UTSI STUDENT LAB USE AGREEMENT
1.
The term “Lab” used in this document refers to the UTSI Student Lab located in building 8122. The Lab is
provided for part & full-time students to have a work area that allows students the ability to work on class
projects, conduct research associated with their thesis/dissertation, and to work on “hobby” type projects.
a. The research conducted is restricted to the limitations of the workspace provided.
b. No commercial entities and/or projects are allowed.
c. Only part & full-time students are allowed access to the Lab, provided training requirements are met.
2. The location of the Lab is in building 8122 also known as the “Blue Building.” The Lab will consist of office
spaces 119 – 123. See attached diagram.
3. Materials, Equipment/Tools, Chemicals
a. Primary construction materials used in the Lab will be wood, plastic(s), foam and other similar materials.
b. Equipment/Tools
1) Authorized equipment/tools used in the Lab will consist of hand tools, hand held power tools and
table mounted tools approved by the Lab Responsible Officer.
2) Heavy and/or industrial type equipment will not be authorized in the Lab.
3) The tools and equipment used in the Lab will be purchased, owned and maintained by the SGA.
4) The use of UTSI equipment / tools could be utilized as necessary. Coordinated requests through the
Lab Responsible Officer.
c. Chemicals
1) The use of chemicals in the Lab requires written consent by the Lab Responsible Officer. The
information in the consent will include the name of the chemical, quantity of chemical, and reason
for use. General cleaning agents do not require prior approval.
2) The UTSI Safety Office will supply an empty MSDS folder to the Lab to maintain the data sheets
related to the chemicals used in the Lab. Students must maintain the MSDS folder with current
information and comply with the campus MSDS program.
3) Disposal of equipment, metals, wiring, chemicals, etc., is not authorized without the consent and
knowledge of the Lab Responsible Officer.
4. Lab Access
a. The Lab will be accessible to students 24/7.
b. Only four (4) keys will be issued to the SGA for access to the lab.
1) It is the responsibility of the SGA to establish a procedure for accountability of the keys.
2) Keys can only be issued to students that have completed the training requirements and have signed
the Use Agreement.
c. A minimum of two (2) students must be in the Lab at all times when occupied.
d. A sign in/out log will be maintained to document student, date and time. The logbook will be utilized to
verify item 4(c) is being met and to track usage and responsibility.
5. Students are responsible for maintaining the cleanliness of the Lab.
6. The Lab will be subject to periodic safety inspections. Students will be expected to maintain and/or correct
any issues related to safety that is within their scope of responsibility.
7. Students are to immediately report any safety issue to the Safety Office so that the issue can be addressed
in a timely manner.

8. Injuries occurring in the Lab are to be reported to the UTSI Safety Office at extension 37402 or 37208.
9. Personal Protective Equipment (PPE)
a. Students working in the Lab are required to wear PPE consistent with the safety needs of their work.
b. The UTSI Safety Office will supply and ensure appropriate PPE is available for student use.
c. PPE issued to the Lab will remain the property of UTSI.
d. Students are responsible for the proper use, care, storage of PPE.
e. Report broken/damaged PPE to the UTSI Safety Office.
10. Training. Access to the Student Lab will not be authorized until all training requirements are met.
a. Students need to coordinate training through the UTSI Training Officer at extension 37208 or by email to
carmstro@utsi.edu.
b. The current list of training requirements are as follows:
1) PPE: Personal Protective Equipment
2) Hazard Communication: An Employee’s Right to Know
3) Protection From Occupational Noise
4) Fire Safety & Prevention
5) Workplace Safety Orientation
c. The above list of training requirements may be modified as needed.
11. Violations of this Use Agreement could result in the loss of use privileges or the closing of the Lab.
12. UTSI shall not be liable for any damage or loss of property to persons, including death; or other injuries or
damages to the Lab. Students shall indemnify, defend, and hold harmless UTSI from any and all demands,
claims, suits, actions, or liabilities for personal injury, including death; property damage, or other injuries or
damages that occur in the Student Lab.
