2016 ELEMENTARY SCHOOL PTO ALUMNI SCHOLARSHIP SUPPLEMENTAL FORM Student must have attended the elementary school for at least two years Students can apply to only ONE elementary school for the scholarship (i.e. Parkview or Meadows, not both) Each recipient will receive a $500.00 scholarship. All scholarship recipients will be recognized at Senior Honors Night. All recipients will be required to present their essay at their elementary school to K-4 students. Habit Seven: Sharpen the Saw I take care of my body by eating right, exercising, and getting sleep. I spend time with family and friends. I learn in lots of ways and lots of places. 3
