Please provide the following, in addition to the information listed in the “General
Assistance Guidelines” (see reverse side).
1) For New Tenancy of 3 Months or Less:
Receipt for Security Deposit (paid in full) o Positive Notice from Forward Service Corporation, if applicable.
Housing Subsidy Benefit Letter , if applicable, and if verifiable income alone is not sufficient to pay rent.
2) Signed Lease Agreement or Current WPS Bill.
3) Referral from Community Agency Caseworker, if applicable o (Residency verification letter is not sufficient)
Additionally, please note:
Household income must be sufficient to cover all housing expenses OR verification of any Agency Assistance and/or Housing Subsidy, if applicable, and if income is not sufficient to pay rent.
SVdP may provide necessity items, with the focus on “need vs want,” to qualifying households once in a 2 year period, or at the discretion of the Conference.
The level of assistance will be determined on a case-by-case basis.
SVdP reserves the right to deny assistance for reasons other than those stated in the
“Furniture, Household Items, and Linens Assistance Guidelines”.
