization by Utah Department of Health have increased awareness and improved immunization
levels among two-year-olds.
Utah has a very distinctive demographic profile. The population is younger, lives longer, has 52,164
births, a birth rate that is 50% higher than the U.S. and has more persons per household than the nation.
The Utah Immunization Program has goals and target measures based on Healthy People 2020. These
goals and target measures are specific to the unique demographic profile in this state and the Utah
Immunization Program works closely with private provider offices, local health departments, community
health centers, and other community partners to assure needs are being met through the goals and
objectives of the program.
Mission Statement
The mission of the Utah Department of Health Immunization Program is to improve the health of Utah's
citizens through vaccinations to reduce illness, disability, and death from vaccine-preventable infections.
We seek to promote a healthy lifestyle that emphasizes immunizations across the lifespan.
From providing educational materials for the general public and healthcare providers to assessing clinic
immunization records to collecting immunization data through online reporting systems, the Utah
Immunization Program recognizes the importance of immunizations as part of a well-balanced
healthcare approach.

Strategic Goals
Goal 1: Education and Awareness
Focus on provider and patient education regarding immunizations across the lifespan in order to increase
understanding of vaccine efficacy and importance. Decrease misinformation or lack of appropriate
information regarding vaccine safety. Increase the knowledge of vaccine preventable diseases, including
the effects, herd immunity, and cocooning strategies.
Goal 2: Access
Work to increase access to immunizations through strategies aligned with changes to the health care
system and insurance. Provide a means for stakeholders to understand how to overcome financial and
other barriers to immunizations across the lifespan. Work with partners to provide increased access for
the underinsured, uninsured adults and other populations with limited access.
Goal 3: Records
Increase health information exchange efforts to improve the completeness, accuracy, and use of USIIS
and its data. Outreach to providers currently not enrolled in USIIS to increase use. Improve ability to
transfer data between electronic medical records and USIIS system. Create the ability for individuals to
access USIIS records.
Goal 4: Policy
Evaluate existing policies and gaps on an ongoing basis in coordination with stakeholders to identify
needed recommendations or changes in current rules, guidelines, and education. Work with partners to
develop appropriate rules, enforcement strategies, and accurate reporting.
Goal 5: Services and Partnership
Increase area-specific support through local coalitions and partnerships. Collaborate with partners to
leverage resources and opportunities to improve service. Review and expand best practices to share with
providers to improve the quality of services.

Goal 2: Access

other barriers to immunizations across the lifespan. Work with partners to provide increased access for
the underinsured and other populations with limited access.
Strategy 1 – Educate providers regarding immunization barriers and solutions to immunization barriers.
Strategy 2 – Identify more school immunization clinic possibilities. Identify and provide local
immunization coalitions and local health departments with information regarding funding and
administration sources for clinics.
Strategy 3 – Increase immunization access for special populations through outreach to community health
centers, local health departments and others.

Goal 3: Records
Increase USIIS participation among Utah providers. Improve the effectiveness of participating
providers through new USIIS training options, and data quality and workflow consulting.
Increase the accuracy and completeness of USIIS data through electronic data interfaces with
Electronic Heath Record (EHR) systems used by Utah providers. Develop a new feature for
individual access to USIIS records.
Strategy 1 – Facilitate additional USIIS training options for participating providers.
Strategy 2 – Require USIIS user training before providing access.
Strategy 3 – Develop programming that allows patients to access their own records on USIIS. Explore
funding options to create a web application that allows users to look up their own immunization records
on USIIS.
Strategy 4 – Facilitate compatibility between Electronic Medical Record systems and USIIS.
Strategy 5 – Develop strategies to overcome FERPA school data exchange issues with partners such as
the Utah State Office of Education.

Goal 4: Policy
Evaluate existing policies and gaps on an ongoing basis in coordination with stakeholders to identify
needed recommendations or changes in current rules, guidelines, and education. Work with partners to
develop appropriate rules, enforcement strategies, and accurate reporting.
Strategy 1 – Improve communication and understanding between the Utah Immunization Program and
Department of Education regarding the purposes of policies and the importance of objectives such as
herd immunity.
Strategy 2 – Spread awareness of standard immunization requirement templates for healthcare, school
and daycare settings.
Strategy 3 – Identify inter-agency partners, including in fields such as mental health and long term care,
to work with populations impacted by immunization requirements.

Goal 5: Service and Partnerships
Increase area-specific support through local coalitions and partnerships. Collaborate with partners to
leverage resources and opportunities to improve service. Review and expand best practices to share with
providers to improve the quality of services.
Strategy 1 – Refine and strengthen the AFIX program to focus on feedback and follow up and to ensure it
is a continuous quality improvement program.
Strategy 2 – Encourage every provider office offering vaccines to identify an immunization champion to
ensure that each patient is offered appropriate vaccines at each visit.
Strategy 3 – Support and encourage providers in implementing systems that encourage immunizations
across the lifespan.
Strategy 4 – Seek out partnerships that encourage organizations to self-mandate vaccinations.
