Student Academic Success Plan: Part 1 - Intake
By creating a Student Academic Success Plan in conjunction with your success coach, you will map out
your goals and your plans for achieving these goals. The plan is designed to help you complete a
successful semester and collegiate career. The Academic Success Plan is divided into two parts.
Part 1 is an intake document to help you identify your strengths, study habits, weaknesses or areas of
concern, and support systems. Your Success Coach will use this information to help you develop your
plan of action (Part 2 of the Student Academic Success Plan).
Part 2 is your plan of action created in conjunction with your Success Coach – your goals for the
semester; strategies to implement and achieve the goals; and resources available at UT, in the
community, and online to help you achieve your goals.
Your Academic Success Plan Involves:
□ Determining why you are in college, your experience with academics, and the resources
available to you;
□ Completing your success plan and worksheets with complete honesty;
□ Meeting with your UT Success Coach at least once every other week;
□ Meeting with each of your professors on a regular basis;
□ Developing concrete goals related to your academic success at UT
