57 year old presenting to A&E department complaining of chest pain
PC
 Began 1 hour ago
 Has now improved but still have a dull ache in chest
 Central chest pain
o 8/10 at start
o 3/10 now
 Came on at rest
 Crushing in nature, dull ache also present
 Some tingling in jaw, now subsided
ICE
 Worried about the cat starving if staying in hospital, since wife/husband
left
 Expecting an ECG
PMH
 The patient denies any previous episodes of chest pain
 High blood pressure
 High cholesterol
DHx
 Takes tablets for hypertension and high cholesterol
FHx
 Father died aged 55 from an MI
 Brother died aged 59 from an MI
SHx
 Smokes 20 cigarettes a day for the last 30 years
 Drinks ‘
65 year old presenting to GP complaining of chest pain
PC
 Has had a few episodes in the past
 Central chest pain
o 7/10 at worst
 Came on recently while mowing the grass
o Always with exertion – stairs etc.
 ‘Tight’ pain
 Lasts less than 15 mins
 Relieved by rest
ICE
 Worried it might lead to a heart attack
PMH
 High blood pressure
 High cholesterol
 ‘Achey knee’
DHx
 Takes tablets for hypertension and high cholesterol
 Takes ibuprofen for the knee
FHx

SHx
 Smokes 20 cigarettes a day for the last 30 years
 Little alcohol
