2016 Scholarship Application Checklist
Please verify that you have included all necessary forms in your scholarship application packet by reviewing
the checklist below. Note: This is an OPTIONAL checklist that can be used to make sure you have included all
items in your packet. Please do NOT include the checklist in your scholarship application packet.
 Scholarship Application Form
 Copy of Academy of Nutrition and Dietetics (AND) membership card
 Letter describing short-term and long-range educational and career goals
 Faculty advisor’s signature (pg. 5)
 If pursuing a research project and applying for a scholarship to fund supplies, books, paper, computer
usage, or potential travel related to your research project; form below is included.
 Texas Academy of Nutrition and Dietetics Foundation Nutrition Research Scholarship (Helen
Lane) – Supplemental Form
 If conducting a nutrition education activity or participating in a research project or event related to
nutrition education and applying for a scholarship to fund tuition, educational materials, supplies,
books, registration fees, computer usage, or display materials; form below is included.

Elizabeth Watkins Cohen) – Supplemental Form
 Subject line of email reads as follows: Applicant Last Name, First Name, TANDF Scholarship
Application
 PDF document is titled as follows: Applicant Last Name, First Name, TANDF Appl
 (3) completed TANDF reference forms with a letter of reference; Reference will email both documents
to director@tandfscholarships.org
 Official transcripts from the TWO most recent universities or colleges attended with ACEND programs
(including official seal of institution), mailed to:

*Electronic official transcripts will also be accepted if this is required by your university. The electronic official transcripts must be
emailed to director@tandfscholarships.org.
All application materials, both emailed and mailed, must be postmarked no later than
December 15, 2015.
Incomplete applications will not be considered. Failure to follow directions will result in disqualification.
A confirmation email will be sent to you once the director receives your application materials.
