People introduced themselves and explained why they were interested. n Ifrah Hassan is a community nurse in Hounslow she works with diverse ethnic group of patients who mainly have diabetes. Elaine Williams is a level three health champion; also she sat on the CCG’s Steering group for the new providers who commission the diabetes services in Hounslow. She is a member of Diabetes UK and attends their conferences/meetings. Amarjit Bahkshi volunteers within the community including being a member of the Patient Carers group. She is also a member of her practice Patient Participation Group (PPG) Calvin Archibald is a type 2 diabetic interested in what the group entails and is hoping to contribute. Nagaret Naidu is a border line diabetic her GP diagnosed her. She is a qualified nurse and she has been controlling her diet and lifestyle which has helped. Daya Deegodamana is Sri Lankan and has type2 diabetes. She is a former IT officer in the Sri Lankan High Commission for the United Kingdom (UK). She is also a carer for her partner and she was diagnosed in April 2015. 1 Prabha Vijayakumar is the manager for the new intermediate diabetes service CLCH. The service manages type 2 diabetes services. They also do some work around prevention and type1 diabetes. 2 Review around the Terms of Reference for the Group AA went through the terms of reference and explained that the group had been created to gather patient feedback and experiences of the diabetes services in Hounslow. Everyone who is a member of the group are either a type 2 diabetic, represent a group of people who are at high risk of diabetes or a carer for someone who has diabetes. (An example of a community representative is Mrs Bahkshi who is involved with the multicultural centre in the Gurdwara and other groups within Hounslow). The group will be interactive and group members should share experiences and make suggestions that can help Hounslow CCG better diabetes services in Hounslow. All group members agreed to meet up once every three months in a year All group members agreed that future meetings to be held at Heart of Hounslow Suggestions made by the group is to include looking into the connection between depression and diabetes Actions: 3 The next meeting in September AA will invite Dr Raquel Delgado the Diabetes Lead for Hounslow CCG AA will email/send out a copy of a journal/article to all members of the group Mental health and the link with Diabetes will be discussed at a future meeting (AA to contact Mental Health Lead and Commissioners). New Intermediate Diabetes Service in Hounslow- CLCH The new providers for Hounslow intermediate diabetes service (CLCH) explained their services. The team consists of consultants, diabetes specialist nurse, diabetes specialist dietician, psychologist, diabetes specialist paediatrics and diabetes specialist educators The target is helping patients to manage their type 2 diabetes effectively and the treatment plans are tailored to the individual. CLCH also provide a short programme called Holding Off Diabetes this programme is design to help patients to reduce the risk of developing diabetes. X-PERT is an award winning programme that meets the NICE guidelines for providing structured diabetes education for patients with type 2 diabetes. It is a 6 week programme that educates patients about diet, exercise, medicine and lifestyle choices. CLCH provides conversations maps which are designed to engage patients in an informative group discussion and help to improve diabetes self-management. There are 4 conversation maps and each session lasts for three hours. The difference between the X-Pert programme and the conversation maps is patients who are pushed for time and are very compliant and already know exactly what they need assistance with, the conversation maps is tailored to these patients more. All services provided are being funded by the Hounslow CCG has commissioned CLCH to provide these services in Hounslow. The service will be delivered for 5 years Actions PV will send copies of the CLCH leaflets to AA 2 4 PV will send Self-referral Forms for educational programmes to AA Overview of the Diabetes Public Meeting Report 18th March Question asked to the group 1. Why is diabetes a priority in Hounslow The group discussed and felt - The reason why diabetes is a priority in Hounslow is because we have a large population who are from different ethnic backgrounds especially in the South Asian and afroCaribbean population. These are the people that are at a higher risk Question asked to the groups 2. As a community and as individuals why do you think we have underperformed over the years in relation to diabetes care The group discussed and felt - The reason why is because some of the ethnic communities having English as a second language plays a part in understanding the impact diabetes has. Now that the communities in Hounslow are now aware of diabetes it is a good sign but still believes there should be more education provided in different languages. The group discussed and felt- Food, stress, physical activities have a role to play in people managing their diabetes if patients are educated properly and are implementing these factors they can properly manage their type2 diabetes. Actions 5 AA will bring in information around healthy walks and activities taking place in Hounslow Closing comments, agenda items and action points Next meeting will be held in September and invites will be emailed/ sent to all members of the group.
