Changes to Forms Due Phase II
We are now pleased to announce Phase II of our plan to help reduce the data reporting
burden to centers. This phase focuses on replacing legacy comprehensive report forms
(CRFs) due in the past with the less comprehensive Transplant Essential Data (TED)
forms.
Specifically:
1) For those recipients transplanted on or before December 31, 2008 where the
Baseline form (Form 2000) and all follow-up forms are still due, all CRFs will be
replaced with TED forms.
2) For those recipients transplanted on or before December 3, 2007 where the
Baseline form (Form 2000) is completed but the 100-Day form (Form 2100) and
all other follow-up forms are still due, the outstanding CRFs will be replaced with
TED forms.
a. For option 1 and 2, if more than one Form 2200 (six-month, one-year and
two-year post-transplant) is due, only one Post TED (Form 2450) will be
made due. All events that occurred during the six-month, one-year and
two-year follow-up time points will be reported in aggregate on one Post TED form (Form 2450)
b. If multiple 2300s are due, they will be replaced with the Post-TED (Form
2450) for the most current time interval due. All events that occurred at
the three year and beyond follow-up visits will be reported in aggregate on
one Post-TED (Form 2450).
3) There are exceptions to the above where CRFs will still be due. These exceptions
are 1) the recipient was participating in a BMT CTN, KGF (including recipients
selected for controls) or Otsuka study or 2) the recipient was transplanted for an
autoimmune disease, immunodeficiency or inherited metabolic disorder or 3) the
recipient is less than 18 years old and transplanted for AML or ALL.
These changes were made to your Forms Due Report on April 19, 2011. Again thank you
